When a blow or a fall causes an injury, where the top of your arm bone pops out from your shoulder socket, it’s called shoulder dislocation. It is a very painful condition and you need to get this relocated to its position immediately. The tissues and bone related to that particular joint get damaged.
If you ignore this situation or keep the shoulder like this, it will be more harmful and painful for you. There may appear a bump in front or at the back of your shoulder that hurts.
- Sharp or a very fast twist of your arm.

In some of the outdoor games, there are high chances to get hurt in the shoulder. Hockey, football, rugby, rock climbing, volleyball etc. bring in high probabilities of injuring the shoulder, thereby resulting in a shoulder dislocation.
What is the treatment or therapy of a dislocated shoulder?
The dislocated shoulder should be very urgently treated. As this injury of the shoulder is very painful, you should consult to a specialized orthopaedist as fast as possible to fix up the shoulder to its proper joint. As it is very painful and swollen, the faster the treatment, the better it is. When your doctor fixes your shoulder in its actual place or the bone of your arm comes back to the socket, most of your pain will go away and you get huge relief from pain. After the shoulder bone is relocated, you can use further conservative treatment to reduce the pain and swelling of that place. Dislocated shoulder may take 3 to 12 weeks to recover. But this is the approximate time and it depends on the seriousness of the injury.
After the treatment, when all the symptoms are gone, the doctor will suggest some exercises for better results and normal shoulder movement and to prevent future shoulder injury. Ones the shoulder is dislocated, the chances of dislocation in future is further increased as the tissue supporting the head of humerus into its socket is torn. Knee is a hinge joint where the lower leg bone tibia meets the thigh bone femur. During osteoarthritis, cartilage or ligament defects and degenerative arthritis, knee replacement surgery is recommended worldwide to get relief from extreme pain. Knee replacement surgery, also known as knee arthroscopy, is a surgical procedure, in which the affected knee joint is replaced with synthetic material. The most likely candidates for total knee replacement are the patients with severe destruction of the knee joint coupled with progressive pain and impaired function.
Modern technological advances have made computer-assisted knee replacement surgery extremely popular around the globe. In this surgery, the surgeon is assisted by a computer to remove the optimum amount and angle of the bone, which otherwise is done by inspecting manually. This is an excellent example of surgery through a small incision and it eliminates the chances of human error. Perfect alignment and balance are achieved and hence longevity is also increased to 20 - 30 years. There are many types of knee replacements, most common being the total knee replacement or Total Knee Arthroplasty. In addition, there is a partial knee replacement, bilateral knee replacement, revision knee replacement and knee arthroscopy.
In knee replacement surgery, the worn out surfaces of joints of the knee are replaced with an artificial implant of plastic and metal. The lower end of the femur bone is removed and replaced with a metal shell. The upper end of the lower leg bone (tibia) is also removed and replaced with a channelled plastic implant with a metal stem. A plastic ball is also added under the kneecap depending on its condition. These artificial components are commonly referred as prosthesis. Patients whose knee joints have been damaged by either trauma or progressive arthritis should consider total knee replacement surgery. Post-surgical hospital stay after knee joint replacement is usually three to five days. The surgery has a very high success rate and shows dramatic improvement after a month. This improvement is most notable one month or more after surgery. The pain caused by the damaged joint is reduced significantly when the new gliding surface is constructed during surgery. Initially, patient will walk with the help of a walking aid until the knee is able to support full body weight. After six weeks, patient can walk comfortably with minimal assistance. Patients with artificial joints are prescribed to take antibiotics during the course of any elective invasive procedures including dental work. Physiotherapy is an essential part of rehabilitation and it will increase the muscle strength and patient can enjoy most activities, except running and jumping.
Shoulder pain can be a persistent dull and nagging sensation in the upper arm or shoulder area. It can be easily differentiated from neck pain as it is related to the movement of shoulders i.e. it will increase and decrease with shoulder movements especially overhead activities. The other signs to identify shoulder pain is if your shoulder movements are reduced.
If you find it difficult to scratch/wash your back, comb your hair or reach your back pocket. Shoulder pain can happen due to various reasons, vary from person to person depending on the activities he/she is involved in, for e.g. sports, manual job etc.. Diagnose Shoulder Pain-
Shoulder pain can be diagnosed only after a thorough history taking and clinical examination. However, some imaging studies can be very helpful like:
- X-rays: Through an x-ray one can view the shoulder in many different views
- Ultrasound: With ultrasound, you can also check the damaged caused to the tendons and muscles of the shoulder but the quality of reporting depends upon the expertise of the doctor.
- MRI: MRI gives a clear picture of the shoulder as it shows everything related to a shoulder joint that is joints, vessels, tendons and muscles that too from different angles.
- Diagnostic Arthroscopy: A surgical procedure often used by orthopaedic surgeons to diagnose and treat issues inside a joint. Arthroscopy has the big advantage that one can use it to diagnose as well as treat at the same time.
Treatment for Various Types of Shoulder Pain-
- For Shoulder Impingement or Rotator Cuff tear: Give rest to the shoulder for a few days only Dedicated Physiotherapy Protocol Bursal injections If these fail then, (Surgery)Arthroscopic sub-acromial decompression If Cuff tear is found, then one can go for Arthroscopic/Mini-open repair
- Labral Lesions: For anterior Labral lesions (Bankart's) following anterior shoulder dislocation then surgery in the form of Arthroscopic Bankart's repair is the only option to reduce risk of recurrence and degeneration.
SLAP Lesions -
- Pain relief and physiotherapy as the first step If this fails then following surgical options are available
- Arthroscopic debridement or repair of labrum
- Biceps tenotomy or tenodesis
In this case, the joint is already destroyed and has to be resurfaced for pain relief and to maintain ROM. If Rotator cuff muscles are working, then Shoulder resurfacing hemiarthroplasty (preferred in young people), that has a life of 10 to 15 years, Total shoulder replacement (preferred in middle-aged to old people) has a life of 10 to 15 years. But, if rotator cuff muscle is not working then Reverse Geometry Shoulder replacement is recommended, that has a life of 10 years.
It is always advisable to get yourself diagnosed properly in case of shoulder pain as management changes depending on clinical situations. Neglecting shoulder conditions can lead to bigger problems later on.
