    Most commonly known as a military neck a straight or forward curve of the neck is abnormal and may cause an unkind progression of symptoms leading ultimately to cervical disk degeneration.

    Reversal of cervical lordosis explained

    The anatomy of the neck features a lordotic curvature in its typical and healthy state. This means that the cervical region has a gentle curvature with the open end of that curve facing the rear of the body. The base and top of the curve will be further posterior than the mid point, which will be further anterior.

    When the lordosis is straightened, the neck becomes more upright and linear. This is more common than the next progression of atypical curvature, which is the subject of this article.

    Actual reversal of curvature means that part or all of the cervical spine develops a kyphotic profile, with the open end of the curve facing anteriorly. Usually, this reversal is extremely mild, but is still very abnormal. What we now see is the middle of the curve being positioned posterior to the top and bottom.

    In essence, picture the letter c and now turn it backwards: This is the shape of a reversed cervical lordosis.

    Reversal of cervical lordosis causes

    The spinal curvature in the neck is constantly in flux to some degree.

    Congenital conditions and developmental conditions can have lasting effects on the natural degree of curvature typically demonstrated from patient to patient. These circumstances may be explainable due to injury or degeneration, or may be idiopathic:

    Scoliosis can affect the normal lordotic curvature in the neck.

    Cervical spondylolisthesis is a major source of reversed lordotic curvature.

    Severe disc pathologies can facilitate a gradual loss or reversal of cervical lordosis.

    Vertebral irregularities, such as wedging, can definitely contribute to lordotic alteration.

    Traumatic injury, including vertebral fracture, can create the ideal circumstances for a reversal of lordosis to take place.

    Severe neck muscle spasms can actually reshape the spinal curves, although these are usually temporary expressions and not actual structural conditions.

    Effects of reversal of cervical lordosis
    The neck is designed to curve in order to balance the spine, absorb stress, distribute force and provide proper movement of the head. When this curvature is diminished or reversed, symptoms may result, although this is not an inherent part of any altered lordotic condition.

    Patients may experience stiffness and tension in the neck. Pain may be present and may even be severe in rare cases. Neurological dysfunction is possible in extreme cases, since the neuroforamen might not align properly, thereby causing a cervical pinched nerve.

    In the worst circumstances, central spinal stenosis in the neck might affect the viability of the spinal cord, possible enacting the most dire of symptoms throughout the body.

    Patients will also be more prone to injury, since the normal shock absorption qualities of the typical curvature have been lost.

    While all these effects are certainly possible, they are not usual. In fact, a great majority of patients have minor symptoms or even no symptoms at all from mild reversed lordotic curvatures.

    The pathology leading to a neck curve reversal (cervical kyphosis shown below right) may be inspired by a multitude of conditions as follows:

    Post head injury
    Stomach sleeping
    Poor sitting/working postures
    Congenital spinal curvatures
    Degenerative cervical discs (a form of osteoarthritis that can either be the cause of or the result of a cervical kyphosis)
    Compression fracture of vertebral body
    Infection of the cervical spine

    Anatomy: straight vs. Curved

    I've always heard that it was good to stand up straight.

    Are these expressions as familiar to you as they are to me? one might think that having a curved neck goes against what we heard from parents and teachers as we were growing up, but the reality is that there is a little bit a truth in both. Maintaining good posture throughout our lives is crucial to both the health of our spine and vital organs. On the contrary, a special type of curve called a lordosis is a good thing, both in the neck and lower back.

    When we look at a person from the back their spine should be truly straight, so that the left and right sides of one's body is symmetrical. However, when we view a person from the side, the front and back of their body is different and this is reflected in a coinciding curvature of the spine. Both the lower back and neck are hollowed out (concave) and the mid or thoracic spine is protrudes (convex). Thus there is an alternation of curves functioning to provide stability, shock absorption and aid in propulsion. A straight spine would be very stiff and not flexible. Imagine the plight of a pole vaulter with an inflexible pole.

    Nature's design of our spine and rib cage facilitates breathing and offers protective and supportive framework for vital organs. Spinal disks are shock absorbers and because they are in the front of the spine, lordotic curvatures keep them from having to bear weight. Kyphosis or loss of such curvatures bears weight upon the disks, leading to their ultimate degeneration. This process of deterioration is a form of osteoarthritis and in the spine is known as degenerative spondylosis.

