    Over the few years of experience in emergency medicine, I have experienced many cases of a time bomb called stroke. In majority of the cases, patients are not aware about the need for urgent action on this disabler. Improving this simple awareness in the community may save a person from being permanently disabled.

    Stroke types:

    1. Ischemic stroke (85% stroke cases) - this type of stroke occurs when an artery supplying the brain gets occluded by a clot which is usually dislodged from heart or from carotid artery plaque. This clot obstructs blood supply to a part of brain supplied by that artery. Blood is used as a transport medium for the nutrition and oxygen supply to all the organs. If the blood supply is obstructed to brain, the brain tissue starts dying and can lead to necrosis. Here, brain tissue can be prevented from necrosis if clot is removed on time.

    2. Hemorrhagic stroke (15% stroke cases): this type of stroke occurs when an artery in the brain ruptures due to previous aneurysm or accelerated hypertension and bleeding occurs inside the brain tissue. This stroke is managed in different ways depending upon location and extent of bleed. A patient might need a neurosurgery for this type of stroke. This type of stroke might have poorer prognosis compared to the earlier one.

    Act fast:

    Whenever you see the symptoms you need to act fast. To make things simple, we have a mnemonic to memorize: (as suggested by cincinnati stroke protocol)
    F- facial deviation (shown in picture)
    A- arm weakness - drifting of or inability to raise one arm when asked to raise both arms (shown in picture)
    S- slurring of speech - patient's speech is slurry as if his tongue has become heavy
    T- time - when you see above symptoms, note the time immediately and shift the patient to nearest stroke centre. Make sure you keep in mind that time is the key.

    In case of stroke, brain tissue starts dying and after its completely dead, there is no chance of recovering it as the brain tissue does not regenerate. According to american heart association guidelines we need to thrombolyse (melt the clot) within 3 hours of onset of symptoms whereas according to indian guidelines it should be done within 4.5 hours. Hence, time is given the utmost importance.


    The medicine used to melt the clot in case of ischemic stroke is called thrombolytic. One of the best aha recognised thrombolytic which is also fda approved is alteplase. The medicine is expensive and costs around 50000rs for 1 vial of injection. A patient might need 1-3 vials depending on his or her body weight. Hence, this treatment is still beyond reach for the low socio-economic class of india.

    Primary aid:

    1. Identify the symptoms using the mnemonic.
    2. Note the time of onset.
    3. Rush the patient to nearest stroke care centre.
    4. Alert the hospital on phone if possible
    5. Co-operate for faster management with treating doctor.


    A very small awareness amongst the population might save a person's permanent disability. Spread the word amongst your friends, relatives and acquaintances as you never know when this knowledge might save your loved one.
