Very very high Knee pain. I am suffering from this pain since 10 years. Now I can not run and can not even walk for more than 5 minutes. Please suggest some medicines or other treatment so that I can get rid of this pain.

Severe knee pain at your age is commonly due to osteoarthritis which means degeneration of joint cartilage causing friction between joint surfaces, leading to pain and stiffness. Lifestyle modifications and physical therapy are important treatment aspects to maintain functionality. 1. Knee stretching and strengthening exercises to improve knee strength. 2. Avoid jogging or physical activity that increases knee pain. But absolute rest is not advised. Engage in activity like swimming or walking, but perform activity which is painless 3. Application of analgesic cream over knee and warm fomentation before exercises. Apply cold pack after exercises 4. Weight loss if obese. 5. Simple analgesics like paracetamol when pain is present. 6. When pain or disability is significant, viscoelastic injection or PRP injection inside the knee joint is helpful. I). Injections of Viscoelastic substance or artificial synovial fluid into knee joint. This decreases friction in joint space ii). Injections of Platelet rich plasma derived from patients own blood into the knee joint. This regenerates the knee joint. Iii). In patients with severe pain, genicular nerve block can be given to decrease pain. For above treatment consult a pain specialist. 7. In advanced cases Knee replacement is the only option.

Most common pain is knee pain.You may worry that exercising could cause more damage or pain. But the opposite is true: Strengthening the muscles that support your knee, and keeping them flexible, is the best way to prevent further injuries. Start slowly, and build your strength over time. Some muscle soreness is normal when you exercise. But you shouldn’t feel more pain, especially in your knee. If you do, stop and contact your doctor.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. In that case my advice for you will be to get few investigations done and send the reports across to me: cbc, esr, crp, serum electrolytes, serum vitamin d levels and thyroid profile. Also you will have to elaborate your problem as to a detailed discussion of your back pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. Lower Back Pain - Know the Causes and Treatments Available!

Lumbago or Lower back pain is a condition that is characterized by mild to serious pain or uneasiness in the lower back region. Lower back pain is more prevalent among the youth whose work includes physical exertion or sitting for long hours and is also common in individuals belonging to the 'retirement age' because of their sedentary lifestyles. The pain might be acute (sudden and extreme) or chronic if it has been endured over a span of three months. 

The most common causes of lower back pain are: 

  1. Overuse of the muscles and includes strains or sprains of soft tissues such as muscles, ligaments and vertebral compression fractures. 
  2. Pressure on the roots of nerves due to conditions such as spinal canal stenosis (narrowing down of the spaces within the spine) or herniated disc (a condition of the rubbery disc present between the spine bones (vertebrae)). 
  3. Osteoarthritis is a condition wherein the shock absorbers, found in the joints, progressively break down. 

When it affects the spine, the condition causes back pain by stiffening the spinal joints. Symptoms of Lumbago (Low back pain) include: 

  1. Stiff back 
  2. Numbness in the legs 
  3. Loss of weight 
  4. Difficulty in urinating 
  5. Sciatica, which is the pain running down the lower back to hips, legs and thighs. This occurs due to a compression by an intervertebral disc in the back, which leads to a pinched nerve. 

How to diagnose lower back pain or a ​slip disc?

Some methods that can be utilized for diagnosing back include Xray, CT Scans, Myelogram and MRI scans. 

Treatment options available -
Exercises along with correction of bad posture for relieving back pain are a common component of any treatment plan. Factors such as type and severity of the pain as well as the patient?s history can help in determining the type of treatment that can be administered. In most cases, the recovery time is usually six weeks without surgery. Typical treatment of back pain includes use of muscle relaxants, painkillers and physiotherapy. Surgery is suggested in severe cases. Some other treatments are discussed below: 

  1. Rest: A period of rest and inactivity would help the injured tissues to recuperate and relieve back pain. However, extended periods of rest lead to weakening of muscles and hence might prolong the existing back pain. 
  2. Heat and ice packs: Both heat and ice therapies are used according to the patient's preference to reduce inflammation. 
  3. Medications: A large variety of prescription and over-the-counter medicines are available. Some reduce back pain caused by inflammation and others prevent the pain signals from travelling to the brain. Examples would include acetaminophen, celecoxib and diclofenac. In case you have a concern or query you can always consult an expert & get answers to your questions!

I am 43 years old my left leg knee have severe pain. I cannot walk in staircase easily and sitting also very pain. If I sit in the chair I feel no pain after I standing and walking is very difficult for few minutes. Please advice me how to recover.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. It looks as if you have started having early knee degeneration with pain which if located in the anterior region, will have to worked up. Can you tell me whether there is any pain apart from the knees, any history of stiffness especially when getting up in morning, also if possible tell me whether there are any other s/s of inflammation or infection like swelling, skin redness, local warmth etc. Kindly send me your latest knee x rays and blood test reports so that I can see and advise you accordingly. Dear I understand your concern for your mother. I would like to recommend you something that would help her to get heal and strengthen back muscles. I am sure she will get a relief. 1. Rest - take rest, but it doesn't mean a complete bed rest. Do not over exert your body. Work take rest and then work so that you do not feel tired and started paining. Precautions: 1. Avoid forward bending, if required then bend your knees first and then bend forward. 2. Avoid weight lifting. 3. Sit in chairs or car seats with proper back support. 4. Try to sleep in a position that helps you maintain the curve in your back like sidelying. 5. Get enough vit d from sun as vit d keep bones strong including spine, 6. Drink plenty of water to enhance the intervertebral disks 7. Avoid standing in the one position for a long time. 8. Try to sleep in a position that helps you maintain the curve in your back. We have various physio treatments which depends on the pain severity. So as per your question please follow the following treatment at home to reduce her pain and will give some percent of relief. 1. Apply heat directly to the area of low back pain will decrease swelling and inflammation, and ease discomfort. Apply it for 10 to 15 minutes only. 3. Relaxing for 5 minutes after ice or heat treatment. Massage - ask someone to massage your painful area using fingers or thumb. Massage using kneading technique. Slightly press the thumb and start rotating as it helps to induce relaxation, relieve pain, and reduce levels of stress chemicals, such as cortisol and noradrenaline. It will give you pain relief for time being. 3. After massage again relax for 5 min and apply any pain relieving ointment eg - volini or oraflame gel etc. Make sure not to do massage with the gel, it should not penetrate the body. Just a topical application of gel should be done. 4. Again relax your body for 5 minutes and perform some normal back extension and hamstring stretching exercises to get relief from pain. Exercises includes: back strengthening, bridging, slr, prone on elbow, prone hip extension, etc if she get pain while doing exercise please stop doing right away. Good luck. Change your sitting n standing posture. Apply mahanarayana oil n massage daily. Take triphla churn a 3gm daily at night. Medicine is also available.

My father was operated for Slip Disc on 13th July 2015. Even after operation (the next day) he reported similar pain what it was before surgery. Its been two weeks and there is no improvement. Today again he is crying out of pain. In between one day he was without painkiller and we thought he is looking good but again the next day pain came back. He is 65 yrs now. We are clueless what to be done now?

Hi you can consult physiotherapist and change sleeping position and avoid sleeping position which gives pain. As recently you have gone through surgery consult doctor and see when you can start rehabilitation exercise to strengthen back and core muscles.
