    There are women who feel their looks are spoilt because of the presence of excessive body hair. While hair on women's body is common, excessive body hair isn't a normal condition and must be taken seriously. Women are generally unaware that the condition is known as Hirsutism.

    What is Hirsutism?

    Hirsutism can be defined as growth of excessive facial and body hair in women. With the world obsessed with flawless skin, such changes from feminine hair pattern (soft, fine, vellus hair) to masculine hair pattern (hard, dark, terminal hair) in a woman is generally unwelcoming and, hence, very distressing for the woman.

    Why do some women develop Hirsutism?

    In order to understand the reasons for such excessive growth in some women, it is important to understand the differences in the physiology of men and women. The male or female appearance of one's body is dependent on a delicate balance between male and female sex hormones. There is a predominance of the hormone estrogen in a woman's body, whereas androgens (testosterone) are the predominant sex hormones among men. However, some amount of androgens is secreted by the ovaries and adrenal glands (located next to kidneys) in women also. But, an excessive production of these hormones results in Hirsutism. Hormonal imbalance is by far the most common cause of Hirsutism.

    Causes of Hirsutism

    The common causes of hirsutism include:

    1. Endocrine causes

    Polycystic Ovarian Syndrome (PCOS) - PCOS is a condition of hormonal imbalance in the body, which can result in an irregular menstrual cycle, enlarged ovaries with a cystic appearance on USG, infertility, obesity and hirsutism. For more information on the causes and management of PCOS.

    Cushing's syndrome - Excessive production of hormone Cortisol by the adrenal glands can lead to symptoms such as high blood pressure, abdominal obesity, skin discoloration, muscle weakness, irregular menstruation, and hirsutism etc. This syndrome is known as Cushing's syndrome.

    Congenital Adrenal Hyperplasia (CAH) - CAH is a condition of excessive production of hormones Androgens and Cortisol by the adrenal glands and generally manifests in childhood in the form of virilization. In such cases, CAH can present with symptoms such as ambiguous genitals, obesity, hypertension etc. However, sometimes the manifestation of CAH is deferred to early adulthood and results in milder symptoms, hirsutism being one of them.

    2. Tumors - Tumors in ovary or adrenal glands can again cause hirsutism due to excessive production of androgens

    3. Medications

    Long term steroid consumption (commonly used for immunosuppression, rheumatoid arthritis, and multiple sclerosis)

    Performance enhancing drugs (often used by athletes)

    4. Idiopathic - the woman may be having hirsutism despite normal hormone levels


    There are essentially two lines of treatment for hirsutism - one is medical management and the other is a cosmetic treatment.

    Medical treatments for hirsutism -

    A. Birth control pills - It's possible to reduce the severity and intensity of hair growth using birth control pills- the most commonly prescribed medical intervention! The birth control pills help by decreasing the production of androgens by ovaries. Besides this, estrogen present in the pills also helps the liver to produce more of sex hormone binding protein- these proteins bind to circulating androgens, thereby reducing their action on the body. Both these actions help in reducing the growth of hair on the body.

    B. Anti-androgen medications - There are three categories of anti-androgen medications, which are commonly used for the treatment of hirsutism.

    I. Spironolactone - Spironolactone is a diuretic medicine, which blocks the effects of androgens on the hair follicles, thus controlling hair growth.

    II. Finasteride - Finasteride works by blocking the conversion of body testosterone into its more potent chemical entity, thereby limiting the effect of testosterone on hair growth.

    III. Flutamide - Flutamide is a potent anti-androgen, which blocks the actions of androgens and helps in controlling the hair growth.

    However, we have to ensure that the woman is not pregnant when taking any of these three medications. These medicines are often combined with oral contraceptives, which also have a synergistic effect in reducing the severity of hirsutism. One should also seek an expert opinion before starting these medications as there can be serious side effects, especially on liver.

    C. Steroids - Low dose dexamethasone can be used to control the overactive adrenal glands when used carefully. However, long-term steroid therapy has got its own side effects and this therapy should only be considered under expert care and guidance.

    D. Insulin sensitizing medicines - Medicines such as Metformin and Pioglitazone (Basically these are anti-diabetic medications) help in reducing insulin resistance in the body, which, in turn, helps in controlling the effects of excessive circulating androgens.

    Cosmetic treatment for hirsutism -

    One very important aspect of cosmetic management of hirsutism is self-care. This can include shaving, waxing, use of depilators and bleaching creams. All these measures help to reduce the amount of visible hair in undesired parts of the body. But, this needs to be done very frequently for a sustained visible effect.

