From last 4 yrs, my stomach feel start anxious about something and it heads towards my heart and I feel like I can die at any moment because of heart attack or loss of breath. I always suffered back ache and neck ache. When I do certain exercises of breathing, I always feel breathlessness with shivering of hands and legs. When I rolled down my head exercise, I started feeling fainting and also fainting normally. I always fear of heights, snakes etc and when I heard a sound, my mind alerted me very fast and heartbeats started rapidly. Due to this, I also feel to commit suicide as no way of getting out of this. Excitement also leads me into rapid heartbeat. All the time, I feel tired and lonely. Also I feel difficulty in removing negativity and memory loss is quite regular. Is their any medication or some test or any special concern. Kindly help me out.

Hi lybrate-user, it seems you are suffering from panic anxiety. There are not tests for this since its entirely due to brain acitvity and no deformity. Its an anxiety disorder and sure shot curable. It has two modes of treatment first is medication for which you should visit a good psychiatrist second in case of anxiety and dissociation ((fainting)) psychotherapy helps a lot where in you will be taught to manage your anxiety through exercises and be master of your mind for which you can consult with me or any other psychologist. Do not worry suicide is not option, when medical attention is available to you. All the best. Seek treatment asap.

How's your concentration?

We all know what concentration means: giving your close, undivided attention to something.

But it's one thing to know what it means and another to be able to concentrate on your studies!

Many students complain that they just can?t concentrate, in other words, their minds race from one thing to another and their thoughts are all over the place - except on their studies. If you feel like this, you?re not alone. But what?s to be done?

Firstly, you should know that everyone has the ability to concentrate: think of a time when you were totally engrossed in something you really enjoyed, for example a movie, a book, a game of rugby or netball. You can concentrate: the trick is to use the right strategies to unlock your natural ability to concentrate and apply these to your studies.

Tips and strategies to improve your concentration

Choose the right place to study

Your choice of study space can influence your level of concentration. So, to promote concentration:

Choose a dedicated study space in an environment conducive to study.
Make sure you have a good chair, a table or desk, somewhere to store your books, a PC, adequate lighting, and good ventilation.
Make sure your study space is tidy, organised and a pleasant place in which to work.
Put a 'Do not disturb' sign on the door.
Leave your cell phone outside or turn it off.
If you like music in the background, that?s okay, as long as it?s not a distraction. (Research on productivity with music versus without music is inconclusive.)
Plan

Draw up a study timetable and stick to it.
Accommodate your day/night-time energy levels.
Get into a routine and make study a habit.
Divide your work into logical sections that have a beginning and an end: our brains are holistic and you?ll find it easier to work on something that forms a whole than something that's left hanging midway.
Set yourself a time limit before you start, for example: 'I?ll summarise Chapter 2 in 40 minutes'
- By doing this, you're setting yourself a goal and,
- when you set a time limit, your subconscious mind starts working on completing the task in the time available.
Study for about 30-45 minutes, review what you?ve learned, then take a 5-10 minute break. Why? Because research has shown that we
- remember best when we study for shorter periods, and then recap and consolidate what we have learnt, as opposed to longer periods when we have to struggle to stay focused and alert.
- learn better at the beginning and end of a study period. (Think of a movie: it?s often easier to remember the beginning and the end than the middle). So, take regular breaks and build lots of beginnings and ends into your study.
Focus

Before you begin studying, take a few minutes to think about what you?ll achieve.
Write down your goals for the study period, i.E. Summarise pages 40-65, complete the outline of Assignment 1.
Make sure you have everything you need: your notes, stationery, water, a healthy snack, etc.
Use active learning to keep you focused.
If you have a lot of reading to get through, try the SQ3R method.
Build in variety

Change the subject or study strategy every few hours ? this will lessen the chance of your becoming bored and stale.
Use your study break for exercise (or perhaps housework); this changes the pace and helps to get rid of extra adrenalin.
Alternate reading with more active learning exercises, for example: mindmapping or writing model answers.
Just say 'Stop'

Every time you notice your thoughts wandering, tell yourself to 'stop' and then consciously bring your thoughts back to your studies.
Repeat this each time your mind wanders, and re-focus.
Initially, you might have to do this many times each study session but with practice, you'll find that you are able to focus for longer periods at a time.
If you find it almost impossible to re-focus, it could be that you need a break:
- Take a five-minute break, have a glass of water, and try again.
- You could also try switching to another subject or topic, or using a different study strategy.
Don't waste your time and energy trying to stop yourself from thinking of something, that?s almost impossible. Instead, write your thoughts down on a piece of paper and put it aside to deal with during your ?worry time?! (See below.)
Schedule worry time

Allow yourself to worry but only at certain set times during the day.
Decide beforehand when and for how long you?re going to worry, for example: set aside ten minutes 'worry time' before Shortland Street (or whatever your favourite programme is). Then, when something distracts you while you're studying, or if you start to feel anxious about something during the day, write your thoughts down and set them aside, telling yourself you?ll deal with them during your 'worry time.
Remember:
It?s important to write your 'worries' down - it?s far easier to refocus on your studies if you know you won?t forget whatever it is that?s troubling you.
Stick to your worry time(s) and use the whole time you set aside. If you don?t have enough to worry about to fill the time, make a conscious decision to reduce the length of your worry time.
Keep a list of your worries ? if something keeps coming up, deal with it! Rather spend some time sorting the issue out than allowing it to keep distracting you and preventing you from reaching your goals.
Learn actively

Last, but definitely not least, to help you concentrate and remember, learn actively. Active learners do something with what they have learnt. They

put what they have learnt into their own words.
compare what they are learning with what they already know.
link new facts to what they already know.
apply what they are learning to their own situation, and
use the new information.

