I have diabetes mellitus. Specifically diabetic neuropathy as per consulted doctor. Can you please tell me the signs and symptoms of this. And I also take ecosprin. please give me detailed information about my condition.

MBBS, Diploma in Diabetology
Endocrinologist, Mumbai
Tingling burning numbness in feet are signs of neuropathy. Gradully it may lead to loss temp sensation, vibration and touch sensations. Claw fingers gradully appears as neuropathy progresses. Calosities, bunions appears as pressure points of feet changes. Calosities may lead to neuropathic ulceration. Ecosprin is blood thinner. Normally diabetics have extensive atherosclerosis and arteriosclerosis which diminish blood flow to feet. Diabetes patients have more viscous blood due to high sugars. Thick blood is more prone to get clotted. Keeping sugar levels under control and regular physical activity will help.

Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) - To Help You Live Your Dream!

MD - Obstetrtics & Gynaecology, DGO
IVF Specialist, Mumbai
Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) - To Help You Live Your Dream!

Of a million sperms released in one ejaculation, a single sperm manages to fertilize the egg, which grows into an embryo and then a baby. Due to various reasons, when this does not happen, it is termed as infertility and is currently on the rise.

With In-vitro fertilization (IVF), couples sometimes go through multiple sessions to ensure fertilizationICSI or intracytoplasmic sperm injection improved this chance of fertilization. It is where a single sperm is used to fertilize an egg in an artificially controlled environment and then injected into the uterus where it grows further.

The next step in this technique is IMSI, where the chances of successful pregnancy are further enhanced and also promises improved quality of the embryo. IMSI, as it stands for, has “morphologically selected” which means under a highly powerful microscope, the best sperms are selected and then used to fertilize the egg. Morphology stands for shape, and the shape of the sperm is a direct indicator of the sperm quality. This not just ensures good success rate but also ensures the sperm which is used produces a high-quality embryo.

Features

  1. High success rate of fertility
  2. Reliable and efficient method of ART (assisted reproductive technology)
  3. More expensive than traditional IVF methods or ICSI
  4. Requires complex equipment, training, and set-up

Indications:

  1. Useful in couples who have failed IVF previously
  2. Male infertility with unidentifiable cause
  3. Poor sperm quality

What to expect?

Both partners are prepared both physically and mentally prior to IMSI.

Before the procedure - men:

  1. A screening is first done to decide if the sperms can be used directly
  2. In case of hereditary diseases, a donor is preferred
  3. The first technical step involves sperm collection, either directly or through a donor
  4. Sperms are collected either through masturbation or through a small incision in the testicles
  5. Sperms used could be fresh or frozen; fresh ones may be stored for later use also

Before the procedure – women:

  1. The woman is put on some ovulation therapy to ensure the release of multiple eggs, so a good one is picked. These high doses of hormones ensure multiple eggs are released.
  2. Egg release is monitored through blood and urine tests
  3. They are collected within after 36 hours of release

During the IMSI procedure:

  1. A healthy egg is chosen and placed in a glass container containing hyaluronidase
  2. Sperms are placed in a medium which will slow their movement, which enables picking a good sperm for fertilization
  3. The dual advantage of this is that in addition to a high success rate of fertilization, the quality of the embryo is also assured

If you wish to discuss about any specific problem, you can consult a gynaecologist.

You have not given blood sugar fasting or post lunch test, with out which we will not be able to tell you regarding the your diabetic status. Your treating doctor might have seen it and has suggested you have sugar, so you have to believe him, otherwise with the reports you have given we can not conclusively say that you do not have sugar.

I am 61 years diabetic since 2005, till 2014 Dec I was under medication, my sugar level was near normal, but I do not want allopathy medicine, by any other treatment my sugar level will be brought to normal.

MBBS, DNB (General Medicine)

I understand your sentiments when you say that you want to control your diabetes without medicines. I want to tell you that it is possible. But only for those patients who are at a very early stage of diabetes which means that they have very low sugar derangement which can be handled by exercise and diet alone. Only way to know that is by trying dietary compliance and good exercise regime. After these efforts if your sugar gets in control then you can sure that you can avoid medicines. But remember two things. Checking of sugars should be regular. One time control does not mean consistent control. Second, if your sugars are out of control then don't keep avoiding medicines because your body is facing the bad effects of diabetes already.

Hello dear High triglycerides and cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Change life style .do meditation, yoga, Go walk daily, do any one physically activity daily i.e.play any game., Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take plenty of water in day time. And 1 glass of water before bed at night. use flax oil, omega-3 fatty meal or cap., take .green veg., whole grain. Garlic,. Cod liver oil. Take anti cholesterol medicines . tab. zyloric 100 mg once a day for uric acid.

MBBS, MD, CCEBDM(diabetology), CCMTD(thyroid dissorders)
General Physician,
For type 2 dm I can sugest you following dietary instructions 1. Do not take simple sugars like sugar, glucose, sweet fruits, sweet beverages, simply anything which is frankly sweet 2. Take a mixed oil diet which should include arachis oil, rice bran oil, saflower oil etc 3. You can take complex carbohydrates 4. Protein content should be normal etc in brief, take small &frequent meals (4hrly), take less rice or replace it with wheat, stop taking sugar, take poly unsaturated fatty acid (pufa) containing oil, and along with moderate excercise and proper medications. Diabetes is chronic disease and it does not have a definitive cure till date. But with proper diet and routine medication and follow up many diabetics have been able to live a normal and healthy life. Thank you.

I am 26 years ago old and I am suffering from diabetic can I get rid of diabetic.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, diabetes mellitus is occurring due to either insulin deficiency or resistance to its action or both. In case of t1 dm there is total absence of insulin. Both these conditions can only be controlled by proper treatment with drugs, diet control and exercise. There is no cure. Every one diabetes has to take treatment for life time and keep a tight control on blood sugar, or else there can be serious life threatening complications.

Hello it appears the insulin dose is not enough. Plus you have to check hba1c and lipid profile as well as creatinine. Then increase insulin dosage every 2 days by 2 units. Simultaneously observe sugar levels regularly. This will also help you to have better control. Also exercise regularly 1 hr a day.
