Dr. I am 22, very depressed due to a sexual problem. One my testicle went up (I don't remember if it's from birth or happened later.) Due to which some of my organs i.e. Private part, testicles and male breasts, didn't grew. It is very small (Probably 2 inches when erected). Initially I used to think that it's a myth. I even tried to discuss the problem with parents but they said that it's just a myth and will resolve with time. Now, when I am full grown adult and know the facts. I have accept the reality. I am not able to sex neither I can produce any sperm. It erects and ejaculates in just 1 minute. My life has become hell. I feel very embraced. I am worried about my future. Sometimes I think of committing suicide because I believe this is an exceptional nightmare. My parents are getting old and I'm the only boy child in the family. They want me to get married in next 2 years. But how can I spoil life of a girl. And there is fear of defamation too. Doctor, I beg you for a solution.

Secondly, there are a number of parts to the problems that you are describing and each will need to be evaluated in detail and properly. This will need a physical examination as well and therefore, you will need to be seen in person. For the time being, understand that there is no need to lose heart! Lot of people share similar problems and get/ have gotten better. So, don't worry. Get yourself properly evaluated and treated. Take care!

Hi, Thanks for writing in, welcome to the forum. Premature ejaculation is a very common problem. There are a number of medication and non medication based solutions to this. But before any intervention, we need to understand and evaluate the problem carefully to see what potential causes are there. Taking any medication without properly understanding the cause is dangerous. I am not an expert on ayurvedic medicines. However, I can reassure you that based on a proper evaluation, a safe and satisfactory solution can be found keeping your preferences in mind. Take care!

Hello Doctor, I am 25 years old male going through some mental situation which I don't able to understand. I don't able to keep myself motivated towards anything I do like my job, my studies or every goal of life which I want to achieve. Full day I feel like my mind is stressed and full of tensions. I sleep more than usual (around 9-10 hours). In the evening time I feel like little motivated and plan a lot of things to do in upcoming day like wake up early, do exercise, start me work, etc but when the net day comes and I ended up waking late and all the plannings gone and I again become apathetic towards life. I don't know what is going on with my mental condition and this is happening from last 5-6 months after I relocated from my hometown to another town in the search of job. Also I am living alone. Sometime I feel I am going into depression. Could you please help me in this?

We need to see if what you are describing is an adjustment disorder, a unipolar depressive episode with personality issues or a depressive episode in the context of a soft bipolarity. This will need a detailed evaluation and appropriate recommendations will follow. Regardless of the exact diagnosis, psychotherapy or counseling will be useful. The need for medications and if needed, the choice of medicine can be ascertained after a detailed evaluation to confirm the exact diagnosis. Eat healthy, exercise and meditate. Don't worry, you will get better. Lots of people with much worser situations get better. Take care!

My sister is 30 years old and unmarried. She was a very good student in school time but later went into depression so could not do well after school, failed to crack competitive exams and later also could not do well in studies. Lately she fell for a guy in her office thinking that he also loved her ,but he soon got married to someone else. All this while she had got too much involved in worshipping and all and started to believe in too much superstition and fiction. She has declined to get married to somebody else and now isn't even willing to finish her studies. And feels very hopeless about life. Nowadays she has become very verbally violent with the family members and blames everyone over one thing or the other. And listens to almost nobody. And also not willing to go to a psychiatrist or a psychologist as she thinks that they won't be able to help her. Please advise on how to help her.

Hi, Thanks for writing in, welcome to the forum. This is not an uncommon situation. It is clear that a psychiatric evaluation is warranted and will likely result in a diagnosis also. Very often, people do not want to visit a psychiatrist because of various assumptions - I can not be helped; I will be admitted; I will not be treated with respect; I will be given medicines with side effects; I will have to take medicines for life etc. But when they do meet a good psychiatrist, many of these assumptions turn out to be false and the person becomes willing to visit again. Therefore, I think what needs to be done here is to ask her to meet a psychiatrist once and see how things go. She can be reassured that no medicines/ admission / procedure will be done without her consent and if she does not like the interaction, she need not come again. Very often, this approach is very effective. Hope that answers the question. Take care!

