Friends, I'll be sharing with you some relevant points regarding obstructive sleep apnea. Obstructive sleep apnea is basically cessation or stoppage of breathing while you are sleeping. So, why does it occur and why is it important to know about obstructive sleep apnea? Usually when you sleep our body relaxes and all the muscles of our body relax. So is the case with the muscles of our airway which includes the muscles of our pharynx and oropharynx. And usually when you sleep, because of the relaxation of the airways, the muscles of the airways the lumen of the airways decreases.
And in a normal person it decreases to almost 40% of the awake state but in patients who are obese who are having a BMI of more than 25 kg per metre square, what happens is because of the fat deposition and during the period of sleep, there is a narrowing up to 100% of the airways and what happens is because of this almost near complete closure of the lumen of the airways, it is very difficult to breathing. So, when you try to breathe these air into the airway, against a closed airway, there is a sound, which you call it snoring and this is the snoring which indicates or suggests that a patient may have obstructive sleep apnea. And this obstructive sleep apnea patients usually complain of increased tiredness, decreased zeal for work, excessive daytime sleepiness, they may have difficulty concentrating in their work, they may have frequent naps in the daytime, they may also have naps while driving; which may be the cause of accidents. So, why does this happen?
Why does excessive daytime solemnness happen? It is because if you are airway gets totally obstructed by your sleeping, then the body cannot get oxygen and when this oxygen enters your body through the airways, oxygen concentration decreases and if they fall to a certain level which causes the brain to get activated and you awake. And these apneas are for a very short period and you may not be aware of these awakenings but that deflects in your daytime solemnness. So, because of these fall in oxygen saturation overnight, during your sleep there occurs a lot of metabolic and cardiovascular changes like there will be rise in your blood pressure, there may be cardiac rhythm disturbances and you may develop metabolic disturbances like diabetes mellitus. So friends, if you think you are having symptoms of snoring and daytime solemness along with the easy fatigue ability or easily getting tired or you are having a decrease in concentration or you fall asleep while driving, then it is better to consult one of your sleep specialists nearby so that he may advise you sleep test which will lead us to know the quality of your sleep and the desaturation which you are having overnight and we may be able to help you with some type of instrument which you called CPAP and other methods which will help you in decreasing these episodes of desaturation and in the long run it will improve your metabolic and cardiovascular parameters.

Girne se unke left thigh me fracture ho gya tha. Operation se vo thik ho gya lekin uske baad pneumonia ho gya. He was not even able to complete his sentences because of breathlessness. Kafi examination ke baad pta chala that patient was also suffering from poor cough. Diagnosis ke baad aspiration pneumonia detect hua. Aspiration pneumonia me aap jo bhi khate hain vo food pipe me na jake wind pipe me chala jata hai.
Iski vjha se hmare lungs me infection ho jata hai. Wind pipe ko trachea bolte hain and food pipe ko oesophagus. Aspiration pneumonia me hmara flap band nhi hota hai or khana thoda sa slip wind pipe me hota hai and thoda food pipe me. Iske causes kya hain? Ye infection older age me and bachon me hota hai. Neurological problems like motor neurone disease, stroke, seizure, in sab se brain ka signal swallow krne ke lia proper nhi hota hai. Discordinate hone ki vjha se dysphagia problem ho jati hai. Aspiration dental problem ki vjha se bhi hota hai or aisi koi bhi problem jo apne senses me nhi hai, cancer ke patients me bhi ye problem ho skti hai.
Iske symptoms kya hain. Iske symptoms are equal to pneumonia. But iske symptoms thode slow rehte hain. Ye long time tak trouble krte hain apke lia. Symptoms iske hain saans fulna, whistling sound while breathing, chest pain, khasi hona, bulgum me smell or blood ana. Unko food swallow krne me problem hogi. Khate time choking or khasi ki problem hogi. Bad breath ki bhi problem hoti hai. Oxygen ki kami bhi ho skti hai.