    Although most physiotherapists or conservative orthopedists can recognize a cervical curve reversal upon viewing the patient's posture, a definitive diagnosis may be obtained via a standing lateral (side view) x-ray of the neck. Cause can often be determined by corroborating a comprehensive history, a thorough examination, x-rays and questions about sleep, work and lifestyle.

    In my professional career I found that the majority of young adults presenting with cervical kyphosis either had a whiplash or were stomach sleepers from an early age. For desk jockeys 40-60 years of age, many hours of sitting with their head flexed forward almost dictates the fate of developing kyphosis. In prior years I considered cervical kyphosis a job hazard for the careers of accountants, attorneys and often teachers because of years spent with their head in a book or paperwork. However, the digital age offers some relief in that respect. A well-planned, ergonomically-friendly office can do wonders for protecting the spine in the sedentary worker.

    Treatment for cervical curve reversal (kyphosis)
    During my chiropractic practice I had the opportunity to note a good percentage of correction toward a more normal lordosis (noted on x-ray) for 70% of patients under my care. This was almost always consistent with those patients that followed all recommendations and were model participants in their own care. Here is the recommended treat plan:

    Spinal manipulation of stiff and fixated spinal segments by a qualified physio
    Flexibility exercises for flexion and extension of cervical spine
    Resistance exercises for flexors and extensors of the neck
    Learn the Alexander technique for maintaining good posture (hint: the basic philosophy is to sit and stand like you were hanging by a string from the vertex of your skull. Liken it to a puppet on a string).
    Elimination of stomach sleeping
    Avoid standing on your head, although some yoga postures may be beneficial
    Use of orthopedic neck pillow while sleeping.
    What are muscle knots?
    Muscle knots are points within a muscle where contracted fibers are unable to release, or, as the dictionary puts it: highly irritable localized spots of exquisite tenderness in a nodule in palpable taunt bands of muscle tissue.

    A common problem for active people, muscle knots technically called myofascial trigger points, or mtps feel like a small knot to your fingertips. These knots can range from the size of a pinhead in smaller muscles to the size of your thumb in larger muscles. 2

    Muscle knots can cause pain in two ways: (1) latent trigger points, which are knots that only hurt when you put pressure on them, and (2) active trigger points, which are knots that actively refer pain along your neural pathways, causing it in non-localized areas.

    Trigger points in a constant state of contraction have excess metabolic waste and oxygen use because blood flow to this area stops this sends pain signals to the brain. Because your brain wants to stop the pain, it commands the muscle to rest, which leads to under-usage of the muscle. This is what makes the muscle shorten and tighten up.

    What causes muscle knots?
    The most common causes of muscle knots are:

    1) accidents acute trauma, such as bad falls sports injuries that strain you joints and muscles

    2) postural stress sitting too long with poor posture, sitting with no support, lifting improperly

    When do you need a knee replacement?
    When you suffer from severe pain or deformity in the joint (knee in this case), the pain or stiffness and deformity makes it difficult to perform simple tasks. Severe grade IV osteoarthritic knees of people over 50 years when have pain, swelling and deformity fail to respond to medicines, physiotherapy, injections and rest.

    Aim of the surgery

    A fracture is a complete or incomplete crack, which appears on a bone due to application of intense pressure or force. A fracture results in extreme pain, and the bone becomes immobile. Any kind of movement boosts the pain. Fracture pain occurs in three distinct stages. Acute pain is felt immediately after a fracture while sub acute pain occurs over the weeks that follow after a fracture. The third stage or chronic pain occurs when the fracture and soft tissues around it have healed.

    Fracture pain is quite intense in nature and proper precautions should be taken while dealing with them.

    Development of fracture pain
    When a bone undergoes a fracture, the ligaments and tendons are also damaged. While the bone slowly heals, ligaments and tendons fail to heal equally well or completely. Post fracture pain also develops when the outside of the bone does not heal completely. This usually does not appear in an X-ray.