I am a 54 years old divorced male living all alone. Children well settled. No liability. After my mother expired I went into depression and also consulted a doctor for the same who advised me nexito 5 and 10 mg morning and evening along with modalert 50 mg. But as of date I have lost interest in everything and feel totally drained out mentally. The medication was ok in the starting but of no use now. Visited the doctor many times but to no positive results. I don't have any suicidal thoughts but have lost interest in everything. I do take lonazep .5 mg at night. Going to gym regularly in morning. Have lost all interest in work and don't feel like working any more. Please advise. I stay in New Delhi.

Dear sir, Thanks for writing in, welcome to the forum. Depression is a complex entity with many different forms and presentations. Certain aspects of depression respond to medicines, while certain others to psychotherapy, and still others to exercise/ diet/ sleep etc. Very often, one antidepressant is initially effective/ partially effective and then loses efficacy. The correct thing to do in such a scenario is to re-evaluate the diagnosis and if that is accurate, shift to a different medicine based on the detailed history/ evaluation and combine it with psychotherapies such CBT. It is great that you are exercising regularly. I assume that blood tests including thyroid and vitamins are normal. The above suggestions will help you to proceed further. Take care!

Hi, Thanks for writing in, welcome to the forum. Forgetting may happen due to a number of causes, some of which are completely reversible and some partially so. The latter will benefit from early intervention. First, we need to assess the extent of forgetfulness in detail and see over what period of time this has developed. We also need to see if any other brain functions are affected or not. Then we proceed towards looking for a cause. Baseline tests like hemogram, RFT, LFT, RBS, thyroid profile, Vitamin B 12, folate are necessary. Based on these reports, pointers towards neurological, psychiatric or psychological causes can be identified. If needed only then additional tests can be carried out. Overall, considering that you are only 41, possibility of more severe diseases seems less likely, In any case get in touch with a professional for a proper evaluation and management. Take care!

Experts I am a guy of 27 from new Delhi India. 2 weeks ago on 12th of May I had a protected sex with a masseuse in a spa. I used a condom all the time. And I'm 99% sure it didn't break since when I pulled it out my semen was still inside the condom. All of it took about 10 mins. Now I'm in the third week and I'm experiencing a little fatigue and cold (running nose) and sore throat. I believe these symptoms are referred to early symptoms for hiv. But I need your advise in this. I need to ask a few questions 1. What is the risk level of transmission under these circumstances? 2. Should I go for hiv test? Hi, thanks for writing in, welcome to the forum. The symptoms you are having, in all likelihood, is not related to HIV. Get a p24 antigen test done to relieve yourself of this worry. Also get STD panel of tests done to check for other bacterial/ viral infections just to make sure and feel safe (as HIV is not the only STD). In all likelihood, all of these tests must turn out normal. Based on the details provided by you, there is little to no risk of transmission. Take care!

Hi, Thanks for writing in, welcome to the forum. The first and the simplest way to approach the problem you are facing is to treat the premature ejaculation. I am not aware if you have had treatments for this in the past. Either ways, a look into the PME problem should make things very much better. Hi, Thanks for writing in, welcome to the forum. Was the medicine started as per directions of a psychiatrist? Headache is a common side effect of many anti-depressants if started at a higher dose. Also, headache could be due to other causes also. If headache was there before start of medicines, obviously other causes are involved. Very often headache is a symptom of anxiety/ depression. So I think an evaluation by a psychiatrist is warranted. Hope that answers your question. Take care!

