Macular Degeneration - 3 Most Common Types!

MBBS, MS - Ophthalmology

Macular Degeneration - 3 Most Common Types!

Even something as small as an eyeball, has multiple parts within it. The retina at the back of the eye ball is responsible for the clarity of vision. The central area of the retina is known as the macula. Macular degeneration is the deterioration of this part of the retina. Macular degeneration is age related and considered an incurable condition. People suffering from macular degeneration experience blurred vision, black spots and may eventually lose central vision while retaining peripheral vision.

There are two types of macular degeneration; wet and dry. The dry form of this disease is more common than its wet form. Of these, the latter causes more serious vision loss. Dry macular degeneration leads to white or yellow deposits on the retina leading to further degeneration. In the wet form of this disease, blood vessels beneath the retina start growing towards the macula and may pull it away from the base when they break or leak fluid.

There are three stages of age related macular degeneration (AMD).

- Early AMD

This is diagnosed by the presence of deposits on the retina. In most cases, ether is no vision loss at this stage but regular check-ups are essential.

- Intermediate AMD

A comprehensive eye exam will show the presence of larger deposits or pigment changes in the retina. At this stage, slight vision loss may be experienced.

- Late AMD

People suffering from late AMD have noticeable vision loss.
Though this disease is linked to aging, the exact triggers for macular degeneration are unknown. It is understood to be a combination of hereditary and environmental factors. The presence of certain genes and their variants has been associated with a number of cases of this disease. Studies also show that caucasions are at the highest risk of suffering from this disease. Depriving cells in the retina of oxygen can also increase a person's risk of contacting this disease. Other risk factors for this disease are obesity, smoking, high blood pressure and a light eye colour. The side effects of certain drugs can also induce this condition.

Macular degeneration is considered to be incurable but certain forms of treatment can improve vision and slow down the rate of deterioration.  Treatment prescribed by a doctor depends on stage of the disease and whether it is wet or dry. Studies suggest that a diet rich in omega 3 fatty acids can prevent AMD and lower the risk of its progression.

Eyelid Surgery (Blepharoplasty or Eye Lift) - Things Your Should Know!

Eyelid Surgery (Blepharoplasty or Eye Lift) - Things Your Should Know!

Eyes are believed to be the most attractive feature in a person. A pair of beautiful eyes can melt millions of hearts. Skin ageing and its consequences can throw life in jeopardy. Sagging eyelids are one such unwanted consequence. With ageing, the skin loses its elasticity to a great extent.

There is an increased skin accumulation around the upper and lower eyelids. As a result, the eyelids become sagging and droopy. Sagging eyelids can be annoying, affecting a person's overall appearance and self-confidence. In extreme cases, it interferes with the normal vision of the affected individual. Eye irritation is also common.

Thus, the problem needs to be treated at the earliest. Blepharoplasty or the eyelid surgery comes as a huge relief. Here, the surgery is mainly performed to enhance an individual's appearance. To treat sagging eyelids, it is important to get rid of the accumulated skin, loose muscles and fat pads. Procedure is done under local anesthesia.

For upper eyelids, the surgeon makes small punctures along the line of the eyelids. These punctures act as the exit points for the unwanted skin, muscles and fats. Following the removal of the skin present in excess, the punctures are carefully stitched. In case of the lower eyelids, the fat pads are removed through an incision made inside the eyelids. The excess skin can also be eliminated through an incision made along the margin of the eyelashes.

My question is that is it true that surgery in one eye can cosmetically cure my appearance of squint? As if one eye remained as it is then the the eyes on which surgery is not performed will look abnormal.

Amblyopia - How It Can Be Managed?

MBBS, MS - Ophthalmology
Ophthalmologist, Ghaziabad
Amblyopia - How It Can Be Managed?

Lazy eye or amblyopia is a childhood disorder wherein the child’s vision does not develop properly, especially in one of the eyes. If the condition is not treated at the very onset, it can lead to impaired vision in the concerned eye. As the child grows up, the brain will fail to pick up images from that eye.

