Hi I can understand your problem. For weight gain take healthy and balanced diet along with regular exercise. For leg pain. *take rest *increase fluid intake *gently stretch or massage your leg muscle. * apply heat to tight muscles or cold to tender muscles. * there are various treatment options in physiotherapy which depends on your health and severity of your symptoms. Kindly reply for private consultation to discuss detailed treatment plan. Wishing you good health.

I am diabetic & my fasting yesterday was 152. I take glocoryl m1 fort before break fast & 1 tab zyoral m4 forte before dinner. I am now experiencing pain in my right ankle joint. Please advise.

MBBS, MD - Internal Medicine
General Physician, Delhi
Dear . As per most guidelines a fasting blood sugar of less than 140mg/dl is desired. However I would not panic on one reading above 140. What is your latest hba1c levels? is the ankle pain less than 4 weeks? in that case the ankle pain can be due to any of the three conditions- trauma -infectious arthritis or gout.

Multimodal Pain Relief After Total Knee Replacement

MBBS, MS - Orthopaedics, Fellowship in Joint Replacement , M.Ch - Orthopaedics

Multimodal Pain Relief After Total Knee Replacement

Knee replacement can be extremely painful. Previously, opioids or narcotics were administered for pain relief. But excessive addition of opioids is not exactly effective for controlling pain.

Multimodal pain management has become an important part of the perioperative care of patients undergoing total joint replacement. The principle of multimodal therapy is to use interventions that target several different steps of the pain pathway, allowing more effective pain control with fewer side effects. Many different protocols have shown clinical benefit. The goal of this review is to provide a concise overview of the principles and results of multimodal pain management regimens as a practical guide for the management of joint arthroplasty patients.

Multimodal denotes administering two or more than two types of medications that work with different mechanisms. The following are the techniques used:

  1. Pre-operative Femoral Nerve Block: Prior to the surgery, a catheter is placed beside the femoral nerve for blocking it. This nerve is located in the upper thigh. Medication is delivered through the catheter for the nerve to be numbed for 24 hours. Thus, pain signals to the brain are blocked. This method reduces the use of narcotics and the consequent side effects.

  2. Patient Controlled Analgesia (PCA): This method is also known as ‘Pain Pump’. An intravenous pump is used to administer pain relief medications, such as oxymorphone or morphine, after the surgery. The control button of the machine could be pressed, by the patient for 6 to 10 times per hour. The machine is used for two post-operative days.

  3. Oral Medications: The oral medications include Non-Steroidal Anti-Inflammatory drugs or NSAID; such as Celebrex which is similar to aspirin, structurally. Alternatively, acetaminophen, such as Tylenol or its equivalent composition, can also be used.

  4. Acetaminophen: It acts on the Central Prostaglandin Synthesis and relieves the patient of pain through multiple mechanisms.

  5. Epidural Analgesia: It produces lower pain scores and involves less time for achieving physical therapy goals. However, this is subject to side effects such as dizziness, urinary retention and itchiness.

  6. Gabapentinoids: These medications include membrane stabilizers such as Gabapentin and Pregabalin.

The objective of multimodal treatments is to provide quick relief to the patient and immediately so. Earlier the rehabilitation, more successful will be the knee replacement surgery. If you wish to discuss about any specific problem, you can consult an orthopedist.

Pain & Tenderness In The Hand Bones - Know The Reasons Behind!

Pain & Tenderness In The Hand Bones - Know The Reasons Behind!

Are you experiencing pain and tenderness in your hand bones? Hand pain can occur due to several causes that may include diseases and injuries. Most of the causes of pain in the hand bones are treatable, and the symptoms can be eased. The most common conditions that cause pain and tenderness in your hand bones include the following:

De Quervain’s tendinitis

  1. This condition causes pain on your wrist on the side of your thumb. The pain develops gradually or suddenly and may travel the length of your thumb and forearm.
  2. Movements such as making a fist, turning the wrist, and holding objects may be painful.
  3. The pain occurs because of swelling of your wrist tendons that are present at the base of your thumb. This happens on the account of inflammation and irritation.
  4. Overuse and repetitive activities lead to this condition.
  5. New mothers are at a higher risk of getting this condition because of the position in which they have to hold their babies, and due to fluctuating hormone levels.
  6. Wrist fracture increases the chances of D e Quervain’s tendinitis.

