How Continuous Glucose Monitoring System Is Of Help?

With the increasing prevalence of diabetes, it is being considered as the next big epidemic – completely related to lifestyle. India has become the diabetes capital of the world with increasing prevalence. The age of onset of diabetes is reducing, with children having diabetes and requiring insulin. The complications arising out of diabetes are also more – there is no bodily system which is not affected by diabetes. For someone who is diabetic, keeping blood sugars under check is the constant challenge. Uncontrolled blood sugars can lead to transient complications including loss of consciousness and fatigue and long-term complications including neuropathy and retinopathy. 

What is CGMS: 

Continuous glucose monitoring system is an FDA approved device. This device has a sensor that is placed either in the belly area or on the arm. These sensors identify the sugar levels in the body fluid and send it to a wireless which is worn by the monitor patient as a belt. Readings can be obtained at regular intervals, ranging from every 5 to 15 minutes.

CGMS measures blood sugar levels every 15 mins, that is 96 times a day The data can be downloaded into a persona device – smartphone, tablet, or laptop and used for further analysis. It does not replace the need for routine monitoring, but helps improve vigilance with a constant monitoring. It can help detect trends and patterns and helps the doctor identify periods of the day when the sugars are too high or too low. The amount of insulin or antidiabetic medication required can be adjusted based on sugar levels. The exercise regimen can be defined including the type, time, and duration of workout. Meal planning can be customized to suit body needs in a better manner. Night-time sugar lows which often go undetected can be monitored with the alarm Highs or low between meals (especially with snacking) can be tracked Determine treatment efficacy. The biggest advantage of CGM devices is that they provide information on what is happening to your blood glucose level every few minutes.

CGMS measures blood sugar levels every 15 mins, that is 96 times a day. The newest devices display glucose readings on a screen so you can see – in real time - whether glucose levels are rising or falling. Some systems also contain an alarm to let you know when your glucose reaches high or low levels. Some devices are able to display graphs revealing glucose levels collected over a certain number of hours on its display screen. The data collected on all devices can be uploaded to a computer for graphing and further important trend analysis.

The Usage 

Continuous glucose monitoring systems (CGMS) may not make life with diabetes any easier. But they can definitely improve health, if you can deal with the hassle and expense. So how do you know if such a system is right for you? You can easily and discreetly view your current glucose values continuously throughout the day, without having to do a finger stick. It’s easy and discreet. The monitors have “trend arrows” that show you if your level is rising or falling quickly, so you can prevent highs and lows. 

Continuous glucose monitoring can help “identify fluctuations and trends that would otherwise go unnoticed with standard HbA1c tests and intermittent fingerstick measurements. The device can capture dangerously low overnight blood sugar levels which often go undetected, reveal high blood sugar levels between meals, show early morning spikes in blood sugar, evaluate how diet and exercise affect blood sugars, and provide up to a 72-hour complete review of the effects of changes made to your therapy by your health-care team.

I am 49 years old I have bp I take olmezest 10 mg when I take the tablet bp is getting down eg. 98/55 when I don't take the tablet bp goes high why is that happening please help me knowing. Please do an ABPM (24 hrs BP monitoring), that will give us an idea of years BP fluctuations in 24 hrs. Go for walking and avoid high salt intake.

You have to do an ECG first and if normal then do a 24 hrs Holter monitor plus an Echo .Then you can b treated accordingly.

Any BP less than 100 systolic is low and if it is less than 90 you should meet a physician or a cardiologist.

Hi doctor, My father age is 57 and his 2D echo report generated his results are IVSD 1.22 CM, LVPWD- 1.22 CM, LVISDD- 4.28 CM, IVSS-1.43 CM in the end they mentioned that mild concentrated LVH please can I know what is the problem, is it any danger please suggest.

This is called LVH (left ventricular hypertrophy) which is mild thickening of the heart muscle which may be due to long standing high BP. It's not dangerous but we need to control the BP if any and do an early Echo.

Cardiac Ablation - How To Prepare For It?

Heart ablation or cardiac ablation is a procedure done to treat irregular heartbeats or arrhythmias. It is performed by an interventional cardiologist, a doctor who specialises in doing procedures for heart problems and it involves threading long wires called catheters through a blood vessel into your heart. The irregular heartbeat is treated by delivering an electrical pulse using electrodes to specific areas of your heart.

Arrhythmias
A normal heart beats at a steady rate, but sometimes, your heart may beat too quickly, too slowly, or unevenly. These heart rhythm problems are called arrhythmias and one of the treatments for this heart problem is cardiac ablation. Other treatment modalities are medications and changes in lifestyle. Heart ablation is prescribed when the other treatments fail. Arrhythmias mostly happen in older people and in people who suffer from other heart problems like cardiomyopathy.

Who will benefit from heart ablation?

How to prepare for cardiac ablation?

If you are a woman and pregnant, you must not get this procedure done as it involves radiation. You will not be allowed to eat or drink anything after midnight, the night before the procedure.

How is heart ablation done?

Heart ablations are done in a special room known as an electrophysiology laboratory. It usually takes three to six hours and is usually done under general anaesthesia or local anaesthesia with sedation.
The catheters are threaded either from your neck, groyne or arm into your heart. Your cardiologist also injects a special contrast dye to help him see areas of abnormal muscle in your heart. He then uses a catheter with an electrode at the tip to give radiofrequency energy to the weak heart muscle to correct your irregular heartbeat.
You may have to stay overnight after the procedure.

