My 27 years old sister is schizophrenia patient. She is under homeopathy treatment. She has developed split personality. One is good and another is extremely bad. Bad personality abuses people, some time she attacks people with knife, she fights like anything and the good personality has that conscious mind that she is doing wrong things. She has the hallucinations where she can see someone is commanding her to do nasty things. Some time she becomes so violent that we are unable to handle her. Till at the age of 11-12 she was normal. After that she stopped communicating with world. Sometime she is very depressive and wanted to commit suicide. We are unable to understand how to deal with it and what are the treatments.

Dear lybrate-user The symptoms mentioned by you in your sister at present is suggestible of either schizoaffective disorder or mood related but with psychosis. I really cannot diagnose it competely till we meet the patient. If violence, homicidal tendencies increases or is unbearable, please consider admitting her in the hospital. First line of treatment would be pharmacotherapy which would have monitored dosage of anti psychotics and a mood stabilizer based on diagnosis. Once symptoms reduces, if the psychiatrist finds it important for psychotherapeutic intervention they would advise for the same bit for now kindly go ahead with admission and pharmacotherapy in your city. I am 24 years old hindu girl and I have met a muslim guy online 2 years back and started dating, it was long distance though. We still didn't meet, we always plan but somehow it gets cancelled. I am confused regarding this as my parents will never agree to this and I never want to hurt them. But on the other hand he is very kind and sensitive and I love him also very much. please help me out to get out of my mind clutter. Dear lybrate-user I understand what you must be going through in your mind but because of our emotion focused dealing is what is being utilized while resolving this problem, you are getting entangled more and more which makes us realize we might have to change our dealing strategy and have to give equal importance to rationality too. This is an age old conflict of mind over mood or mind over heart. Kindly pick up a sheet of paper amd try to write all the options you are feeling the conflict within and then carrfully jot down your personal reasons of being in each of the option, then familial reasons and quickly weigh the short term and long term pros and cons, potential hurdles of obstacles in each option and the back up you have for each miniscule problem you identify. Rational mind and heart goes with where there is more benefit. Try doing this exercise. And do get back if you need further assistance via a personal consult or video consultation. Please suggest foods that one must have to prevent or eliminate neurological problems in middle age or senior age. Also my mother is 68 years old and is suffering from ring from dementia. She is forgetting small small things. How can it be cured? Please suggest tests that need to be done and foods, medicines that she should take.

Dear lybrate-user ji Vitamin B12 are considered to be good supplements for nerve related issues and keep your mother well hydrated. Kindly get her consulted with a neurologist because I am not aware of her other comorbid illnesses for which she would have been under medication and hence an expert in this would be better to suggest supplements. Regarding Dementia- it is an age related deterioration and we would have to evaluate the extent of deterioration and which areas of brain is affected so that those specific areas can be retrained via neuropsychological rehabilitation. We also need to evaluate the past history whether she has hypertension, diabetes or has had stroke etc in order to come to a diagnosis of type of dementia (vascular, alzheimers, lewy bodies etc) and whether Dementia is followed by Parkinsons or not wherein we call it as Parkinson plus syndrome. You would have to take her to a neuropsychologist and if you placed in Delhi-NCR, you can consult me in my clinic or we can see how to work out so that I am able to brief the family members also about the prognostic factors and do’s and do not’s based on the present condition of your mother. Hi from last 3 years I am not able to live a normal life. When I am alone I think a lot of negative things. Sometimes in my friends I feel lonely. Its like I am not able to breathe properly. While driving a car or sitting in a car I feel the same. Sometimes when I walk or in standing I am having the fear of falling. I feel thirsty always. I need water anytime while walking or sitting or driving. I hav taken shelcal xl 25 for 1 n half years in 2014 to mid of 2015 bcaz of the heart palpitation. But it has gone away. But still I feel depressed and can not able to think properly. I have done my ecg test also but its normal. I need 1 person every time near me. What should I do. I know that its just stress n depression but I can not handle it properly. Kindly suggest asap.