Sir; I am under treatment of sugar and bp since last four years. My bp is stand on 140 and sugar on fasting 120 and after meal 180 maintain even I am taking zee telmi-h for bp and glycomet gp-1 for sugar od. 0.5 mg thyrox tab and ecosprin. O. 75mg and one cap of omeg are regularrly. Kindly send your advise to take further action and oblige.

You r taking good medicines and your reports r just above normal you should go for hba1c test for your 3 months sugar control, after that we can advice you to increase or decrease your medicines.

Menstrual periods are often irregular during the first few years after menstruation starts. It may take several years for the hormones that control menstruation to reach a balance. Menopause or peri-menopause is the phase when a female again faces irregular menses. Pregnancy is the most common cause of a missed period. If you might be pregnant, treat yourself as if you are pregnant until you know for sure. Use a home pregnancy test as the first step to finding out whether you are pregnant. If you are not pregnant, other causes of missed or irregular periods include: Excessive weight loss or gain. Although low body weight is a common cause of missed or irregular periods, obesity also can cause menstrual problems. Eating disorders, such as anorexia or bulimia. For more information, see the topic Anorexia Nervosa or Bulimia Nervosa. Increased exercise. Missed periods are common in endurance athletes. Emotional stress. Illness. Travel. Management: Take more of green vegetables, nuts, salads, seasonal fruits, yellow coloured veggies, milk & milk products, plenty of water and enough of sleep & rest. I'm 19 (female). I'm frequently getting headaches and especially when I go out in sun. Being in india and now being summer I can't avoid sun and stay indoors. I'm afraid to take tablets for this regularly. Sometimes it's so painful that I can't sleep, eat or anything at all.

I am aged 64 years, pulse rate is in between 55-65. Blood pressure 80 to145 Not diabetic, I regularly walk in tread mill for about 5-7k. M /day. Thyroidectomy done about 12 years back. Taking thyroxine 100 mg daily in the morning. I am taking one lansoprazole cap 10 mg on alternate days before food for hiatus hernia. Not having any ill effect due to reduced low pulse rate. What is the cause of this anomaly? Should I take any medication?

Mr. lybrate-user, thanks for the query. Pulse rate of 55 to 65 per minute is absolutely normal. I do not understand why you have felt this as a problem. In healthy and fit individuals, more so who are regularly doing exercise or involved in some sports activities, pulse rate at rest is usually low. That only shows that the heart has extra capacity to supply more blood if needed in strenuous activity. However, it is better if BP is maintained around 130/ 80 mm Hg. Any idea what is the TSH level? If you are feeling fine there is no reason to worry. Thanks.

My TSH WAS 8.8 so doc made my thyronorm to 125 from 100 mcg. I got test repeated and it became 0.26 in 3 months. After 6 months level is 0.04. Should I continue with 125 MCg or I need to alter the dose. I have this problem from past 15 years I am 47 year old, uterus removed, I am under glucosamine (loss of synovial) from past 6 months, take revital women I am under no other medication. Weight is 52, height 5 ft Kindly guide.

lybrate-user, Thanks for the query. With TSH at 0.04 mU/L, you need to reduce the dosage by at least 25 mcg per day and then check it after 6 to 8 weeks. If the level is around 3 mU/L maintain that dose. Your other medications can continue. Thanks.

CONSTIPATION:is due to an incorrect lifestyle and irregular eating patterns, not taking enough fluids,sedentarylifestyle.Foods that are heavy and difficult to digest, as well as oily, spicy, fried, and junk foods are the culprits. Eating food in a disturbed atmosphere or in front of the television and being awake late at night all lead to bowel disturbance. Psychological factors like stress, anxiety, fear, jealousy, and sorrow also play an important role. DIET & LIFESTYLE ADVICE: Increase intake of fiber, especially fruits and cooked vegetables. An Apple or Pineapple/Pomegranate each day is helpful.  Drink 7-8 glasses of lukewarm water each day. Take daily morning and evening walks for at least 20-30 minutes each. Eat whole grains /nuts/sprouts and Muesli in breakfast Avoid refined foods like white flour, bread, pasta, pizza, white rice,or high protein foods like cheese,meat and cold foods like ice cream or chilled drinks  Take meals in a relaxed and calm atmosphere. The food should be warm and freshly -prepared, as cold food slows the digestive power. Take 30 ml/6 tsp of syp Duphalac thrice daily after meals with lukewarm water. Hello Lybrate User, If you want to gain weight, then it is very important that you do it right. Binging on soda and donuts may help you gain weight, but it can destroy your health at the same time. If you are underweight, then you want to gain a balanced amount of muscle mass and subcutaneous fat, not a bunch of unhealthy belly fat. Therefore, it is absolutely very important to eat mostly healthy foods even if you are trying to gain weight. Here are food groups and few tips to gain weight, follow this - * You need to eat more calories than your body burns in order to gain weight. Aim for 300-500 calories per day above your maintenance level, or 700-1000 calories if you want to gain weight fast. * Protein forms the building blocks of your muscles. Eating sufficient protein is required to gain muscle weight instead of just fat. *High-protein foods include meats, fish, eggs,  many dairy products, legumes, nuts and others. * In order to gain weight, eat at least 5 meals per day and make sure to eat plenty of fat, carbs and protein. * Don’t drink water before meals. This can fill your stomach and make it harder to get in enough calories. * Drink milk. Drinking whole milk to quench thirst is a simple way to get in more high-quality protein and calories. * Get enough sleep. Sleep for 7 to 8 hours. If possible take afternoon nap for half an hour.
