I am 46 yrs of age I find discomfort at my back . Drs suggest me to go for l5-s1 fusion surgery. Is the success rate good or any alternative treatment for this? please suggest me the better option.

Not all patients of backache & pain joints need surgery. Most of them can be managed without operation morning stiffness in back & other joints is quite a common condition. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take bio d3 max 1tab odx10days ibuprofen 200mg od & sos x 5days do back (spine) exercises contact me again if need be. Make sure you are not allergic to any of the medicines you are going to take if no relief in a week, then kindly visit me personally.

If there is no swelling or any other s/s of inflammation, I would advise you to undergo: - intensive physical therapy for strengthening of your periscapular muscles - adequate calcium and vitamin d intake which is important - a short course of gentle anti inflammatory medication like tab etoshine or tab paracetamol can also be initiated along with physical therapy - hot water fomentation at home and application of analgesic spray like dynapar qps spray for local application can be done after fomentation. But your mainstay of management will remain intensive physical therapy, so try to find out a good rehab & physical therapy center at your place. Hey doc, my friend has changed alot, she doesn't laugh alot this days and she isn't her same. Few months back she started self harming and continuously cried. It all happened after some personal issues. She doesn't feel like playing or talking, she would rather sit in her room and read stories from her phone. Sometimes she doesn't even eat food the whole day, she cries to sleep and she has given up all hopes. She doesn't know what to do anymore. What should she do? What happened to her? She is under depression. Try to find out the cause. She needs assurance and compassion.She needs professional counselling as the problem is somewhat difficult to be handled without professional assisstance at this point.Do not lose hope . Can Lower Back Pain Be Cured With Physiotherapy?

Lower back pains may underline a host of diseases, some of the most distinctive being the degeneration of lumbar disc and strain of back muscles. If left unattended, these acute durations of severe pain can lead to further complications, beyond the scope of both active and passive physical treatment regimes. Hence, it is always recommended to attend to these minuscule symptoms as soon as they are detected.

How physiotherapy works wonders in lower back pains:

  1. Physiotherapy is recommended when lower back pains are frequent, or stretch for days at a time. Most medical literatures, which shower their unanimous support in favour of physiotherapy for back pain, make note that physiotherapy relieves pain, as well as improves the functions, which are most likely to be affected by this disorder.
  2. Pain emanating, following a surgery, is recognized as one of the most significant aspects where benefits of physiotherapy are more visible. Generally, patients are advised to undergo physiotherapy sessions after surgical procedures irrespective of age and gender, unless the move is contraindicated in specific cases.

  5. The area of the body considered to be the core of lower back pain (lower back muscles and muscles of abdomen) requires more than just stretching or exercises involving machines. Workout focused on these core areas are based on a complex set of exercises, which may have to be improvised such that the aforementioned muscles absorb the maximum shock.
  6. In addition to these conventional exercises, which are based on applications of gravity, some centres offer a different environment for individuals who are unable to perform activities in normal conditions. The concept of aquatic pools addresses the issue, and makes exercising quite an easy feat for the physically challenged individuals. Hello everybody! I am Dr. Kaushik Reddy, arthroscopic and sports medicine specialist, working at the Apollo Hospital, Hyderabad.

Today will be talking about anterior cruciate Ligament injuries of the knee joint. Anterior cruciate ligament is one of the Major ligaments of the knee joint that provides rotational stability mechanical stability and proprioception and it is one of the most common ligament injuries which is affected in sports persons especially in people playing football, basketball, tennis, badminton it can also occur in people who have been Injured in road traffic accidents or recreational activities like dancing. So any patient while playing sports or during dancing if he experiences sharp Pain in the knee joint and pain while movement of the knee joint and if he hears a Popping sound from the knee joint if he feels any instability in the knee joint that is any giving way of the joint then you have to suspect that it would be an ACL injury of the knee joint. ACL injuries generally occur during sports whenever playing sports if there is any sudden stoppage from running or any direct blow to the knee joint during tackles or if there is any unsteady landing from a jump or any sudden shifts in the Direction can cause an ACL injury.

Whenever any ACL injury occurs during field or anywhere first medical attention needs to be given this is followed by rest and ice pack compression bandage and elevation of the leg needs to be done and once this first aid is given patient needs to be given Some analgesics and anti-inflammatory medication painkillers to reduce the pain and swelling and later after few days then he needs to visit an orthopedic surgeon where an orthopedic surgeon does a few clinical examination or test to diagnose whether it is an ACL injury then few investigations needs to be done to confirm an ACL injury like an x-ray to see if the injury of the ligament has occurred along with a bone fragment or an MRI to confirm the diagnosis of ACL injury whether it is a sprain or a partial ACL injury or a complete ACL injury.

The treatment for an ACL injury of where there is only a sprain of the ACL is usually conservative management where the doctor gives the patient a brace and he needs to be on Good physiotherapy and rehabilitation program in cases of complete ACL tear or injury conservative treatment doesn’t all good in that cases surgery needs to be done where an arthroscopic is ACL reconstruction needs to be done for these patients in which it is a small Dk surgery keyhole surgeries where the patient’s own hamstring tendons are taken and these are kept in the place of the ligament by making tunnels and fixed by various methods following an ACL reconstructive surgery the patient needs to be on good rehabilitation program and he can return back to sports within four to six months.