Dear lybrate-user I can understand how problematic it would be when we lack attention and focus at work due to the racing negative thoughts. One noticeable thing is that the negative thoughts are usually automated in nature. There is a new concept regarding positive intelligence and attitude. IT is difficult to practice but definitely with assistance from psychologist within few sessions you would be able to rule out all your apprehensions and worry. Shelcal is not related to these symptoms that you have mentioned and let s do a preliminary analysis of your intensity of symptoms and decide whether any medication is required if at all but a sincere advice would be to engage yourself in psychotherapy sessions. Stress is meant to lead to a feeling aspect of fear which in turn activates our anxiety switch and thus respiratory rate, heart rate, BP rate etc goes to a toss but these symptoms are definitely manageable. IF any difficulty, please feel free to contact or consult us through video consultations if you do not live in Delhi so that we can engage you firstly in educating you about your symptoms and further plan of intervention using CBT (modification of maladaptive automated thoughts) and Mindfulness based stress reduction training. Dear lybrate-user Firstly, if the medicines have been prescribed by the psychiatrists, I would suggest kindly do not stop it by your own self because the psychiatric medicines work at the neuro-chemical/neurotransmitter level too so it takes time to adapt and show any effect, Dizziness, heaviness/headache, sleeplesness are few common conditions, we witness when someone abruptly leaves the medicine. There is a myth that psychiatric conditions are lifetime and medicines are also for lifetime which is not really true. Thus, I would advise that you kindly consult your psychiatrist and be medicine compliant and also request your psychiatrist to suggest you a good psychologist who can help you at the symptoms level and to relieve them for future betterment. If you unable to find, please feel free to drop a message to me and I would suggest few in Surat. Hope this is useful.

Dear lybrate-user I am sure it would have been difficult on your part as well as your colleagues and family the temporary loss in business. It is easier for anyone to say that it is part and parcel of life but these situations does leave an impact in each one of the us. May be the loss was deeply ingrained at the thought level and thus it is taking time to completely avoid it. I appreciate that you have been working on yourself and doing well. We all tend to realize any thought, mental related symptoms only when they are manifested in our body such as breathlesness, choking etc but modification or working at the grass root level would help you to be away from physical symptoms as well thus if you unable to help yourself at the thought level to modify and embrace your present, you can seek assistance from a counselor/psychologist nearby or us through in-person/video/telephonic consultation.

Dear lybrate-user Firstly, get an thorough evaluation about the severity of ADHD (Attention Deficit Hyperactivity Disorder) features in your child based on which the psychologist can plan an tailor-made intervention for your child as wel as advice if at all any medication is required for your child. Generally, behavior modification techniques (token economy, rewards and punishment techniques) are used to manage the hyperactivity in the child and attentional retraining in game format is utilized by the psychologist. Dear lybrate-user Looking at the limited symptoms mentioned by you, it could potentially be panic attacks or anxiety disorder as well so you should review with a psychiatrist or psychologist and confirm diagnosis based on which further detailed intervention can be planned. Panic attacks are anxiety disorders amd because you mentioned Tension in some format, it might activate our sympathetic nervous system which increases our BP, heart rate, respiratory rate, dilation of pupils, problems related to digestion etc. Hence, we would have to evaluate and decide the intervention. Dear lybrate-user Firstly you would have to understand the nature of your illness and do inform us about the duration and history related to the illness in order to identify the requisite factors maintaining the illness in you. Regarding the medications, yes they are good medicine and I am sure the psychiatrist must have been prescribed to you based on few indicators. I would advise you to engage in paychotherapy sessions adjunct to your medications for better results. Poor concentration, memory lapses and confusional state, heavy headedness are symtpoms of depression which would be relieved once you couple psychotherapy and pharmacotherapy. Looking at the place you are in Kangra, I am not aware of a good psychologist but if feasible you can connect to us for video consultations. Hope this is useful.