    Treatment of fracture pain
    Modern medications can be utilized to treat a fracture pain. Steroid injections and prescribed anti-inflammatory medicines help in treating a fracture pain. However, these remedies cause side effects, which are harmful for the body. Cortisone shots are utilized for the same purpose as well.

    Prolotherapy is an approach where ligament and tendon strengthening is stimulated along with repairing. This technique ensures complete treatment of the fracture, and the risk of long-term complications like arthritis is absent. In cases of fracture pain, which occurs due to the incomplete healing of the outer part of the bone, Prolotherapy helps in strengthening the fibro-osseous junction at the source of the pain. This will stop the nerve endings from firing. Prolotherapy provides speedy recovery and accounts for making the injured bone stronger than before. This process is often undergone by athletes. The procedure is undertaken only after thorough examinations for detection of the root cause of the pain.

    Prolotherapy is considered to be the best treatment for repairing tendons and ligaments, which commonly cause fracture pain. The body is simply stimulated and the painful areas are repaired.     Get a diagnosis before anything else. It is really important to know what is causing your pain. See a doctor before you start doing any exercises or taking any medication. There are many reasons your hip could be in pain, including arthritis, bursitis, or an injury you've got while playing a sport. Always ask your doctor what you should and should not be doing, given the cause of your hip pain.

    1. Ice your joints: holding ice to your hips will reduce inflammation of your joints. You should hold an ice pack to the affected area for 15 minutes several times a day. If you find that the ice pack is uncomfortably cold, wrap it in a towel and then place it on the pained area.

    2. Heat your joints if you have arthritis in your hips: heating your joints can soothe the pain you feel. Consider taking a hot bath or shower, or soak in a hot tub if there is one available to you. You could also consider purchasing a hot pad that you can place directly on your hip. Do not use heat to soothe your joints if you have bursitis. Heat can cause hips affected by bursitis to actually become more inflamed.

    3. Get some rest: if you have injured your hip, the best thing you can do is to simply give your hip time to heal. Avoid anything that causes you to feel pain in your hip. Instead, grab an ice pack, a bowl of popcorn and watch some movies. You should give your hip a rest for at least 24 to 48 hours.

    4. Avoid high-impact activities: if you are in severe pain, chances are you won't feel like running or jumping anyway, but it is good to keep in mind that these activities should be avoided. High-impact activities will cause your joints to become more inflamed, thus causing your more pain. Instead of running, try taking a brisk walk, as walking has much less of an impact on your joints.

    5. Consider losing weight: the more your body weighs, the more weight your painful hip is burdened with supporting. Losing weight can help ease hip pain simply by removing some of that weight that stresses the cartilage and joints.

    6. Choose the right shoes: you should buy shoes that give you as much support as possible. Look for shoes that have great cushions, or have removable insoles so that you can add orthopaedics. The sole should have good shock absorption, should limit pronation (turning or rotating the foot) and will evenly distribute pressure along the length of your foot.

    Joints are intersections between bones; they provide support to your body during movement. There are a host of conditions which might lead to joint pain; they are bursitis, osteoarthritis, rheumatoid arthritis and gout. The most common joint pain disorders are knee pain, hip pain and shoulder pain.

    Ayurveda associates joint pain with loss of bone density in the joints. This problem is characterized by a feeling of discomfort and cracking sounds. If the condition is not treated, it might lead to immobilization of the joint. The condition of the bones tends to deteriorate as they do not receive enough nutrition.

    The other cause that leads to joint pain is toxin accumulation in the joints. As the toxin accumulates, it leads to heavy and stiff joints. If preventive measures are not taken on time, continuous accumulation of toxins may lead to swollen joints.

    It is that time of the year when insects are all around. Suddenly one feels a buzzing sound or an itchy feel in the ear. To your horror, one feels a live insect is in your ear, which can be extremely annoying and is like a nightmare. Insects often get stuck when they enter our ears as they cannot fly or crawl out. They try to find their way out and their movements inside our ears can be very uncomfortable, painful and can be itchy as well. Moreover, they can easily produce infection.

    In this scenario, do not try to remove an insect with cotton swab, tweezers or hair clip as it will make the situation worse and can lodge the insect deeper into the ear canal, which can possibly damag the eardrum, leading to permanent hearing loss. If an individual is not certain about the potential harm taht can be caused by the insect in the ear, one should seek medical care immediately. Insects in the ear are common reasons for visits to doctor's clinic, especially in children.