Causes:

This disorder is known to start when one eye has better vision than the other eye. An example would be if one of the eyes is affected by farsightedness (unable to see objects at a closer range) and the other is normal. In the case of a child, when the brain receives images from both the eyes, it will ignore the faulty eye’s image. When this situation repeats itself over months or years, it might lead to absolute deterioration of vision in the faulty eye.

Strabismus is another cause of amblyopia wherein there is a significant misalignment in the eye; one of the eyes may turn outward or inward. This does not allow the eye to focus properly on an object, thus leading to double vision. The brain again ignores the image from the faulty eye that causes vision deterioration in that eye.

Treatment:

The treatment begins by correcting problems in the affected eye and then putting a patch on the normal eye. Initially, the child may have difficulties adjusting to this new method, but bear in mind that it is absolutely necessary to wear the patch. The entire treatment process may range over a few weeks to even months. Regular visit to the doctor is of utmost importance.

In mild cases, an eye drop called ‘atropine’ may be prescribed for the good eye. The child may also require glasses to improve focus. In case of cataracts, a surgery might be needed to treat it.

The treatment should begin early, as it can get difficult to treat the condition once the child crosses his/her 7-8 year mark.

Refraction - Factors That Result in Abnormal Results!

Refraction - Factors That Result in Abnormal Results!

Refraction is a test for eye examination, which measures prescription for eyeglasses or contact lenses for individuals. The test can be performed by either an ophthalmologist or optometrist or in layman terms, an eye doctor.

Procedure for the test
In this test, the individual sits on a chair, which has a special device attached to it, called the refractor or phoropter. The individual has to look through the device and properly focus on an eye chart which is exactly 6 meters or 20 feet away. This device has lenses of various strengths that can be moved into the individual’s view. The test is conducted on one eye at a time. The doctor will be asking the individual about the chart whether it appears clearer or less clear, thereby determining the refractory error of the eye.

No special preparation is required for this test and also this test does not cause any discomfort. The purpose of the test is to determine whether the individual has a need for glasses or contacts or not. In other words, it checks if the individual has a refractory error or not.

How can the results be inferred?
If the vision without glasses is normal, it means that the refractive index is zero and the vision should be 20/20. This means that one can read a one-centimeter letter at a distance of six meters.
If an individual has a refractive error, a combination of lenses will be required for a 20/20 vision. In case of a refractive error, there would be a prescription and that would have in it a series of numbers describing the power of the lens required to correct the vision.

Abnormal results can be due to the following reasons

  1. Myopia or near-sightedness (capability to see near objects clearly, but having difficulty in seeing objects far away)
  2. Hypermetropia or far-sightedness (capability to see far away objects clearly, but not the nearby objects)
  3. Astigmatism (blurred vision due to abnormally curved cornea)
  4. Presbyopia (inability to see nearby objects clearly, which usually occurs due to age)

Other reasons that cause abnormal results can be

  1. Macular degeneration causing loss of sharp vision
  2. Corneal infections or corneal ulcers
  3. Retinal detachment, which means separation of the retina from its supporting layers
  4. Retinitis pigmentosa or damage of the retina
  5. Blockage of a small artery carrying blood to the retina or retinal vessel occlusion

One should have an eye examination conducted every three to five years even if they suffer from no serious eye problem. However, after the age of 40, one should schedule eye appointments every year and have a test for refraction error of glaucoma conducted.

When you use the phone or computer, one should blink more often. You can use some lubricating eye drops like genteal eye drops four times a day. This would help in providing the needed wetness in the eye please use these drops and tell me about it do you have any other complaints like poor vision etc.

It is common for many eyes to have such dots and that's not a cause for concern in most eyes. However, if they have appeared recently and/or you also experienced flashes of light in any eye, you should get your Retina examined by a suitable doctor, esp because you have -6 myopia. If your Retina is normal, just ignore them.
