Fistula is not curable without surgery. You can go for Seton bya surgeon. Seton means tyinga thread.

Operation is final treatment for the 3rd stage piles. It can recur if constipation continues or straing at defaecation occurs.

If there is pain during defaecation it is anal fissure. Bleeding after defaecation is piles. Piles are usually pain less. Confirm your diagnosis with a surgeon.