    If one suspects an insect in ear, one may experience pain, swelling, blood or crackling. One may even feel biting, stinging, hearing loss or dizziness. It is best to stay calm in this situation as being active may lodge the bug further in ear or cause it to move further back or cause serious damage to the sensitive eardrum

    One way to try to remove a bug in ear is by tilting the ear toward the ground and attempt to wiggle the ear. Grasp the earlobe and give it a wiggle. If the bug is not too far into ear canal, it may fall out on its own. If the bug is still alive and is not too far inside of the ear canal, it may simply come out on its own. If one stays calm and keep objects including fingers away from ears, it is likely that the bug will find its way back out of ear.

    One can also try to flush the ear with warm water with a dropper or a bulb syringe. This can be done by holding head upright and stretching the ear canal by pulling the outer ear and then putting a steady stream of warm water into ear. Tilt head to the side to drain out the ear. Do not try this if one suspects that ear drum has been ruptured to prevent additional damage.

    To avoid stinging or eardrum rupture from scratching or biting , one may use a drop or two of mineral, baby, or olive oil inside your ear canal to kill the insect. Finally, visit an ENT specialist doctor as they can remove the insect by special suction devices. Post insect removal, one must look out for signs of infection as swelling, dizziness, hearing loss, fever, and pain. Finally follow up with ENT specialist or Otolaryngologist for the final opinion.

    Frozen shoulder is a condition where the patient encounters continuous shoulder stiffness and pain. It could last for several weeks. Inflammation around the shoulder increases the inability to move or stretch properly. This happens when somebody is recovering from an injury. Frozen shoulder can also occur when somebody is wearing a sling or a cast. It also occurs if someone is recovering from surgery or experiencing joint pain. It may also be experienced as a symptom of thyroid, joint disorders or diabetes.

    Some of the causes and risk factors for creating a frozen shoulder are as follows:

The major occurrence of hip pain could be a recent injury or a previous untreated injury. It is also an indication of an injury or even a disease.     1. Hip Pain Causes: Pain can occur in any part of the body. One such part is hip. The hip is a ball and socket joint which connects the torso of the body to the leg. Shoulder pain may result from an injury to any of these areas.

    The various causes of shoulder pain are:
    1. Impingement: Impingement of the shoulder muscle occurs when the top of the shoulder impinges on the rotator cuff muscles. This condition can lead to severe pain in the shoulder.
    2. Bursitis: Bursa is a fluid filled sac that cushions the bones and help in preventing friction. Bursitis is a disorder that results in inflammation of the bursa, thus resulting in shoulder pain.
    3. Tendinitis: Tendinitis is a condition that is characterized by gradual wear and tear of the tendon. Usually the rotator cuff tendons in the shoulder are affected by this condition.
    4. Instability of the shoulder: Shoulder instability is a disorder of the shoulder wherein the upper arm bone dislocates from its socket. This can cause excessive pain in the shoulder.
    5. Fractures: A fracture in the bones of the shoulder results in excruciating shoulder pain.
    6. Arthritis: Arthritis is a disorder that leads to inflammation in the joints, thus resulting in pain and impaired movement of the joint.

    The treatment for shoulder pain consists of medications and physiotherapy. You may also be asked to discontinue any strenuous activity that requires shoulder movement. Medications such as NSAIDs are prescribed to help in controlling the pain. Various stretches of the shoulder are also recommended to improve shoulder flexibility and mobility.

    Do exercises such as the overhead press with light weights to improve your shoulder strength. Also make sure to include certain external rotation exercises to strengthen the rotator cuff muscles.

    From marathon runners to footballers, amongst athletes, 'pulling a hamstring' is the most common type of injury. The hamstring is a group of four muscles at the back of your thigh. When one of these muscles gets overloaded or begins to tear, it is known as a hamstring injury. While a hamstring injury is painful, it does not indicate the end of an athlete's career.

    Minor hamstring injuries may heal by themselves. In cases where one of the muscles is torn, surgery is required to repair and reattach the muscles. Recovery time depends on the severity of the injury. Stretching and exercising the muscles of the leg are the most important parts of rehabilitation. The healing of a hamstring injury can be divided into four stages.

