Hello sir, I am 19 years old female. From last 5-6 months I have been having excessive mood swings, for one moment I feel like I am the happiest person alive and for another moment I am the sides person in the whole world. Also there are times, when I just hate each and every human being. Then I don't to talk to anyone. All I want to do is sleep despite sleeping for 12 hours at stretch. Most of the time I feel worthless. It seems nobody cares whether I am dead or alive. I don't feel connected to anyone. And from about last 15 days I don't feel like eating anything otherwise I never skipped a meal. I don't know what to do. Please help me out! Sorry for such a long question.

Dear lybrate-user, From what you have mentioned, it seems that you are suffering from symptoms of Obsessive Compulsive disorder (OCD). OCD causes symptoms of repeated thoughts, in your case regarding contamination. It could also be doubts. Due to these thoughts, compulsive repeated behaviour results, in your case repeated cleaning and hand washing. In case of doubts, it could be repeated checking behaviour. OCD is a treatable condition. Medications and psychotherapy in form of exposure and response prevention will help you recover from all the symptoms. Hello doctor! It's been couple of years I got in depression and undergone doctor and after 6 months of medication and treatment I got fine and doctor told me that I am absolutely fine and stop the medicines as there is no need. And yeah I am absolutely fine but I got addicted to medicine maybe. I gradually reduced it from 2 to 1/4th dose daily but I am unable to completely stop it. If I don't take it, I feel vomit, irritation in head, feel so low that I can't even walk, difficult to recall things in my mind, memory issue and don't feel good too. What should I do. My husband is also upset with this thing and saying me to stop it as it will lead to bad side effects in future while pregnancy ,liver etc. Please help me by telling what's the problem, is this the addiction symptoms or what? .And also let me know the side effects of these type of medicines for myself and for future pregnancy and on baby too?

Dear Ms. lybrate-user, I went through your query in detail and can understand what you must be going through. It's definitely very frustrating when we are not able to quit any medicine even after we feel that we are fine. You haven't mentioned the name of the medicines that you are taking and this makes it explaining exactly what you are going through a bit difficult. For depression, most doctors use a combination of an anti depressant and a benzodiazepine sedative. The right course of action is to taper and stop benzodiazepine after a few weeks and continue the anti depressants for few months. Benzodiazepines can be addictive. If they are not tapered and stopped in few weeks, there are chances that we become dependent on that and develop withdrawal symptoms when we stop it. This is the most probable cause in your case. The other scenario where it is the antidepressant which you are still taking means that you haven't completely recovered from depression/anxiety and will need further treatment depending on actual clinical condition. Benzodiazepines definitely cause adverse effects in pregnancy and we try to avoid it completely in pregnant patients. The best way forward will be to seek a psychiatric consultation and start treatment in order to quit the medicines. A substitute could be started and tapered off gradually. Mood swings bother you and everyone around you!

Everyone experiences disappointment or sadness in life. When the "down" times last a long time or interfere with your ability to function, you may be suffering from a common medical illness called depression. According to research, women experience twice the rate of depression as men, regardless of race or ethnic background. An estimated 1 in 8 women will face major depression in their lifetimes. Despite high prevalence rates people are reluctant to acknowledge clinical depression as a disorder that should be evaluated and treated.

Researchers suspect that, rather than a single cause, many factors unique to women's lives play a role in developing depression. These factors include - genetic and biological, reproductive, hormonal, abuse and oppression, interpersonal and certain psychological and personality characteristics. However, the stages in a woman's life can also subject her to depression- menstruation, pregnancy, childbirth and menopause. Given below are some of the signs and symptoms that you may be experiencing related to your life stage.

Premenstrual Dysphoric Disorder - This affects around 3-8% of the women. In this behavioral changes are observed in women 5 days prior to the commencement of their menstrual cycle. The common symptoms are:

  1. Lack of interest in common activities, such as hobbies

  2. Moderate to severe feelings of tension or anxiety with low energy

  3. Mood Swings, such as feeling overly sensitive

  4. Moderate to severe feelings of hopelessness or depression, feeling overwhelmed

  5. Physical symptoms, such as weight gain, joint or muscle pain, or a headache

  6. Changes in sleeping patterns, such as sleeping too much or too little

  7. Changes in eating habits or food cravings

  8. Problems focusing or concentrating

  9. Unusual irritability or anger

Management:  Be calm and relaxed. Keep a check and monitor your cycle and emotional outbursts. If you notice some continuity it's important that you consult a specialist. Ignoring the issues can have an effect on your relationships. Be open and talk to your family and your partner about the same.

Pregnancy and Post Partum Mood Disorders- Pregnancy is a stage in which a woman's body is going through a number of hormonal changes. These changes can have a major effect on the mood and can increase the risk of developing depression. Even post pregnancy the risk doesn't end. The following are a list of symptoms. Be informed if you are pregnant or have just delivered a child.

  1. Lifestyle or work changes resulting in relationship problems

  2. Episodes of depression and irritability

  3. Feeling of lack of social support and lost feeling

  4. Mixed feelings about being pregnant

  5. Feeling that pregnancy has pulled you down

Please note that a woman can feel depressed during pregnancy and the feeling can become worse post child birth. It affects 10-25% of women in India. It's important to understand that it's not just you but your body that is going through change. Hormonal fluctuations, psychological pressure and social support all attribute to the same. Be open and consult specialists. Also, talk to your family and your spouse.

Peri Menopause and Menopause Depression- Risk of depression may increase during the transition to menopause, a stage called Peri menopause, when hormone levels may fluctuate erratically. Depression risk also may rise during early menopause or after menopause - both times when estrogen levels are significantly reduced.

Women need to aware that they are vulnerable to develop depression and need professional help to avoid it from causing personal and functional impairment.

I am having depression since one year and on medication for the same. I am taking 5 mg of clonafit plus at the bed time. Next half day I feel gud but after that a severe pain in chest occurs. Problem in breathing also occurs for seconds only. I got tensed and scared of anything now a days unable to do household work. Sometimes I talk to myself and start crying. Dear Ms. lybrate-user, I went through your query in detail and from what you have mentioned, it is clear that despite being on medicines, you are still having the symptoms of depression. The medicines that you have mentioned consists of two salts, an anti depressant Escitalopram and a sedative Clonazepam. Youu haven't mentioned how long you have been taking this tablet. If it has been more than two weeks, had I been your psychiatrist I would have considered increase in dose of the antidepressant. The best way forward will be to talk to your psychiatrist and discuss with him the option of increasing the dose. You can also consider adding psychotherapy sessions which will help you recover. Feeling tired, lack of concentration, feeling sad, anxiety, sleep disturbances, headache are all symptoms of depression. There are multitude of reasons for the same. Over time, any partner can gradually lose interest in sex. I also noticed that you have mentioned your age to be 29 and your husband's to be 46. It is quite possible that the age difference might be contributing. However, in your case, since its only 4 months, the best way forward will be to talk to your husband directly about it. Being a businessman, he just might have become slightly busier or may be under stress. But you will never know until you talk to him. Considering the fact that your Echo and ECG are normal, anxiety /depression seems to be the most likely cause. You should seek a psychiatric consultation to undergo complete evaluation.