Arthritis

  1. Arthritis is a very common condition that is associated with hand pain or pain in the hand bones.
  2. It causes your joints to lose the cartilage, slowing them to move against each other slowly.
  3. The deterioration of the cartilage causes pain and swellings.
  4. The areas in your hand that get affected by this condition include the base of your thumb, end joint located near your fingertip, and the middle joint of your fingers.
  5. The most common form of arthritis is known as osteoarthritis, which leads to the progressive degeneration of the cartilage. It occurs because of ageing, and because of injuries such as dislocation and fractures. It causes swelling, stiffness, and swelling in your hands.
  6. Another form of arthritis that affects your hand bones is called rheumatoid arthritis. This is an autoimmune disease in which the cells lining your joints get attacked by mistake by your own immune system. It leads to stiff, swollen, and painful joints. Hand pain associated with this condition is aching and throbbing in nature. The pain worsens in the morning and after a period of being immobile.

A Ganglion

  1. This is a fluid-filled swelling which develops around a tendon or joint. A ganglion resembles a soft and smooth lump.
  2. It is made of synovial fluid which surrounds your tendons and joints for lubrication during movement.
  3. These are most likely to occur in your wrists, fingers, and hands.
  4. It is recommended for you to consult a doctor on experiencing hand pain of any form, and at any location on the hands. This will allow proper diagnosis so that you can follow specific treatment measures.

LOW BACK PAIN AND HOMOEOPATHY

LOW BACK PAIN AND HOMOEOPATHY
Long term use of painkillers for low back pain is harmful

When it comes to treating low back pain, there is too much reliance on painkillers in the allopathic system of medicine. The effect of these is only temporary and lasts for a few hours. Once the effect wears off, you have to take another painkiller. Prolonged use of painkillers is known to cause detrimental effect on the kidneys.

Homeopathic treatment for low back pain

When it comes to the medicinal treatment, homeopathic medicines are very helpful. They not only treat the acute stages but are also very helpful in the chronic stage of back pain. It must be said here that homeopathy offers a much better alternative for the permanent cure for low back pain. The focus of the homeopath is not just to relieve the immediate pain but to also eliminate the root cause of the pain. That is precsely why the homeopath asks so many questions which may or may not seem related to your immediate problem. A common cause that is easily overlooked is the presence of stress in an individual. Mental stress can also contribute to low back pain and should be taken into account while taking the details of the patient. This is absolutely necessary if you want a lasting cure. In my practice, I have treated hundreds of cases of low back pain.

I am male ,sir I have continuous back pain from 1 month ago when I stand or sit more time ,no pain when lying on bed, what is the reason for this sir.

BHMS

I am male ,sir I have continuous back pain from 1 month ago when I stand or sit more time ,no pain when lying on bed,...
Please note that there could be several reasons for your back pain - injury/strain/standing/sitting too long at a stretch/disc prolapses, etc. I suggest you follow the below for your condition: take mag phos 6x - thrice daily for next 2 weeks and update. Also do x-ray of the affected area. Heat and ice packs. Heat and/or cold therapy helps relieve most types of low back pain by reducing inflammation. Often patients use ice, but some prefer heat. Both may be used alternately.

Lower back pain you can do it this exercise, 1 Lying on your back, bend your right knee into your chest and place a strap or rolled-up towel around the ball of your foot. Straighten your leg toward the ceiling. Press out through both heels. If the lower back feels strained, bend the left knee and place the foot on the ground. Hold for 3-5 minutes and then switch to the left let for 3-5 minutes. 2…. Lying on your back, bend your knees into your chest and bring your arms out at a T. As you exhale lower your knees to ground on the right. Keep both shoulders pressing down firmly. If the left shoulder lifts, lower your knees further away from the right arm.
