Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to be undergoing total hip replacements. For your age, I would think ceramic on ceramic bearing surface will be a good choice, or ceramic on poly bearing surface. Kindly let me know if you need any other information. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

It feels like you are having knee Osteoarthritis. Osteoarthritis can occur at any time of life but the incidence increases with age. It usually affects people over 40 years of age. There is no cure for osteoarthritis but the progression of the disease can be slowed and pain and disability reduced. Treatment options include: Healthy body weight: Maintaining a healthy body weight will reduce stress on the arthritic joints. Nutritional supplements Supplements such as glucosamine have been reported to relieve the pain of osteoarthritis and halt, or even reverse. So please try to take this supplement once a day. Exercise Exercise can assist with the maintenance of muscle strength and the mobility of arthritic joints. Activity at a level that does not cause pain is not thought to worsen osteoarthritis. Non weight-bearing exercises such as swimming or aqua jogging in a heated pool may be beneficial. Range-of-motion and stretching exercises will help maintain the maximum possible movement in the joint. Stretching will help maintain or restore flexibility to the joint’s muscles and tendons. Cycling is a good exercise. You can apply cold or heat several times a day to reduce your knee pain. Just remember to cover the cold pack with a towel and keep your heating pad at a low setting to avoid burning your skin. Whether you use heat or cold, remove it after 20 to 30 minutes. If the knee is swollen or sore, the following positions and activities you should avoid until knee pain and swelling resolve: Squatting Kneeling Twisting and pivoting. See a Physiotherapist for detail exercises programme and management. Best wishes.

All About Hip Osteoarthritis

When joints get inflamed, it is known as arthritis. This causes swelling and pain in the joints of your body such as the hips and the knees. When the cartilage gets damaged at the ends of bones, a type of arthritis called osteoarthritis occurs.

There are two main types of osteoarthritis:

  1. Primary: This type is more generalized. It affects the knees, fingers, hips, spine and thumb.
  2. Secondary: This type can occur after injury or joint inflammation, or as a result of any disease that affects the ligament's composition, like hemochromatosis.

How is the hip affected by osteoarthritis?
Patients have difficulties walking if they have hip osteoarthritis. Diagnosing hip osteoarthritis is difficult. It is because the pain appears in varied locations, such as the groin, buttocks, knee or thigh. The pain can be a dull throb or can be sharp and stabbing. The hip is usually stiff.

What are the causes of hip osteoarthritis?
The causes of hip osteoarthritis are unknown. However, the factors that can lead to osteoarthritis are joint injury, excessive weight and rising age.
Along with the abovementioned factors, the following may contribute as well:

  1. The joints may not have developed accurately
  2. The cartilage may have inherited the defects
  3. If the joints are too irritated due to added pressure on them, like excess body weight or stressful activities that include hip movement, osteoarthritis may develop.

What are the symptoms of hip osteoarthritis?
The symptoms of hip osteoarthritis are as follows:

  1. Joints feel stiff when you get out of bed
  2. Joints feel stiff if you have been sitting for some time
  3. The joints feel painful, swollen or tender
  4. You can hear (a "crunching" sound) or feel the bones rubbing against each other.
  5. You find yourself unable to do certain things, like putting on socks, as it requires your hips to move.

How is hip osteoarthritis treated?
The main objective of treatment is to restore the patient's ability to move around. A part of this objective includes correcting the function of the hip and managing the pain. The treatment can include:

  1. Joint care and rest
  2. To take weight off the area affected, a cane is used
  3. Pain relief that does not involve drugs
  4. Shedding excess weight
  5. Exercising
  6. Surgery
  7. Alternative and complementary therapies

This condition is known as post viral reactive arthritis which is usually self limiting. In some cases the pain may persist and patient may behave clinically like Rheumatoid Arthritis and few of them also become postive for Rheumatoid Factor (RF) and Anti CCP bucause this kind of viral infection often causes immune dysregulation. If he is a smoker ask him to stop immediately as it is one of the strongest risk factor for developing Rheumatoid Arthritis. If symptoms persist we will have to plan to start disease modifiying anti rheumatic drugs (DMARDS). Power doppler usg of the joints can be a helpful guide. Consult your local Rheumatologist.

PRP is extremely safe. But its experimental at best. If knee is too damaged I wont recommend it, better save money on replacement. If knee is active (inflammed) because f rheumatoid arthritis, a steroid injection is much cheaper and very effective. No body knows how definitely PRP works (dont hit googlemit has asnwers for evrything , I am talking of hard scientific evidence). Who should go for PRP ? Patients who have money to try a safe therapy which might or might not be effective, to avoid or delay surgery (which can be very costly), immediately (might still require even if PRP is successful). How many times PRP required ? Nobody knows, one to three injections might be helpful. There is no definite protocol. It it helps might be worthwhile to repeat in long run. But If knee is severely damaged I won't put too much money on PRP.

Hi pain or soreness in the foot, that eases with time during the day, is generally indicative of plantar fasciitis. The first step you take generally after sitting or standing for a long time or when you get up from the bed is painful. As you start walking the pain clears up. Plantar fascia is a ligament that connects your heel bone to your toes. Other causes of foot pain can be rheumatoid arthritis, faulty arch, ligament tears, fractures, and gout. +wear soft padded footwear. +do stretching of calf muscles +learn strengthening exercise of ankle +take contrast bath at physio clinic. Do reply to discuss detailed treatment plan. Wishing you a good health.
