Spinal Cord Stimulation - How It Can Help Manage Pain?

Pain is a perception by the nervous system. When a painful stimulus is applied to a body part, the nerve endings of that part will send the sensation all the way to the brain where it is perceived as pain. If the pain is experienced by the legs or hands, then the sensation travels via the spinal cord to be perceived as pain. In case of people suffering from nerve problems, there could be cases of chronic pain, therefore conservative treatment measures may fail at times. In these people, though the pain source cannot be reduced, the perception can be reduced. This is exactly what a spinal cord stimulator does, it masks the signals that are reaching the brain, and thereby reduces the perception of pain. 

What is it and how does it work?

Spinal cord stimulator is implanted in patients with chronic intractable neuropathic pain when other conservative or surgical treatment has failed. Trial electrode/s are placed in the outer covering (epidural space) of the spinal cord, higher than the level of pain. Correct placement of electrode is confirmed by stimulating the electrodes through an external generator and different modes/intensity of current are tried to get best pain relief. Pain message is electrical in nature and reaches pain center in brain through multiple relays. Spinal cord stimulator converts electrical energy to a different feeling called neuromodulation and usually pleasant. Upon relief of atleast 60%, the device is implanted in the body like pacemaker for heart. An external remote device allows the device to be used when it is required. It is very safe and free from sideeffects of medications as no drug is used with this device.

Benefits of using stimulator 

  • Useful for people with prolonged case of sciatica and chronic regional pain syndrome
  • It reduces pain significantly
  • Improves a person’s quality of life by reducing pain (especially back and legs) which can be debilitating
  • Useful when other modalities of treatment have failed
  • Useful when surgery is not possible (either no response to prior surgeries or patient’s overall health does not allow for surgery)
  • Is a reversible option, the stimulator can be removed any time the patient feels it is not working
  • Requires minimal surgery and can be done as an outpatient procedure
  • Once trained, patients can manage the stimulator on their own
  • Is not affected by devices such as cell phone, microwaves and other electronic gadgets
  • Can be used on a trial basis before a patient opts for a permanent stimulator.

Sir, (1) For ACL surgery, which make screw and button your preferred? Which are the reputed make components/materials used in ACL surgery? I want every components used in ACL are reputed make. Kindly suggest. What ever the cost for same that I Can bear but I require reputed tools/components/materials. (2) What is the success rate of ACL surgery? Is it 100 percent? (3) I know the advantages of ACL but I also would like to know the disadvantages of ACL reconstruction/Repair? (4) After ACL reconstruction, the button or screw having noise or pick by metal detector? Will it create the problem during travelling? (5) If my one of Knee having ACL full thickness tear. If we doing regularly knee exercise then the load will transfer by strong mussels. If I am not a sport player and having only official work and routine occasionally travelling. Is it necessary for ACL reconstruction/Repair?

1. In ACL reconstruction latest is the all inside technique. Reputed make is Arthrex (UK) or Smith and Nephew (USA) endobutton and suture wheel is made up of titanium. This is a suspensory method of fixation of ACL graft. I personally prefer the interferential fixation with bio screws, which are made up of hydroxyapatite and are self absorbable, which gets incorporated into the bone, Reputed make is Arthrex (UK) or Smith and Nephew (USA). 2. Success is 100%. 3. ACL reconstruction frees you from instability and gives you a stable knee. Not much known side effects. 4. While travelling you will have to take your medical papers along as implant removal post reconstruction is not done. 5. If one lives a sedentary lifestyle then he may not need ACL reconstruction. But in cases of instability, even routine functions become troublesome.

I'm 24 years old and Acl reconstruction surgery in right knee last year .Now range of motion is 100 degree .knee alignment is not perfect .lower leg is around 3 degree outward. When I stand my knee falls a bit inward and toe goes out. Kindly guide me.

Dear lybrate-user, ACL is a ligament in our knee that prevents forward shift of our tibia over the femur, As you say you got surgery for ACL reconstruction, and now have problem with valgus deformity at the knee. It is quite possible that you would be having other injury that has been missed or your ACL laxity has occured, you need to consult a good Knee Surgeon, and get your problem corrected.

Kindly change your sleeping bed at once. Hard bed with firm plinth no sagging, and good soft matrss consult a physiotherapist in your city and learn spinal strengthing and mobility exercises.
