Hi, have bilateral sub-total eardrum perforation with dry ears. Only once in a while right ear discharge that lasts only few hours without any pain past year the hearing and tinnitus was worse but now can hear well except that I miss hearing 1-2 words in a sentence or misunderstand a word or two sometimes and mild tinnitus mostly at night. Is it necessary to get the eardrum fixed or leave it as it is for the mean time. Also, can the hearing deteriorate? thanks.

MS - ENT(Gold Medalist), MBBS
Ear-Nose-Throat (ENT) Specialist, Delhi
First of all get an audiometry (hearing test) done that will give an idea about type and amount of hearing loss. And I am telling if you are having bilateral subtotal perforation then it is useless to take any medication bcz nothing can cure this. You should go for surgery otherwise your condition and hearing will worse with time on recurrent infection.

Please note the following for your condition: One particular type of chronic dizziness not related to vertigo (a feeling of turning or whirling usually associated with inner ear problems) -- “Patients with this syndrome have chronic nonspecific dizziness, subjective imbalance and hypersensitivity to motion stimuli, which are exacerbated in complex visual environments (e. G. Walking in a busy store, driving in the rain),”. Anxiety can also cause chronic dizziness Treatment: Deep breathing is one of the best ways to deal with dizziness. It will help provide an adequate amount of oxygen to the brain, which in turn will relax the nervous system and reduce dizziness.

Is there any connection between chronic sinusitis and severe hair loss. I have sinusitis since childhood. I am in my ...
Hair fall is mainly due to protein deficiency and other nutritional deficiencies and regarding sinusits main symptoms for maxillary sinusitis are thick nasal discharge, headache, watering eyes, forehead pain, recurrent infection of nose and mouth, post nasal drip, sinusitis, is nothing but a physiological disorder of nose which led by malfunctioning of aerodynamics of nose (temporary sinusitis, secondly it is because of repeated infections of the sinus (permanent sinusitis) sinuses regulations and air conditioning of the air which we breathe and to get the benefit of above we must have normal, well sized opened sinusitis and hence nose breathing is required, those who breathe from mouth, they have tendency to get more throat and lungs infections. Recurrent sinus infection, pollution, poor diet, spicy food, long term steam inhalation from nose, and allergies leads to thick discharge from nose. This thick mucus if stays in the sinuses, it leads to irritation of the sinus skin which is mucosa and this go on growing hence leads to permanent sinusitis this extra growth is called nasal polyps this is where patient needs surgical correction. If patient comes to nose surgeon at right time, doctor can revert his/her symptoms and surgery can be abandoned. Sinusitis if patient does not take serious steps to correct might lead to cardiac symptoms, lung diseases, etc. Once there are nasal polyps you have to go for surgery immediately.

Increase in Fungal infections of Ear with arrival of Monsoon Fungal infection of ear canal, also called as Otomycosis, is a superficial fungal infection of the external auditory canal. It is the most frequently encountered fungal infection in ear, nose and throat clinics. The organisms responsible for this clinical entity are usually fungi especially A. Niger. The fungi are usually secondary invaders of tissue already rendered susceptible by bacterial infections; physical injury or excessive accumulation of wax in the external auditory canal. The fungus tends to grow in humid, dark and warm areas and ear canal is one of the best places, especially in the rainy season. Excessive use of antibiotic ear drops may also be a cause of fungal infection of the ear. The fungal infection of the ear canal usually presents with a itching, irritation, discomfort, pain and scanty discharge from the affected ear. Patients also complain of feeling of blockage in the ear due to collection of fungal material in the canal. Sometimes irritation is so severe that patients tend to insert pins or sharp material in the canal leading to injury or bleeding from the canal. Pruritus and discharge are the most common symptoms which are usually unilateral but bilateral involvement is also been seen. There is greenish or black fuzzy growth on in the canal resembling wet ‘blotting paper with black dots’, which may fill up the entire canal. Careful drying and cleaning of external auditory canal is the first step in treatment, which is done preferably by suction evacuation by an ENT Specialist. The topical therapy with antifungal or other antimicrobial agents is also necessary. After cleaning the canal, antifungal drops, are given. Because the infection can persist asymptomatically, the patient should be re-evaluated at the end of the course of treatment.

Thank you for your query. 1. Almost all people at some point in their lives experience tinnitus. Tinnitus does not have a cure, however, it may be controlled. How severe is your tinnitus on a scale of 1 to 10? 2. Get your ears examined to rule out any ear wax which is one of the commonest causes of Tinnitus. Also rule out any perforation of the ear drum. Decreased hearing also makes one more aware of tinnitus. Get a PTA (Pure Tone Audiogram) done. If you have a hearing loss, amplification may help mask the tinnitus. You may also get your middle ear pressures checked (Impedance Audiometry or Tympanometry). 3. The exact cause of Tinnitus is unknown. However one of the the agreed pathologies is damage to or loss of to hair cells in the inner ear. The role of the brain in the perception of Tinnitus also exists. For example, some people have been relieved of Tinnitus after a cerebral stroke, while some continue to hear it even after the Cochlear (Hearing) Nerve is cut. 4. What is your age? What is your exposure to occupational noise and loud music / earphones / mobile phones? Exposure to loud noise is also a leading cause for tinnitus. 5. If your Tinnitus increases on clenching the teeth, it may require dental bite correction. 6. I have noticed patients who have increased Tinnitus when they are stressed, have fluctuations of blood pressure, low hemoglobin, low blood sugar levels and so on. 7. Cochlear Synaptic Tinnitus may be treated in up to 50% people with Caroverine infusion followed by capsules for two months. Other treatments such as transtympanic steroids, gingko biloba, multi-vitamins and anti-oxidants may also help in reducing Tinnitus. 8. Kindly let me know the results of your investigations. This will help in suggesting further treatment. 9. I must emphasize that in a vast majority of patients of tinnitus, the cause is never found. However there are further treatment options depending on the type of tinnitus. I hope that I have answered your query. If you have any further questions, I will be available to answer them. Regards.
