If you have felt a nodule in the breast, it is better to show it to a Breast Surgeons a Surgical oncologist. They would take a detailed history and examine you and if need be, ask for an ultrasound examination. Fibrocystic changes are very common and are common cause of breast pain especially cyclical. Some times some of the cysts may enlarge causing a lot of pain, which need to be aspirated. Also they need to rule out other causes of pain like ill fitting bra, trauma, abscess amongst a few. Spontaneous resolution is seen in around 20-50 % patients and may sometimes be related to pregnancy or menopause. Evening Primrose oil (EPO) capsules contains Gamolenic acid (GLA, which is an essential fatty acid needed to regulate the tissue response to hormones. The dosage required is 240-360 mg of GLA daily (2000-3000 mg EPO. The capsule contains 40 or 80 mg GLA. Thus the no of capsules can range from 4-8 per day taken orally. Few patients (<5%) report nausea, loose motions or headaches. It is to be avoided if you are pregnant, planning to conceive or have a history of epilepsy. You need to continue the primrose oil for at least 2-3 months to see any improvement in symptoms. If symptoms improve then you can halve the dose of primrose oil and continue for further 3 months. Some people are able to stop the capsules altogether without return of problem. Vitamin E 800 to 1200 I.U. (2-3 capsules) Daily also has been shown to work in cyclical mastalgia. The other treatment options are NSAIDS like Combiflam or enzoflam which provide symptomatic relief till the pain settles spontaneously and Antiandrogens like Danazole and anti estrogens i.e. Serm like Tamoxifen. The last 2 are quite effective but come with a lot of side effects. If you still have any queries, you may contact me directly.

AMH low means there is very low ovarian reserve. We can try some drugs considering your younger age. But most likely, you may need ovum donation. So, don't delay your treatment.

Drinking Water Empty Stomach - What Happens When You Do It?

Imagine water flowing through a pipe or a tube that has a lot of things passing through it. It would definitely have a cleansing effect, don't you think? Now, apply the same to your stomach which is also a tube through which all the food passes through. When you drink water, it runs through this tube and cleanses it of all the food debris that is along the walls. This helps in a big way by removing the debris and toxins and is one of the most effective cleansing mechanisms.

The Japanese, in particular, are known to have used this over the centuries. They drink neither cold nor hot water, but water at moderate temperature. If you incorporate the same into your daily lifestyle, it can do wonders.

Here are some other aspects of drinking water empty stomach that you might want to consider:

  1. Drink about 600 mL water on an empty stomach in the morning, even before brushing
  2. You can then brush and rinse your mouth, but do not eat anything for the next 45 minutes
  3. Eat a good breakfast after 60 to 90 minutes
  4. Again, do not eat anything after that for about 2 hours

This is a regimen which should be followed religiously as it comes with multiple benefits. The same have been listed below:

  6. Weight loss: If you need something to fill your stomach without having to add calories, drink up water. Doing this on an empty stomach is even more effective, as it also reduces bloating feeling and clears out debris and toxins. Water also improves metabolism and so helps in weight loss. Mixing lemon and honey to luke warm water and drinking it on an empty stomach can help reduce the waistline and keep you energized throughout the day.

Severe pain during intercourse causes difficulty in a relationship in both partners. Sometimes a sustained occurrence of pain might be an indication of some serious medical dysfunction. As such, consult a doctor as soon as the pain arises. Contrary to popular beliefs, sexual pain can occur in both male and female.

In female, the reasons behind the painful sexual intercourse might be:

  1. Vaginismus, a condition when the vaginal muscles tighten up mainly due to anxiety or fear and causes significant pain with a tearing sensation.

  2. The pain can arise due to vaginal infection due to yeast or any form of sexually transmitted disease or pelvic inflammatory diseases.

  3. A severe injury in the vagina during childbirth or any other accident and psychological stress can also cause severe pain during sex.

  4. Sometimes, uterine or menopause problems (formation of fibroids in the uterus) can also cause pain.

  5. Cyst formation in the ovary or endometriosis (when the inner tissue lining of the uterus tends to grow outside) can also cause severe pain during intercourse.

Among men:

  1. Some men can experience pain during intercourse because of allergy to the fluids of the vagina or the chemical content of certain contraceptives.

  2. You can experience pain because of undiagnosed sexually transmitted diseases like herpes or gonorrhea

  3. The pain might also occur due to urinary tract infections or side effects from certain medications.

  4. Severe pain just a few moments before ejaculation can be a result of inflammation of the prostate glands and should be immediately diagnosed as it might be a signal of prostate cancer.

For treating pain during sex in female, you should opt for the following procedures

  1. If the pain is caused due to vaginal dryness after menopause, you should consider the application of prescribed lubricants or jellies to reduce the pain.

  2. If the pain occurs due to severe anxiety or stress, sex therapy can help you improve physical intimacy.

  3. If there are other symptoms like severe bleeding or irregular periods in addition to the pain, a visit to the physician is recommended.

Quite similarly in men:

A novel study from Karolinska Institutet and University of Michigan in the U.S. shows that gaining weight from one pregnancy to the next can increase the risk that women will face stillbirth or lose their second babies within the first year of life.


SWEDEN : A novel study from Karolinska Institutet and University of Michigan in the U.S. shows that gaining weight from one pregnancy to the next can increase the risk that women will face stillbirth or lose their second babies within the first year of life. The findings are being published in The Lancet and build on data from more than 450,000 women in Sweden.

"These tragic events are still very rare among infants of mothers with high weight gain. However, as many women gain weight between pregnancies, our results are very important from a public health perspective", says principal investigator Sven Cnattingius, professor of reproductive epidemiology at Karolinska Institutet.

In their study, researchers reviewed data from the first two pregnancies of nearly 457,000 women who gave birth in Sweden from 1992 to 2012. The women's information was recorded in the Swedish Medical Birth Register, which since 1973 has collected information on about 98 percent of all births in that country. Weight was assessed at the beginning of each pregnancy. The results show that stillbirth risk rose with larger gains in body mass index (BMI) from first to second pregnancy. Compared with women who kept their weight, women whose BMI increased more than four units had a 50 percent increased risk of stillbirth.

Among women of normal weight in first pregnancy, high weight gain also increased the risk of infant mortality: when their BMI increased by 4 units or more, risk of infant mortality increased by 60 percent. On the other hand, the results also point to the opposite situation. Women who were overweight by their first pregnancy, defined as a BMI of 25 or more (corresponding to at least 70 kg of women with average height), but who lost weight before the second pregnancy, reduced their risk of infant mortality.

Every fifth women in the study material gained so much weight that it influenced risks of stillbirth and infant mortality (i.e. at least 2 BMI units, corresponding to 5.5 kg). However, the researchers point out that stillbirth and infant mortality are very rare events in Sweden, and only 2.4 per 1000 births resulted in a stillbirth and 2.1/1000 in infant mortality. There are annually around 100,000 births in Sweden.

"Previously, we have published that risks of stillbirth, infant mortality and morbidity increase with maternal weight, and in this new study we find that find that weight gain influence mortality risks. Taken together, our results support the conclusion that mother's weight per see may influence infant chances of survival", says Dr Cnattingius. "Still, the explanation for the findings is still speculative. We cannot differentiate from the data whether it is the weight gain during the pregnancy or in between pregnancies that is of significance".