    1. Acute phase: The first five days after a hamstring injury is known as the acute phase of rehabilitation. The aim of treatment at this phase is to reduce inflammation and keep the other muscles from atrophy. For minor hamstring injuries resting the leg, using ice packs and keeping the leg elevated can accelerate the healing process. Once the pain and swelling has decreased, patients can use a cane or crutch to exercise the leg.

    2. Subacute phase: From the fifth day to the 3rd week after the injury, the focus of the treatment shifts from minimizing swelling to strengthening the leg and increasing the range of possible motions. At this time, an athlete can resume cardiovascular training. Swimming and other forms of aquatic training are said to be especially beneficial to the treatment of hamstring injuries.

    3. Remodeling phase: The remodeling phase continues upto 6 weeks after the injury. Strengthening the leg is the focus of this stage of treatment. Using ankle weights can be beneficial towards strengthening the leg muscles. It is important to be careful about the weight being used and to gradually increase this weight to avoid an added injury.

    4. Functional stage: This stage can last upto 6 months until the athlete can regain full function of his or her leg. As the athlete's gait gets back to normal, they may start jogging and short sprints. Gradually, more sport specific exercises may be added to the workout.

    There is no fixed time when an athlete can return to active sports after a hamstring injury. While players with minor injuries may return to the field after 3 weeks, athletes with muscles tears may take upto 6 months to recover completely. Hamstring injuries tend to reoccur and hence before they can get back to active sports, an athlete must pass an isometric strength test and flexibility test. This ensures that there are no subtle after effects of the injury that can increase the risk of a repeat injury.

    It is believed that about 27% of all males and 3% of females can have a hernia during their lifetime. Factors like obesity, pregnancy, smoking, chronic lung disease aggravate the severity of the hernia.

    What causes it?

    When an organ residing in a cavity such as the abdomen tries to push through the muscular layer it resides in, it is called as hernia. Though said to be genetic, hernias can be caused by things such as improper heavy lifting, incorrect posture, or chronic constipation and as a result of surgical complication or injury.

    Types of Hernia

    Hernia can be an Inguinal Hernia, Umbilical Hernia, Hiatal Hernia or Femoral Hernia. It can also originate from an incision (Incisional Hernia).The Inguinal Hernia is commonly observed in males. Here, the intestine squeezes through a tear in the abdominal wall into the groin (Inguinal canal). At times, the small intestine bulges through the abdominal wall (weak spot) near the belly button, which results in Umbilical Hernia. In Hiatal Hernia, the upper part of the stomach passes into the chest through the diaphragm, and of all the hernia types, the Inguinal Hernia is the most common.

    Treatment of Hernia

    In case of Hernia, the affected area tends to protrude or bulge out. If left untreated; Hernia can prove to be detrimental. One needs to consult a physician for proper diagnosis and treatment.

    Physical examination can help in the diagnosis of Inguinal Hernia. An ultrasound is needed to diagnose Umbilical Hernia. A Barium X -ray or endoscopy can be of great help in the diagnosis of Hiatal Hernia.The severity of the Hernia depends upon its size. In case the Hernia is rapidly increasing in size, a surgery is needed for the repair.

    The surgery performed can be open or laparoscopic. In open surgery, Hernia is identified through an incision. Once located, the Hernia is removed from the adjacent tissues. In laparoscopic repair, small incisions are made in the affected region. Through these incisions, specialized instruments are inserted. It is through these instruments that the surgeon visualizes and performs the surgery.In such repairs, a mesh, held in place by sutures is used as a scaffold. This facilitates the growth of new tissues in the affected person. This technique significantly lowers the chance of a recurrence.

These have become very popular, although, are not a replacement of natural hearing. It is only prescribed for people who have severe hearing loss or are deaf.
    Hearing aids: although selecting a hearing aid is difficult as the patient needs to be comfortable, once selected it is one of the best solutions for the hearing impaired. The functionality of these devices can be extended with the hearing assistive technology or hat devices that can focus the audio waves depending on the situations and give a more tailored approach for every individual's needs.