Hi, Thanks for writing in, welcome to the forum. Naltrexone is just one of the many anti-craving medications used in the treatment of alcohol use disorders. Anti-craving medicines are one of the multiple groups of medicines that are used. It is also important to understand that medication is only one part of the treatment or 'de-addiction' Naltrexone definitely needs to be prescribed under medical supervision and monitoring. Based on a detailed evaluation, we must first see if naltrexone would suit you or other anti-craving medicines would be better. Also, the evaluation will guide us to decide whether other medicines/ psychotherapy/ lifestyle changes/ medical interventions/ tests are needed or not. Most psychiatrists in Delhi prescribe naltrexone. Hope that answers your question. Take care!

I will be restarting with my mba from 1st june. I feel sleeplessness and get worried daily usually in the night time. How do I know whether I am depressed or not? I really need help urgent. My mood is always bad in the night time. Due to some gastric problem I had consulted a doctor who helped me with an antacid medicine and anti depressant. He said the gastric problem is recurring because of your mental stress and anxiety. Please help what is wrong with me I am just not able to figure out.

Hi, Thanks for writing in, welcome to the forum. You are obviously having problems adjusting to the troubled circumstances in your family/ life. There are certain symptoms of depression that you are reporting, but it is not clear if you have a depressive disorder or what is known as an adjustment disorder. This is important because antidepressant medicines have no role in adjustment disorder whereas they may have a role in a depressive disorder. Therefore, it is important to have a proper evaluation by a psychiatrist. It is tempting to have a quick, short cut kind of a solution and is only human and natural. But in the interest of your career and long term perspective, an evaluation by a mental health professional is certainly warranted and advisable. Do not neglect food and drink. Practise some form of exercise for atleast 20-30 minutes 4-5 days a week. These will serve as first aid measures. Take care!

Hi, Thanks for writing in, welcome to the forum. It seems that some kind of an emotional problem is present. Some of this may or may not be part of your personality, but it appears that atleast some part of this is a disorder, that can be improved significantly with professional help. With the brief description provided by you, it is very difficult to exactly tell what the problem is and a detailed evaluation will be needed. However, it appears that the problem is not very severe, but not too mild to be ignored either. Seeing a professional may help to clarify things. In the meantime make sure you get 8 hours of sleep a day, exercise 20-30 minutes a day and have a balanced diet. Spend more time with your well-wishers/ close friends even though you may not feel like. Take care!

Hi, Thanks for writing in, welcome to the forum. Considering the longstanding history, I think reasonably good progress has been made. I am not sure if your question is whether you should see somebody else or ask your own doctor to increase dose/ change medication, or, you are asking if the medicines can be stopped now. A lot depends on how much symptoms is still present and how troubling it is. Medicines will certainly need to continue at the time being but whether a dose increase or change of medicines is needed depends on how troubling the residual symptoms are. Although I am not aware what medicines you are on, it may not be wise to decrease/ change the existing medicines right now, risking re- emergence of symptoms. Very often, in OCD, symptoms improve significantly and continue at a low level. The exact scenario in your case is a little unclear to me and will need a proper evaluation to clarify. Take care!

Hi, Thanks for writing in, welcome to the forum. This kind of anxiety is common and can be part of social anxiety alone or a more generalized phenomenon. The reason or cause behind this is complex. It can involve one's nature/ personality or an anxiety disorder or a combination of both. If this has been there since childhood, there is likely a large component of personality. If it is something of recent onset, the component of the disorder is likely larger. The disorder part may respond to medicines whereas the personality part will need therapy/ counseling. Usually, a combination of both medicines and counseling works best. After a proper evaluation, a personalized management strategy can be planned. Take care!

Anxiety can be of various types and can be part of one's personality, be a disorder in its own right or present as a part of other psychiatric disorders. There is no one best treatment for all of anxiety. SO we will need to see the exact nature and characteristics of the anxiety and come up with a personalized solution. Exercise, good sleep, meditation, breathing exercises, balanced diet and plenty of water are advisable. Medicines, counseling/ therapy may be needed. Please see a professional in person for a proper evaluation and management. I am aware that it is tempting to have a quick solution, but the reality is it is best to deal with these kinds of problems after a proper evaluation. Take care!

