Hot fomentation x twice daily. Neck exercises. Neck stretching. Postural correction. Shoulder shrugs. Core strengthening exercises. Take frequent breaks at work use cervical pillow. Use back support. Self massage the back of neck.

Hi I have de quervian's tenosynovitis on my right hand. It's fine the whole day but it hurts in the night if I don't put on the wrist splint. Is this how it will have to be for the whole life? I have gone to a foctor who gave me some pain killers for a week and suggested complete rest of the hand for a week or so. That is not an option for me. Is there a way out of this other than a surgery?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. De quervain's tenosynovitis is inflammation of sheath of 1st extensor tendons due to which there is pain at the level of styloid process. The treatment depends upon the symptoms you are having, normally ice packs for local application, thumb spica splint for 3-4 weeks not only at night but all throughout, anti-inflammatory medication as per your suitability, local analgesic spray for application but no massage, ultrasonics and laser as physical therapy modalities are useful in many cases. But if it does not get better, then release is better, it is actually a daycare procedure which take about 8-10 minutes to do in ot under local anesthesia, so it is very safe procedure. Pain, paresthesia, numbness etc in thigh especially in outer aspect can be due to compression of a nerve which is called as meralgia, also it could be due to compression and symptoms from the back region. Initially you can start with: - physical therapy like tens - medication like pregabalin or gabapentin in small doses can be started and we can monitor the response - can also take an anti-inflammatory medication for some time especially if there is any pain associated. Let me know in around 1 week -10 days how are you feeling, we can get some tests to establish the diagnosis and also step up the medications as per the response. 5 Ways To Control Lower Back Pain

The lower back region of your body contains many important supportive structures to carry your upper body weight and also transfer the weight to the hips and the legs. This weight is primarily carried by lower vertebrae of the spine along with muscles and ligaments. There are many important nerves running through the spine which perform important functions of the nervous system. Lower back pain could be caused if any of the above is adversely affected.

Causes of Lower Back Pain

If you sprain your ligaments or muscles it could cause lower back pain. Curvature of the spine could also affect the nerves within the spine, pressing up against them, thus causing pain. Some of the common causes include poor posture, arthritis, fracture, improper lifting, or a ruptured disk among others. There are no variable symptoms associated with this condition but only one, which is pain in the lower back.

Tips to Control Lower Back Pain:

  1. Restorative sleep: Getting lots of sleep or rest is a great way to correct lower back pain, especially the ones caused by muscles or tendon wear and tear. Paradoxically, irregular sleep might aggravate your lower back pain. Make a strict sleep routine and stick to it. Make sure to get at least 8 hours of sleep and more if prescribed by the doctor to reduce the pain and let the body heal itself.

  2. Exercise is the key: Your lower spine is supported by the muscles residing in your back and abs. These muscles need to be strengthened through exercise, during the period of a normal day. There are a wide range of exercises which can be performed for about 30 minutes to keep your core muscles engaged and provide relief to the nerves of your lower back.

  3. Heat Therapy: Heat therapy brings nutrients and stimulates blood flow to the affected area. There are several forms of heat therapy like a hot shower, heating pad, heat wrap, hot water bag, or soaking the affected area in a hot-tub which provides warmth and heals your lower back.

  4. Cold Therapy: This therapy helps to reduce the pain to a great extent. It is defined as a process of ice pack application on the affected area which decreases inflammation by slowing down the nerve impulses. It reduces pain and by allowing you to rest also help in healing.

  5. Correcting your posture: It is very important for you correct your posture as spinal deformities arising from years of bad posture is a leading cause of lower back pain. Try to sit in a corrective posture and slowly increase the time that you can spent in that posture until good posture becomes your default. If you wish to discuss about any specific problem, you can consult a Pain Management Specialist.

Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day.

Hand splints should be used. She requires hot fermentation. Active movements of hand should be done otherwise there will be more stiffness. Squeezing smiley ball activity can be done, this is for hand strengthening.

1)DO NOT bend forwards.Bending forward does no good to anyone.It might twist your spine for all you know.So whenever you need to bend and pick up something,bend your knees first and not your back and pick up.A slipped disk (also called a herniated disk) happens when a disk between the bones of the spine bulges and presses on nerves,this is often caused by twisting while lifting.Hence,kep your back straight and bend at your knees. DO NOT bend over to lift anything heavy off the ground with knees straight.Lift heavy items with your legs, not your back. Do NOT maintain any one position for a prolonged period.If you have been sitting for an hour,take a break walk for sometime and then continue with your work. Same applies if you have been standing for a prolonged time,take a break and sit for sometime.Stretch your muscles and relax.The best way to stand for a prolonged time is the hotel bar position: one foot on the ground and the other foot on the brass rail, when rail is not available use any raised platform.Every 5 to 15 minutes, switch the foot you’re resting on the stool. DO not arch your back while exercising,watching TV,standing or sitting on a chair.Keep it upright.Avoid crouching postures. DO NOT move anything heavy by pulling it in front of you.Always push and never pull. DO NOT sit with crossed legs,sit with knees slightly higher than the hips.Keep your feet on the foot rest. ...etc

Weight gaining naturaly decreases back muscle strength. - avoid being overweight - do regular back exercises like bridging, knee to chest, prone on elbow. - Avoid any jerky movement to spine. - maibtain correct posture.

Hello Sir, I met with an accident 5-6 years back and from that time there is a pain in my right hand at wrist joint.
