What Causes a Uterine Prolapse?

The uterus is a muscular structure held in place inside your pelvis with the help of muscles, ligaments, and tissues. These muscles weaken in women due to pregnancy, childbirth or delivery complications and can lead to severe complications. One such complication is a uterine prolapse. Uterine prolapse occurs when the uterus sags or slips from its normal position into the vaginal canal.

The causes of uterine prolapse are varied and include:

- Delivering a large baby
- Pregnancy
- Difficulty in labor and delivery
- Reduction in estrogen levels post menopause
- Traumatic childbirth
- Loss or weakening of the pelvic muscle
- Conditions which lead to increased pressure in the abdominal area such as a chronic cough, straining, pelvic tumors or accumulation of fluid in the abdomen
- Loss of external support due to major surgery in pelvic area

Uterine prolapse can be complete or incomplete depending on how far the uterus sags into the vagina. Women who have minor uterine prolapse may not have any visible symptoms. However, if the condition worsens, it manifests itself in visible signs.

Symptoms of moderate or severe prolapse are:

1. A feeling of fullness or pressure in your pelvis when you sit
2. Seeing the uterus or cervix coming out of the vagina
3. Vaginal bleeding or increased discharge
4. Painful sexual intercourse
5. Recurrent bladder infections
6. Continuing back pain with difficulty in walking, urinating and moving your bowels

Without proper attention, the condition can cause impairments in the bowel, and can also affect bladder and sexual function.

diabetes of your husband will not affect pregnancy outcome. get shirodkar stitch around the mouth of uterus around 12 weeks and your doctor will guide you. stitch is given under general anesthesia under most sterile conditions guided by ultrasound. stitch is removed when labour pain start and outcome is good.good luck to you to get a full term baby next time.

In traditional surrogacy, the surrogate is pregnant with her own biological child, but this child will be raised by others. Once a suitable surrogate has been identified, and the screening process is complete, the cycle can begin. Timing depends on the surrogate’s and intended parents/donors menstrual cycle .

The surrogate needs to prepare her uterus for implantation with natural estrogen and progesterone. Because each woman is a little different, the dose, duration, and method of administering these hormones may need to be individualized. This can be determined ahead of time by conducting an evaluation cycle. This is a “dry run” in which we duplicate each part of the cycle except the actual transfer of embryos in order to determine how to maximize the chances of success. The evaluation cycle can be completed anytime before the actual procedure. In some circumstances, the evaluation cycle can be waived when the response of the uterus to hormonal stimulation is well known. This is fairly common for women who have undergone many treatment cycles in the past.

it happens often in twin pregnancy that one of the twin does not grow and this seems to be happening in your case. the medicines are being given for growth of the second twin. It should help and a sonography after 2 weeks would give a fair idea about the same

She has irregular periods. I advise her to do meditation for mental strength by doing slow and deep breathing relaxation exercises in the morning. Take proper balanced vegetarian diet. Take lots of water and fluids. Avoid garlic and red chilly and non veg and eggs. Follow this and give feedback.

Depression is considered the common cold of all mental health issues and can strike in varying levels due to causes like pressing circumstances, problems in one's personal life and relationships, hormonal changes and other long term, chronic health conditions. Your intimate relationships are also subject to dejection when physical ailments enter the picture. Erectile dysfunction is one such area that creates depression. This is basically a condition that men suffer from. Due to this condition, a man is not able to get and hold an erection long enough to enjoy the act of sex. This can be caused due to related physical problems as well as stress. It can also lead to depression and problems in one's relationship due to anger, frustration, hopelessness and sadness. Here are a few ways to deal with depression caused due to erectile dysfunction.

  1. Recognising the Symptoms: The very first thing to do is to come out of denial and recognise the symptoms that are caused by this kind of depression. Usually, any long term or chronic problem can lead a person to look at himself or herself as a hopeless case. This can cause symptoms like dejection, a negative attitude towards everything and sudden outbursts of anger and frustration which may crop up in other areas of the patient's life.   3. Psychotherapy: One of the foremost methods used by most therapists includes psychotherapy that can help in unravelling the cause and finding a way to cope with the problem in a way that is focussed on discussions.
  4. Cognitive Behavioural Therapy: This form of therapy examines all aspects of the person's life and reality so as to find the cause of the problematic behaviour and reprogram the brain of patient with great deliberation.
  5. Antidepressants: While some medications can affect sexual functioning, there are antidepressants that do not have major sexual side effects. Please consult a Psychiatrist and discuss about this option too.

Tackling depression and erectile dysfunction is a matter of striking a fine balance with professional help.

Placental Insufficiency -  Know the Facts!

The placenta is a link between you and your baby during pregnancy. It helps your baby to grow normally by providing it with food and oxygen. When the placenta is not able to do its work properly, the condition is called placental insufficiency or dysfunction.

Placental insufficiency is a serious complication of pregnancy and can happen when the placenta does not develop properly or is damaged. If this condition develops, the baby doesn’t get the nutrition and oxygen it needs to grow and can develop complications like low birth weight, premature birth, and birth defects. The mother can also develop dangerous complications if it’s left undiagnosed. Thus placental insufficiency must be diagnosed as early into the pregnancy to avoid these complications.

Causes of placental insufficiency
Actually, placental insufficiency is a blood flow disorder which happens due to a reduction in the mother’s blood supply. It can be triggered by vascular disorders, medications, lifestyle and  the following:         

  1. Diabetes
  2. Chronic hypertension
  3. Blood clotting disorders
  4. Serious anemia
  5. Blood thinners
  6. Smoking
  7. Drug abuse 

Other causes of placental insufficiency are cases where the placenta doesn’t attach properly to the uterine wall or if the placenta breaks away from it.

Symptoms
Unfortunately, there are no symptoms of this dangerous condition in the mother except that she may notice that the size of her uterus is smaller than in previous pregnancies.  She may also notice that the fetus is moving less than before. As far as symptoms of placental abruption or disruption go, there may be vaginal bleeding or pre-term labour contractions. In case placental insufficiency is not diagnosed, the mother can experience a serious fatal condition called Preeclampsia, which means high BP and protein in the urine as well as disruption of the placenta and pre-term birth. The picture for the baby is pretty dim too. The earlier placental insufficiency happens, the more grave the risk for the baby. The baby can suffer from:

  1. Cerebral palsy and learning disabilities
  2. Low body temperature 
  3. Low blood sugar 
  4. Premature birth
  5. Cesarean delivery
  6. Stillbirth
  7. Death

Placental insufficiency can’t be cured, but it can be managed. Once diagnosed, it can be managed through proper prenatal care like treating maternal high blood pressure or diabetes, education on preeclampsia and ways to self-monitor for the disease, frequent doctor visits, bed rest to conserve energy for the baby and consultation with a specialist. These can improve the baby’s chances of normal growth. 

Proper prenatal care of pregnant women is thus, very important as it can diagnose placental insufficiency early. A simple ultrasound can diagnose this dangerous condition by measuring the baby's size and growth, and assessing the size and placement of the placenta. I want to know after embryo transfer how many progesterone injections are needed after embryo transplant? Because my wife is perfectly alrite.

She would still require progesterone ( tablets or injection - in one of these forms) everyday. Because there's no corpus luteum to support pregnancy in case of IVF. So take whatever your doctor has prescribed