Irregular Heartbeat - How Can It Be Treated?

Atrial fibrillation is a form of irregular heartbeat or arrhythmia, which you may experience due to a problem in the electrical system of the heart. Such a problem causes the upper parts of the heart or the atria to fibrillate. Due to this quivering, the normal rhythm between the lower parts of the heart and the atria gets disrupted. The ventricles are likely to beat faster in an irregular rhythm. This is a severe condition where the blood may get collected in the atria; this could lead to the formation of blood clots. These clots can block the blood flow and lead to a stroke.

Causes
Several conditions cause strain and damage to the heart. These include the following:

  1. High blood pressure or hypertension
  2. Heart attack
  3. Valvular diseases
  4. Coronary artery disease
  5. Medical problems like heart failure, lung diseases, high level of thyroid or pneumonia
  6. Heart surgeries
  7. Excess consumption of alcohol

Symptoms
The primary symptoms of atrial fibrillation include the following:

  1. Light-headedness and dizziness
  2. Shortness of breath
  3. Feeling weak and fatigued
  4. Feeling as if the heart is pounding, fluttering or racing (known as palpitations)
  5. Feeling that the beating of the heart is uneven
  6. Chest pain and fainting

In many cases of atrial fibrillation, the symptoms may be absent.

Diagnosis
Conducting several tests, physical examinations and an analysis of your health history is the first step towards diagnosing atrial fibrillation. An electrocardiogram (ECG) needs to be carried out for the detection of this condition. This test is performed for checking problems regarding the electrical activity of the heart. Other laboratory tests and an echocardiogram maybe required as well. An echocardiogram helps in observing the pumping function of the heart and to check whether the valves have been damaged.

Treatment
The treatment options for atrial fibrillation depend on the cause, symptoms and the risks of getting a stroke. Several medicines are used for treatment along with other methods. They are as follows:

  1. Blood thinning medicines for the prevention of a stroke.
  2. Heart rate control medicines which will prevent irregular beating of the heart.
  3. Rhythm control medicines for restoring the heart’s rhythm to normal.
  4. A process known as cardioversion may be used to bring the heartbeat to a normal rhythm. This can be carried out by medicines or an electric shock therapy known as electrical cardioversion.
  5. In case of severe symptoms, ablation may be carried out where the affected areas of the heart are destroyed by the creation of a scar tissue.

Coronary Artery Disease - Preventive Measures That Can Be Of Help!

The heart pumps pure blood to all parts of the body through a network of arteries. These are thicker in the beginning and become finer and thinner as they reach the various organs. These arteries are lined by a layer of epithelial tissues and as blood flows through them, the heavier cholesterol / fat molecules settle down along the walls.

This attracts more and more fat molecules to settle down.  This is known as atherosclerosis (hardening of the arteries). Over a period of time, the vessels circumference reduces and the blood supply to the target organ reduces. This impacts proper functioning of these organs and when this happens to the major organs like the heart, kidney or the brain, conditions like stroke or thrombosis or heart attack can occur.

This condition, known as coronary artery disease, is becoming a major cause of deaths.  While that is the bad news, the good news is that it is largely lifestyle dependent, and if steps are taken, it can be prevented, and in the early stages, the damage completely reversed.

Preventive measures:

1. Diet: A low-fat, high-fiber, heart-healthy diet consisting of Omega-3 fatty acids is recommended by doctors, especially to people who are prone to develop heart disease.  This also requires reduced salt, increased unsaturated fats, reduced triglycerides and reduced sugar.  Include loads of fresh fruits and vegetables, whole grains, nuts and seeds and fish oils. Include multivitamins or other supplements after checking with your doctor.

2. Exercise:  Regular exercise in any form increases the efficiency of the circulatory system, keeps the cholesterol levels in check and helps in blood pressure management.  Exercise in any form is advisable, based on individual preference. A moderate physical activity of 30 to 45 minutes per day is advisable.

3. Smoking: This is one of the major risk factors for smoking, and quitting or controlling smoking is one of the best methods to prevent coronary artery disease.

4. Alcohol consumption:  While moderate alcohol consumption is believed to be healthy for the heart, excessive alcohol consumption is a strict no-no.  Binge drinking especially is shown to cause heart attacks.

5. Weight management: Check with your doctor on what is ideal BMI for you and work out a plan to keep your weight under check.

6. Regular medications: If you are on blood pressure or diabetes medications, ensure you do not miss them.  Keep a constant check to ensure your readings are managed well.

7. Watch out:  Ask your doctor if there are specific symptoms that you need to watch out and seek medical support if you see any of them.

Coronary disease is not treatable fully, but can be prevented and managed effectively to improve the overall quality of life.

My dad, 70,had CABG in 2013, he was on Deplatt A 75 initially and then Deplatt 75 since Nov 2017. His platelet counts are around 45000 and hence doc had prescribed Ecosprin 75 in May 2018. However platelet is still around 44000? All other parameters like WBC, RBC, haemoglobin are within parameters. Is it proper to have Ecosprin 75 on alternate days?

First you have to meet a Hematologist to find out the cause of low platelet count. Both Deplatt and Aspirin are antiplatelets .He needs further blood work up to ascertain the cause and accordingly he can b given antiplatelets. Alternate day aspirin will not be the answer.