    Permanent hair removal is also possible using electrolysis or LASER treatment. In electrolysis treatment, electric current is passed into the hair follicle using a needle, which damages the root of the hair permanently. Similarly, LASER treatment uses LASER to destroy the hair follicle. Both these are effective methods to get rid of the undesired hair. However, we do not yet know if there are any long-term complications or side effects of these methods and these methods are not yet approved by the US FDA.

    Some kind of menstrual irregularity, often accompanied by pain in lower abdomen is one of the most common presenting complaints about a gynecologist.

    An irregular period is generally a symptom of some hormonal imbalance (estrogen and progesterone) in the body and is commonly encountered in young girls at the time of menarche ( a time when a girl starts to have the menstrual cycle) and around menopause. Menstrual irregularity can also be seen immediately after childbirth or after removal of intrauterine contraceptive device (iucd). Other common causes include thyroid gland dysfunctions, the presence of fibroids or polyps inside the uterus and excessive stress, including examination stress.

    It is important to know a few more important details in order to evaluate and understand the same and make any recommendations. These include-

    Duration of menstrual irregularity
    Kind of irregularity- the interval between two cycles, amount of blood flow and no of days of flow.
    Any associated pain in lower abdomen.
    Associated weight gain, a growth of excessive facial hair or discharge from nipples.

    Early assessment of menstrual disorders also requires an ultrasound scan of the lower abdomen to assess the condition of uterus and ovaries, specifically to rule out polycystic ovaries, and estimation of following hormone levels in the body - tsh, fsh, lh, prolactin and testosterone. These blood tests should be done on the second or third day of the periods.

    The women experiencing menstrual irregularities should try and manage their weight so as to maintain a bmi of less than 25 kg/m2.

    A healthy lifestyle including intake of a balanced diet, moderate exercise and avoiding any stress are very useful simple steps every woman can take on their own. In cases with severe irregularity, the woman may be required to get rid of the iucd, start a birth control pill to regulate the sex hormone levels during cycle and treatment for thyroid dysfunctions, if required.

    4 Dietary Changes that can Increase Your Chances of Getting Pregnant

    At times being able to conceive becomes a challenge due to certain biological reasons. Feeling positive and eating healthy naturally takes you a step nearer to getting pregnant. In case of no medical condition acting as an obstruction, consuming the right food can increase your chances of getting pregnant.

    Here?s a list of supplements you can include in your diet to maximise your chances of getting pregnant.

    1. Choose whole grains over refined carbohydrates: Reducing the intake of refined carbohydrates (found in milk, potatoes, corn to name a few) can reduce inflammation of the genitals, which will help increase fertility. You can still continue to have your favourite foods, but ensure they are low gi (glycaemic index- a figure used to represent the ability of carbohydrate to increase glucose in blood) carbohydrates.

    You can consume whole grains, which help in reducing insulin levels that in turn reduce the risk of heart diseases and diabetes and also help lower cholesterol levels. Try having whole grain bread instead of white bread and porridge instead of sugary processed cereals to reduce your gi ingestion.

    2. Harness the power of proteins: Proteins are an important part of your diet and the type of proteins you require careful consideration as they help make new hormones. Try having 25gms of vegetable protein like seeds, legumes and nuts instead of animal protein.

    Add dairy foods to your diet as they are also a rich source of protein. They provide vital minerals and nutrients like magnesium, calcium and vitamin b12 to the body and at the same time are low in calories. Include milk, yoghurt and cottage cheese to your meals so that you have a minimum of three servings of dairy products a day, which is essential to meet your nutritional needs and increase your chances of having a baby.

    3. Welcome good fats: Processed foods, fatty meats and takeaways are high in saturated fats including trans fats. These lead to inflammation and an increase of oxygen free radicals in the body, which decrease fertility. So, incorporate good fats in your diet in controlled quantities. Try having avocado and tomato on a whole grain toast, for instance.

    4. Make folate an integral part of your diet: Folate is an essential nutrient that helps in decreasing the incidence of birth defects in the babies, while ensuring a healthy conception. Along with the prescribed supplements, try to increase its intake in its natural form. The best way to do this is by having green leafy vegetables, which are a rich source of folate.     Modern lifestyle - minimal physical activity, unhealthy food habits and stress - has a negative impact on overall health for all of us. While we may not realize the negative impacts of our so called modern lifestyle in our daily lives, our lifestyle actually affects every function of our body and leads to many disorders and diseases.

