Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Shoulder pain can be due to multiple reasons, so it can be due to an injury like a soft tissue one, bony injury, tendinitis, bursitis etc if there is no h/o any injury to the affected shoulder and there are no s/s of inflammation or infection like swelling, skin redness or local warmth etc, my advice to you initially will be: ice packs at home, restriction/modification of physical activity, local analgesic spray for application, gentle anti inflammatory medication which suits you and see how it goes for next 7-10 days. Normally most pains settle down with decrease in inflammation, in the event your pain persists or increases we will have to get some investigations done for further evaluation. For pain apply hot water and cold water fomentation thrice a day for 12 days of 15 minutes duration each and consult neuro physiotherapist and take ift and laser therapy for 12 days of 15 minutes duration each followed by spinal extension exercise use only firm hard and even mattress for sleep

Hello, sometimes back pain is related to digestive trouble. If you have gas or constipation problem first solve it. Do regular yoga for back pain.

Self suffering from frozen shoulder since a year. Ultrasound therapy has also been made at a renowned hospital near my home. Free hand exercise has also continuing as suggested by an orthopedic doctor. But still pain is existing in my arms upto wrist, shoulder and upper back portion of my body. As such, medical advice is solicited for quick relief without any injection.

Frozen shoulder or adhesive capsulitis is a condition in which shoulder movements get progressively decreased, it is commonly associated with diabetes although association with other conditions is also known. Treatment includes intensive physical therapy, short course anti inflammatory medications according to one's suitability and maintenance of calcium/ vitamin d metabolism. Normally pain subsides over period of time, ultrasound may not help much but regular exercises meant for this condition are helpful. Only medical advice if you do not want injections is to take a short course of anti inflammatory / analgesic medication but mainstay will remain physical therapy and sometimes injections. Hello, first of all you should know what is backache and by which factors it caused and aggravated. The usual cause of low back pain is the sedentary lifestyle that we lead these days. There is hardly any movement. We remain seated most of the time and stand for very short periods of time. Unfortunately walking, running and cycling like activities are also not a part of our daily routine. To add to it, our diet consists more and more of junk food. Fresh fruits, vegetables and high fibre foods are consumed less and less. These daily practices lead to obesity. The increased body weight puts lot of pressure on the spine which has to bear all the extra unwanted weight. The back muscles which are already weak due to lack of exercise and wrong posture are unable to cope with this increased load. This causes pain in the back muscles. there is a need to work on the local muscle strengthening. Back muscles need to be strengthened so that they can bear the weight of the upper body. For this exercises of the back muscles are very helpful. Regular walking is also very helpful as it is a wholesome exercise of the body, improves blood circulation and helps lose weight. Most importantly in your case you are overweight then losing weight is an important part of the whole treatment plan .Long term use of painkillers for low back pain is harmful Treatment for back pain:- Homeopathic medicines are very helpful. Hello lybrate-user, knee pain occur after 30-35 are due to degenerative changes in bone. It may be due to osteoarthritis or rheumatoid arthritis. So consult a physio and learn some strengthening exercise and start doing it guided by them. From xray images and symptoms of patient we can come into conclusion either it is due to osteoarthritis or rheumatoid arthritis. So in both the condition knee strengthening exercise plays important role. There are also ultrasound, ift, swd modality availablele in physio setup to get you releive from pain. So it would be better if you visit a nearby physio and start doing exercise guided by them.

I am 60 years male height 6' weight 95 kgs. MRI report says the following; 1. Diffuse disc at L3 - L4 indenting anterior thecal sac mildly with bilateral mild neural foraminal narrowing 2. Diffuse disc bulge at L4-L5 more so on left side indenting nerve roots with significant bilateral neural foraminal narrowing (left>right) 3. Diffuse bulge with posreocentral disc extrusion at L5-S1 indenting nerve roots with bilateral significant neural foraminal narrowing. Diagnosis and remedy.

Requires detailed examination and investigation. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

Start with physiotherapy sessions and learn exercise for spine. Don' t sit on floors. Use lumber spine belt also. Take calcium and methylcobalamin supplement with omega 3.

Take sanjeevani ras vati twice a day with warm water. Take pranacharya jwar nigraha capsule and punarnavadi mandoor 1-1 twice a day with warm water. Avoid salt as more as possible. Drink luke warm water. Wipe your body with clean towel.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned neck pain as only problem. Since you are very young, if you have only localized neck pain with no other associated symptoms like radiating pain in upper limbs, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture while working and sleeping and neck ergonomics - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application like dynapar qps spray can be used and is easily available. We will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. Do not hesitate to contact me if you need any further assistance.

Cortisone Shots - Are There Any Risks Associated with them?

Cortisone shots are injections that help relieve pain and inflammation. They are injected directly into the joints such as the ankle, wrist, knee, hip, shoulder, elbow or spine. These injections are a combination of corticosteroids and local anaesthetics. These corticosteroids are similar to the hormones produced by the adrenal gland. Cortisone injections are often used to treat arthritis, carpal tunnel syndrome, trigger finger, shoulder bursitis and plantar fasciitis. Though they do not cure the disease but they provide temporary relief.

These injections may be slightly painful but provide relief in a day or two that lasts for a few weeks or months at a stretch. Hence, cortisone shots provide relief faster and have longer lasting results than other forms of anti-inflammatory medication. Another benefit of cortisone injections as compared to other drugs that need to be taken in through the mouth is that it avoids side effects of anti-inflammatory medication such as an upset stomach etc. Cortisone injections can also be used to treat backaches and pain that radiates from the spine to an arm or leg.

However, cortisone injections are associated with a number of risks. These include:

  1. Infection in the joints
  2. Nerve damage
  3. Thinning of tissue and skin around the injection site
  4. Pain and inflammation in the joint
  5. Weakening or rupturing of tendons
  6. Osteoporosis or thinning of bones
  7. Lightening of skin around the injection site
  8. Osteonecrosis or death of bones in the injected area
  9. Blood sugar spikes – this is seen mostly in diabetic patients
  10. Weakening of the immune system if the person has an underlying infection

In most cases, these side effects are short-lived. Higher dosages and frequent administration can increase the body’s exposure to the corticosteroid. This increases the risk of long-term side effects such as:

  1. Acne
  2. Easy bruising
  3. Weight gain
  4. Cataract formation
  5. Puffiness of the face
  6. High blood pressure

Cortisone may also be held responsible for the deterioration of cartilage inside a joint. For this reason, doctors do not advise cortisone injections to be had more than 3-4 times a year. The minimum interval between two cortisone injections must be 6 weeks. Doctors also typically limit the number of injections given in one joint.
When used judiciously, cortisone injections can greatly improve the quality of a patient’s life. Hence it is very important to get treated only by a qualified doctor and to completely understand the pros and cons of using this form of treatment. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Back stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day.

I will advise to avoid weightlifting for some time and give muscles adequate time to heal. You can use conservative medications and a local analgesic spray for time being. Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days. For further guidance and treatment.