Hi, I'm Male 26 and I have been experiencing depression and anxiety since 5 months. I had a trauma in my life which I could not get out of my head. It was related to my career. I lost my job in Nov 2017 due to demoralization and that put me into shock because I did not have any backup at that point of time. I got so depressed and I even had suicide tendencies. I had serious health issues that time which included heart palpitation, vomiting, headache, anxiety, dizziness etc. I did not consult any Doctor for the same as I myself took control over all the problems. I used to get a feeling of being in such a rush like the world is gonna end the very next day. I was running after my career because I had lots of responsibilities on my shoulders. I'm engaged and I used to think about that a lot. I have gotten a new job now and I'm satisfied with it but I still get the weird feeling. I feel so low, I am unable to focus on things properly, I was the person who used to talk to people so much but now I don't feel like talking to them. I sometimes fumble while speaking. Please suggest what needs to be done to get it right. Will really much appreciate that. Thanks.

Many a time, these partially remitted forms remit completely on their own or in many, they tend to become chronic. The latter can get problematic. I am not able to judge myself what I am going through. I can't stop thoughts racing all over in my mind specially for my life. I become optimism as well as pessimism at the same time. I usually tend to behave in front of everyone like everything is ok but unfortunately whenever I stay alone I can't stop crying pathetically and even sometimes laugh at the jokes on which no one even gives reaction and again I am the only one who become like a foolish person ever. I become so much talkative and want to discuss on any topic with anyone to stay out of stress but no one is able to get awkward behaviour. I had sleepless nights from last three years but 2016 was worst of all. Effects on my health are ridiculous. I don't know how but whenever I got anxiety attacks I guess or whenever I cried a lot feeling lost and without any hope I suffered from constipation a lot. I start drinking excessive of water or eating a lot whenever facing any kind of fear. This year due to constipation and my other health effects like feeling tired always or oversleeping the attendance also became very less. Even I have a very bad habit of biting skin at the side of the nails while thinking curiously about some or the other thing or being nervous at most of the times and specially whenever I study. I want to get rid of all this nonsense. Cannot lead my life like this further anymore.

Hi, Thanks for writing in, welcome to the forum. I think there is an underlying current of mood instability. This could be seen in a variety of conditions including personality styles like borderline personality and in more severe forms, in bipolar disorders. It seems that your problems are mostly in the personality domain but it is very difficult to make an accurate statement without seeing you. I suggest that you should meet a psychiatrist at the earliest and get a proper evaluation done so that the problem can be sorted out. Do not worry! People have much worser problems than this and get better. I am quite confident that with appropriate help you will get better. Take care!

Hi, Thanks for writing in, welcome to the forum. If neither partner has HIV, HIV cannot be created by having intercourse. If one of the partners has multiple partners, one might want to double check if he/ she is truly negative by repeating the test after a few weeks. Rarely, the test may be negative whereas the person is actually HIV positive. But if the person is truly negative, there is no question of transmission. Take care!

I am suffering ocd from last 5 years, earlier 2 year my condition was serious, but in last two years I only have problem when I leave my medicine, though the mg of my tablets is reduced. Currently my treatment is going in Agra. But my question is, Is their a need of any other treatment for​ me so that ,I can leave my medicine, is there any therapy which I should take so that my medicine will be permanently closed.

Hi, Thanks for writing in, welcome to the forum. If symptoms are controlled with medicines one must always remember that in certain people a phenomenon known as discontinuation related resistance is seen; that is the same medicine that was effective earlier does not work after a period of stopping the medicine followed by re-emergence of symptoms. So, one must be very cautious when thinking of stopping the medicines. The form of treatment that comes closest to your goal and may in some people allow one to gradually withdraw medicines are specialized form of Cognitive Behavioral Therapy like exposure and response prevention, thought stopping, thought flooding, thought habituation and acceptance based therapies.
