When I see some person or when I was walking or going the thoughts came to my mind that I kill that person but in reality I do not do that and then I feel depressed, uncontrollable I know that these thoughts are not real but still I feel very bad but thoughts came to my mind that now I kill someone so I got arrested but in reality no such things happen so what should I do with such situations please help me. I would happen 2 days ago but yet the same thoughts came to my mind although I see that person is alive. Also any imaginary thought came to my mind like I push blade in my anus or I cut my penis with knife but in reality no such things happen only I imagine this things and get worried. I know these thoughts are not real but still they come to my Mind all the time each thought is replaced by another thoughts and same procedure repeated. They are uncontrollable, I try to think and believe in reality but thoughts came to my mind are not controllable. I have go through the psychiatrist for treatment and my treatment is continuing for one week. But still I cannot find relief. Please consult me .How should I deal when these thoughts came to my mind.

There is a high likelihood of you suffering from OCD. Treatment should consist of medicines, counselling including CBT. Good treatment outcomes can take 3-6 months, so please be patient. First try and not pay attention to such thoughts, and try and keep yourself occupied and do thought charting and allot a timeslot where you can note down your thoughts. Please take care.

Hi, I am 21 Female. I am in a relationship. It's been 5 years now but our relationship doesn't look good. There is no understanding between us. He can't understand me he loves me but he takes everything so easy. He can live for months without talking to me but I can't even be a day without communicating him. I don't know how to change him. I just want to live happily without him but I'm failing every time he convinces me so quickly and easily but I'm not getting that how to change him and convince him I just want to live happily without him but I don't know how. Please suggest me either to change him or to leave him please I am almost getting mad. I'm always thinking about him and I have no thought. I'm unable to eat and sleep. Please suggest me.

One of the major issues appears to be your pre-occupation with the partner and relationship. It appears to be a good time to rekindle with your original self, the way you were before the relationship. Allow yourself to nurture your original hobbies and activities, your previously pleasurable activities eg. Meeting up with other people (friends/cousins. But with a focus on yourself and not to change him. Let time take it's course from there on. Try and meet a counsellor for individual therapy. Best of luck.

Depression and headache can vary from patient to patient, wrt severity and duration. So we need to assess regarding the above and also need to rule out possibility of migraine. With assessment, we will then be able to let you know whether the management requires both medicines and counselling/psychotherapy (which is frequently the case) or counselling only. Take care.

Depression should be objectively treated as an illness. Low mood, withdrawn behaviour, lack of interest and motivation, low energy and anxiety are symptoms of the same. Please lift yourself and approach a psychiatrist asap, the earlier you start, the earlier you are likely to regain your ownself. Medicines alongwith psychotherapy should take care of you completely. I have panic attacks and frequent anxieties, I feel depressed every time this sickness comes. I am a student so I can't do anything living with sleeping pills. What do I need to totally cure this sickness?

You appear to be suffering from panic disorder. This is seen in 2-3% of the people in the general community. Clonazepam (clonotril) is used as part of initial treatment but is not a definitive treatment. Proper assessment followed by treatment with an ssri medication is definitive treatment. Also, we need to assess for any comorbidities and assess triggers for such panic attacks. Treatments including cognitive behaviour therapy & relaxation techniques would be extremely helpful. I have smoking habit for the last 20 years and I want to quit but don't know how to do it? If you can help it will be great.

Smoking is a common problem but is easy to quit. A psychiatric assessment followed by treatment with medicines including nicotine replacement therapy alongwith varencline (champix) alongwith self help tips and counselling is all you need to leave smoking in 3 weeks. But perseverence with treatment is the key to permanent relief from thr problem. Get going. Best of luck.

You do appear to be having symptoms of depression. Would recommend that you have an assessment by a psychiatrist as it can improve with appropriate treatment. You are likely to require medicines and/or counselling depending upon your assessment. Best of luck.

My wife is schizophrenia patient, do not want to take medicine What will do? please suggest.

In a patient with schizophrenia who is not taking medicines, should be assessed by a psychiatrist asap either in the clinical/hospital setting. But if the patient is not willing to visit a psychiatrist, the patient should be seen on a homevisit, and if the latter is not feasible, patient should be admitted directly. Intensive medications should be initiated with timely initiation of insight oriented psychotherapy. Long acting antipsychotic (medication) injectables are used for such patients quite frequently, which can be given once a fortnite or month for patients who have problems with compliance.
