It not advisable to do any surgery in first trimester of pregnancy, Given the risks, any surgery during pregnancy is generally avoided. But if the inflammation is severe, if there's an infection, or if the colic is acute, an operation may be necessary to prevent more serious complications. If you need gallbladder surgery, it will probably be done laparoscopically. In general, the goal of treatment during pregnancy is to reduce the symptoms and complications, and then treat the disease itself after pregnancy. If you had gallbladder disease that was controlled during pregnancy and no surgery was required, it's important to have a follow-up evaluation. Symptoms can occur in the postpartum period or later in life, so have an evaluation when you're not in pain.

If there is no bleeding then nothing to worry. Infact you should avoid scratching. Keeep the area clean and dry and apply any dusting powder. Keep diabetes well controlled. Avoid scratching because once you scratch then itching never goes.

MS - General Surgery, MBBS

What to do in asymptomatic gall stone disease?

If gallstones are detected incidentally, always go for surgery. As in diabetic patients, gallstones are not symptomatic. But it can cause complication in future. As stone can slip forward into common bile duct or it can cause pancreatitis (inflammation of the pancreas).

I have gone through your query and can understand your concern. It's not clear what causes irritable bowel syndrome, treatment focuses on the relief of symptoms. Following steps should be taken 1. Eliminating high-gas foods such items as carbonated beverages, vegetables — especially cabbage, broccoli and cauliflower — and raw fruits. 2.Some people are sensitive to types of carbohydrates such as fructose, fructans & lactose, called FODMAPs .FODMAPs are found in certain grains, vegetables, fruits and dairy products such as wheat, rye, barley, onion, garlic, apples, apricots, avocados, blackberries, cherries, etc 3.Taking fiber supplements, such as psyllium or methylcellulose, with fluids may help control constipation. Fiber obtained from food may cause much more bloating compared with a fiber supplement. 4.Anti-diarrheal medications such as loperamide, can help control diarrhea. 5.Anticholinergic and antispasmodic medications. These medications, such as hyoscyamine and dicyclomine, can help relieve painful bowel spasms. They should not be used if you have constipation. 6.Counseling. You may benefit from counseling if you have depression or if stress tends to worsen your symptoms. 7. In some people improvement in diarrhea symptoms occurs, if they stop eating gluten (wheat, barley and rye) Don't worry you would be fine Hoping your query is solved.

Tips For Dealing With Gastric Problems

MBBS
General Physician, Trivandrum
Tips For Dealing With Gastric Problems

Excessive gas can be defined as bloating, belching, flatulence and burping. It does not matter what you call it, but what matters is how you treat it and get relieved from the embarrassing symptoms.

Here are some ways in which you can avoid gastric problems:

  • Avoid eating foods that can cause gas or acidity. Vegetables which are known to cause acidity are Brussels, Onions, Garlic and Broccoli; dairy products like cheese, milk, ice cream and whole grains can cause acidity. The reason is that these products have fiber, starch and sugar which cannot be digested easily, thus causing pain in the intestines. Fruits which contain sorbitol and aerated drinks should also be avoided. Eat everything but in moderation.
  • Avoid drinking before meals as the stomach acids are lost and the food cannot be digested well. Water and liquids should be taken half an hour after a meal for a better digestion.
  • Food should be eaten slowly. Eating and drinking quickly, allows a lot of air to go into the stomach with the food. This in turn can cause a stomach pain.
  • Over the counter drugs can be taken for immediate help in digestion. These medicines contain enzymes which aid in digestion. The enzyme works to dissolve the sugar that is found in many fruit and vegetables.
  • Some habits like chewing gum, drinking from a straw and smoking, etc. causes air to fill in the stomach, thereby, leading to gas.
  • Artificial sweeteners such as sorbitol should be avoided as these are difficult to digest and can easily lead to a feeling of bloating.

However, all the pains might not be due to gas and could be a sign of something serious. These could be:

  • Lactose intolerance: This is the inability to digest milk and milk products. The sugar in the milk is known as lactose and cannot be digested easily.
  • Irritable bowel syndrome: People who suffer with pain in the lower abdomen constantly might be suffering from IBS.
  • Colon cancer: If the pain lasts too long then getting a screening done is a good idea to rule out if one is suffering from colon cancer or not.
  • Upper gastrointestinal disorders: If one gets belching occasionally, then it is alright, but a constant belching might be a sign of gastrointestinal disorder in the upper tract. These could be gastroesophageal reflux disease (GERD), gastroparesis, or peptic ulcers.