    Infertility or subfertility among men is one such outcome of unhealthy lifestyle, and is on a rise these days. There are many biological factors which can affect a person's ability to father a child. However, in this piece I will only discuss the five most common and easily correctable lifestyle related factors, which affect the fertility in men. All these behaviors have the potential to adversely affect sperm production and/or maturation in men leading to poor count and/or quality of sperms. However, if addressed in time, these men can still regain their fertility potential and realise their dream of having a baby.

    1.Body weight: obesity has a direct correlation with reduced fertility among both men and women. Unhealthy eating habits, minimal physical activity and genetic predisposition are three most important causes of obesity. Its important to understand that while no one can really address the genetic predisposition, it really are the other factors that allow genes to take an effect. So, if one can ensure having a balanced diet and adequate amount of physical activity, he can still keep the body weight under check.

    2. Excessive caffeine intake excessive caffeine intake can affect the sperm motility, thereby reducing the speed of sperms and slowing down their journey inside a woman's body to reach the egg for fertilization. All this can be avoided by simply controlling the intake of caffeine. Please note that caffeine is found not just in coffee, but is present is big amounts in all carbonated and energy drinks. So, the trick is to avoid such drinks as much as possible.

    3. Smoking: Smoking leads to dna fragmentation in the sperms, which increases the risk of miscarriage. Smoking also reduces the sperm motility, thereby reducing its potential to fertilize the egg. Smoking in the woman partner further reduced the fertility potential as it damages the dna in eggs also. I cant overemphasize on the need to immediately and permanently quit smoking in both the partners. One cant wait till the time they plan a pregnancy for quitting smoking - they have to do it now!

    4. Alcohol consumption: its a myth that one can consume alcohol in moderate amounts and still remain healthy. Even so-called "moderate" alcohol consumption, generally defined as 30 - 60 ml per day, has a negative impact on fertility. Alcohol impact the sperm production process, thereby leading to reduced sperm count (called as azoospermia) and also affects the sperm maturation process, thus causing quality issues with sperms.

    5. Stress: Its no secret that we live in an increasingly complicated world, which gives us not just physical but also serious mental stress. Mental stress and tension create a cycle of negative emotions and affect all the cognitive functions in the human body. Stress doesn't allow proper erection, which is essential for a successful effort at intercourse. One should try and remain as stress-free as possible and can also try stress busters such as yoga and meditation for a healthier lifestyle.

    It doesn't require any magic or medications to help boost a man's fertility potential. These are all simple lifestyle measures everyone can manage on their own. All that is required is to allow the human body to work properly and then live a stress free life. Nature then takes its own course!

Therefore, it is very important for all young men and women to know the fundamentals of causation and detection of this important disorder.

    What is Obesity and how to confirm that?

    The most often asked questions in this context are- what is a normal acceptable weight, when does one get labelled as overweight and what is the definition of obesity? Another important question we often get asked is- what’s the difference between being overweight and obese?

    In very simple terms, what a physician evaluates to check for obesity in a person is a simple index known as “body mass Index” or BMI. BMI can be calculated by simply dividing the person’s body weight in Kg by the square of person’s height measured in meters. BMI is measured as Kg/m2. Following are broad guidelines for detection of obesity-
    1. BMI of < 18.5 indicates that the person is underweight
    2. BMI between 18.5 and 24.9 indicates that the person has got normal weight
    3. BMI between 25 and 29.9 indicates that the person is overweight
    4. BMI of > 30 indicates obesity

    How does Obesity affect fertility?

    Excessive body weight in women causes multiple hormonal imbalances, which can result in irregular menstruation and reduced frequency of ovulation, thus leading to infertility. Also, the outcome of Infertility treatment in obese women is poorer than in their normal weight counterparts. Obesity also increases the chances of pregnancy related complications such as miscarriage, pregnancy induced hypertension and gestational diabetes and hence increases the requirement offers cesarean delivery. And not to forget that the labor is that much harder and prolonged for obese women.

    Obese males also suffer from reduced fertility as they tend to have lower level of testosterone, which suppresses sperm production. Obesity can also case erectile dysfunction in men, which can result in reduced fertility.

    What can you do?

    1. Watch your meals- take a healthy diet and avoid excess carbohydrates and fat
    2. Watch your beverages- drink adequate quantity of water, avoid soft drinks as they contain empty calories and refrain from alcohol
    3. Stop smoking and keep in mind that there is nothing like occasional/ moderate smoking
    4. Exercise regularly and maintain a healthy, active lifestyle- short sessions of brisk walking, swimming, jogging or skipping are good means to keep one active and healthy
    5. Avoid stress and practice Yoga/ meditation or other stress buster techniques that suit you