Hello, Kindly follow advises given below 1.Avoid using computers continuously, take rest for 5 min after working for half an hour 2. In pain free interval do neck exercise (kindly learn exercise by registered Trainer) 3. you can use thin pillow while sleeping so that normal posture of neck can be maintained 4.Maintain proper posture of body while watching TV/ studying Kindly go for cervical X ray AP and lateral and review with reports Consult Orthopeditian for further management

Back exercises. Many people go to physiotherapists with back pain. Here are two exercises that help to relieve pain and reduce stiffness in the back: •Dear, 1. Lie on your back with both of your legs straight. In this position, bring your left knee up close to your chest. Hold this position for 10 seconds. Return your leg to the straight position. Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

Steps Involved in Knee Replacement Procedure

Knee replacement is a procedure where the weight-carrying surfaces of knee joint are replaced surgically to ease the pain or any disability. People suffering from osteoarthritis, rheumatoid arthritis or psoriatic arthritis undergo knee replacement. All these conditions revolve around stiffness and painful knee. This surgery is usually performed on people aged over 50.

Surgery Types: 
Knee replacement is mainly of two main types:

  1. Total knee replacement where both the sides of knee joints are replaced
  2. Partial knee replacement where only single side of the joint is replaced

Procedure: In case of partial knee replacement with minimal invasion, a smaller incision, which is 3 to 5 inches, is required. This leads to minimal tissue damage and the surgeon can work between the fibres of the quadriceps muscles. Here, an incision through the tendon is not required. This may result in less pain, recovery time is reduced, and motion is better as scar tissue formation is less.

In total knee replacement, four steps are performed:

  1. Removal of damaged cartilage surfaces, which is at the ends of the femur and tibia, with a small quantity of underlying bone.
  2. Replacement with metal components, which help as a recreated surface of the joint
  3. Incision of knee cap with a resurface made of a plastic button, which is optional based on the case
  4. Insertion of a medical grade plastic spacer amid the metal components. This creates an effortless gliding surface.

After general or spinal anaesthesia, an incision of 8-12 inches is made in the front part of the knee. Joint part which is damaged is removed from the surface of the bones. The surfaces are then formed in a way to hold a metal or plastic artificial joint. The thigh bone shin as well as knee cap is attached to the artificial joint with either cement or a special material.

After Effects of the Procedure: After the surgery, patients may stay in a hospital for three to five days. Post surgery, notable improvement can be seen after a month or later. The patient is gradually relieved from pain with the construction of new gliding surface during surgery.

There will be slow progress in the movement. In the beginning, one may walk with a support of parallel bars and then with the help of crutches, walker, or cane. After full recovery in about six weeks, people can enjoy normal activities except running or jumping.

Presently, over 90% of total knee replacements function well even after 15 years of surgery. Hence, knee problem is no problem at all!

You may be having headache because of migraine/ stress or high bp/ other causes like sinusitis get bp checked avoid mental stress, regular physical exercise like morning walking and meditation and yoga avoid triggers for headache like direct sunlight further treatment can be done after physcial examination.

A good way to diffuse general body pains: 1.take a long hot bath in water in which a few cups of Epsom salts have been added. 2.If you suspect your body and pains are due to stress-get plenty of sleep and try to reduce or eliminate the cause of your stress. 3.Meditation, deep breathing and yoga.

Hello, There are few lifestyle changes like brisk walking daily, avoid intake of Yoga-Poses: Kalapbhati: This is a very effective form of pranayama. Sit down on the floor in a comfortable cross-legged position. Take a deep breath & then exhale quickly, while making a sound. Always remember while doing Kapalbhati – you have to forcefully and quickly exhale, but slowly and deeply inhale. Continue doing this for 10 times & then release. Ujjayi Pranayam: Sit down on the floor in a cross-legged straight position. Inhale slowly and deeply by both the nostrils. Try to make a hissing sound while you are sucking in the air. After that, stretch your inhale time for a while & then exhale as much as you can without giving any kind of pressure to your other body parts. This type of pranayama is very effective for those suffering from bronchitis, asthma & thyroid. Yoga Nidra (Corpse Pose) also works well in both hypo and hyperthyroidism in reducing stress and calming down. Foods to be avoided in Hypothyroidism: Hypothyroidism causes skin to become dry. Once it is controlled with tsh levels becoming normal, most symptoms will disappear. Carrots and beet root are fine, can be eaten. Only cabbage, cauliflower, broccoli, radish, mustard, peanuts, soy beans and soy containing products and coffee need to be avoided. For pain in joints: Apply Bakson’s Rheum Aid oil gently (2-3 times a day) and take orally Bakson’s Rheum-Aid syrup/ 1 tablespoon thrice daily. Also, increase your dietary intake of calcium which is present in green leafy vegetables, milk, milk products, sea food, white legumes, dried fig, black-eyed peas, almonds, oranges, seasame seeds. And vitamin D which is essential for absorption of calcium in our body and is synthesized naturally after getting exposed to sunlight, so early morning sunlight for 10-15min. 2-3 times a day is mandatory. Apart from this, do walk daily along with physiotherapy will also help you.
