Management of Gastroesphageal Reflux Disease (GERD)

GERD is a digestive disorder that affects the lower esophageal sphincter, which is the ring of muscle between the esophagus and stomach. Gastroesophageal reflux is the backward flow of acids from the stomach into the esophagus. Doctors believe that some people usually suffer due to a condition called hiatal hernia. Pregnant women usually suffer from heartburn or acid indigestion caused by Gastroesophageal Reflux Disease.

The primary goals in the treatment of this disease are relief from symptoms, healing of erosive esophagitis and prevention of complications of esophagitis. The symptoms include heartburn and regurgitation. The setting includes a proton pump inhibitor (PPI). Patients suspected due to GERD with non-cardiac chest pain are recommended to have diagnostic evaluation before institution of therapy. For diagnosing GERD, barium radiographs should not be used.

The other points that must be remembered to diagnose GERD are
1. Endoscopy is recommended only in case of alarming symptoms. Even in typical symptoms, the upper endoscopy is not recommended.
2. For preoperative evaluation, esophageal manometry is recommended, but it has no role in diagnosing GERD.
3. Ambulatory reflux monitoring is a test that can assess association of reflux symptoms.

Management:
The suggested lifestyle modifications for the patients with GERD are

  1. Weight loss is recommended for patients who are overweight. So, avoid large meals and decrease dietary fat intake.
  2. Avoid alcohol, chocolate, onions, garlic, acidic foods and peppermint.
  3. Do not lie down within 3 to 4 hours after meal (recommended for nocturnal GERD patients).
  4. For symptom relief and to heal erosive esophagitis, a 8-week course of PPIs is the therapy of choice.
  5. Medications that help in potentiating GERD symptoms, including alpha-adrenergic agonists, nitrates, calcium channel blockers and theophylline should be avoided.
  6. For patients without diagnostic evaluation, therapy for GERD other than acid suppression, including prokinetic therapy should not be used.
  7. For patients with partial response to PPI therapy, additional symptom relief can be achieved by increasing the dose twice daily or switching to a different PPI.
  8. It is recommended to elevate the head of the bed 10 to 20 cm.
  9. Avoid wearing clothes which are tight around the waist.
  10. If patients experience heartburn relief, Histamine-receptor antagonists (H2RA) therapy can be used as a maintenance option in patients without erosive disease.

Some surgical options
1. For long-term therapy for patients suffering with GERD, surgical therapy is the best treatment option.
2. In patients who have no evidence of erosive esophagitis, preoperative ambulatory pH monitoring in mandatory.
3. Gastric bypass is the preferred operation for patients who are suffering with obesity and are contemplating surgical therapy for GERD.

Apart from these, there are potential risks that are associated with PPIs such as switching them can be considered in setting of side effects and the therapy can be a risk factor for Clostridium difficile infection.

Prostatitis treatments vary depending on the underlying cause. They can include: Antibiotics. This is the most commonly prescribed treatment for prostatitis. Your doctor will base the choice of medication on the type of bacteria that may be causing your infection. If you have severe symptoms, you may need intravenous (IV) antibiotics. You'll likely need to take oral antibiotics for four to six weeks but may need longer treatment for chronic or recurring prostatitis. Alpha blockers. These medications help relax the bladder neck and the muscle fibers where your prostate joins your bladder. This treatment may lessen symptoms, such as painful urination. Anti-inflammatory agents. Nonsteroidal anti-inflammatory drugs (NSAIDs) may make you more comfortable. Prostate massage. This is done by your physician using a lubricated, gloved finger — a procedure similar to a digital rectal exam. It may provide some symptom relief, but doctors disagree about how effective it is. Other treatments. Other potential treatments for prostatitis are being studied. These treatments include heat therapy with a microwave device and drugs based on certain plant extracts

From last 15 days I am facing problems while passing stool .in morning after 2-3 droppings suddenly blood starts dropping from anus. Its not painful while passing stool btu while washing its causing slight burning sensation om inner side .and when I stretch the skin little bit and try to clean anal area the burning sensation increases .i have stopped eating non veg, oily and spicy food from last 15 days and intermediately using polurite cream as recommended by my family doctor. Even after following this the blood is dropping randomly after 2-3 day. Please suggest if it s a serious issue and what can be done to cure it.

Your are having fissure in anus with probably internal hemorrhoids your blood sugar and blood pressure should be in control since both will effect high bp will cause bleeding and blood sugar will effect in healing fissure though you need proctoscopic examination start tab rakat sthambak vati 1 twice a day cap sunarin 2 twice a day sunarin ointment for loacl application tab triphala 2 at night with warm water.

It is advisable to apply a good sunscreen with SPF 25-30 or covered the exposed areas whenever you get out in the sun. Being a homoeopath,I will suggest you a natural way to glow your lips along with a lip balm. Lemon, Sugar Lip Scrub: In a small bowl, mix in 1 teaspoon of freshly squeezed lemon juice and coconut oil. Add in and stir thoroughly 2 teaspoons of granulated sugar, a bit at a time to make thick paste of uniform consistency. If needed, you can add in more sugar. After mixing it well, you can transfer the scrub into a suitable storage container. It can be safely kept in fridge for about one week.Apply a little bit of the scrub to your lips, and scrub it in circular motion. To effectively exfoliate your lips, use a small toothbrush. Hi. You can use clindac foaming face wash or fash face wash which ever available for acne can use deriva cms gel alternate night (mon wed fri) and for scars use melaglow rich cream alt night (tues thur sat)