Asthma starts in early childhood and may persist in adult life. It is an episodic disease which is intermitted initially. Later, if left untreated, it become continuous and perennial. Whne one is exposed to dust, fumes, pollen and smoke then the inner lining of the airways swells and becomes inflammed and the lumen narrows. So it becomes difficult to breathe. So these patients get cough, wheezing, snoring and running nose.
The diagnosis of this condition is very easy. What you have to do?
You do Spirometry or a lung function study in which you blow into a computer and you get a graph. You repeat the test after giving bronchodilators, say 15-20 minutes. If you find an improvement in FEV1, one of the parameters, more than 12% you clinch a diagnosis of Asthma.
The treatment of Asthma is very simple these days. The main stay of treatment is inhaled corticosteroids which is given through the inhalers. There are many types of inhalers available with good drugs which act for 12 hours which you need to take only in morning or evening and your Asthma gets controlled. When I say controlled, that means you will no longer have cough, wheeze or sneeze. You can sleep undisturbed in the night and can perform all the activities. You can lead a normal life.What you require is small dose of this inhaler.

Good morning I am DR Rajput, Pulmonologist. I deal with the respiratory disease which include Asthma, COPD and sleep apnea. The sleep disorder, sleep apnea is very common these days. People snore and they do not know that they have a disease.
I am going to talk about, "Why do people snore and how it affects your health?"
You know the structure. We breath through nose and we eat through mouth. There is a pasaage in the neck behind the tongue where the food and the air we inhale pass through it. But when we sleep, our posture changes due to which muscles get relaxed and makes the passage narrow. Then the tongue drops and makes it further narrow. If you are over weight, due to the neck fat, the passage gets further narrow. So, when the air passes through the narrow passage, it produces snoring. The stage beyond snoring when the two valves in the neck meet each other, the air flow stops and cause Apnea. When the air flow stops, the oxygen level in the body goes down, the brain gets aroused and sends the signal to these muscles and then you ultimately snot and this snotting opens the passage a bit. So, this repeatedly occur in the night and this results in oxidative stress. BEcause 1/3rd of your life, you are sleeping, i.e, 8 hours out of 24 hours.
So, during sleep, if the air flow is less which is called Hypopnea, the air flow stops is called Apnea. This leads to Endothelial dysfunction, which leads to High blood pressure, diabetes, cardiac problems.
So, it is important to know about Sleep Apnea and its relation to lifestyle diseases like hypertension, diabetes, cardio vascular diseases.
The diagnosis is very simple these days which is Polysomnography. It is a sleep study in which we record the brain EEG, EOG, EMG, air flow, snoring, oxygen saturation ECG. This gives an index which is called Apnea Hypopnea Index. If the Index is more than 5 than you have to be treated.
The commonest treatment which is available these days is the CPAP Therapy. CPAP is a very simple therapy. There is a mask which fits on the nose and it is connected to a hose pipe to a machine. The machine gives pressure to the air and this pressure splints this air way and the air flow is maintained. Oxygenation maintains, snoring stops and oxidative stress dissapears. This helps in controlling blood pressure, the diabetes become normal and the risk of cardio vacular problems decreases.

The aim of premarital counseling is to prevent oneself from having unwanted pregnancies, to prevent oneself from infectious diseases, prevent from having an unsuccessful relation and to have a planned pregnancy. We should know what the fertile period is, what is the safe period and when is the time when there are high chances to get pregnant and what are the period when you are less chances of getting married. So and you should know completely about contraception. Contraception is very important because if you are planning to get married and into a relationship you should use contraception it’s depending on whether you are planning a baby or you not planning a baby. Just after marriage using a barrier method like a condom and oral contraceptive pills are the best methods to be used together just for 3 to 6 months till you decide which more of you are comfortable in. it is always better to discuss with your doctor. You should start planning 1 month, at least, one month prior to your marriage, because contraceptive pills oral contraceptive pills have effect starting one month if taken one month before. By failing to prepare, you preparing to fail.
