Dear lybrate-user, You might be genetically prone to get a psychiatric disorder. Probably that was precipitated by 'joint' What you are suffering now, are due to the original disorder - NOT withdrawal of ganja. This psychiatric disorder if treated properly, you will do your job like before. Your brain is NOT damaged due to ganja. But it is altered due to a treatable psychiatric disorder. Please consult a psychiatrist. She/he will diagnose you properly and treat you. Sometime the treatment may go on for months though the brain is NOT damaged permanently. All the best.

This is for My father of Age 75 ,weight 43 kg and height 177 cm. He is under treatment for dementia related issues as described below. We are consulting with Neurologist and he was taking the tablet Donep of 10 mg for the last one year. Now he added one more medicine since there is no improvement and stills showing more angry and aggressive behavior. The medicine name is QUETIAPINE 25 mg. One tablet night after food When am browsing in internet it is not approved to older people with dementia related issues. It may cause serious issues. Is that correct? Or shall we continue with the medicine? New prescriptions cilacar 10 morning one Telma h 40 morning one Donamem 10 night one quetiapine 25 night one Atorfit cv 10 night one 1) He is having serious memory issue 2) He is not getting words while talking 3) He is able to read, write, Identify all the friends but always angry with everyone most of the time 4) some day he is very silent and normal 5) Most of the time he is getting he want to go from home, no answer where he want go, but someday he used to go to some of his friends home. Some days to temples. May be frequently 6) Some days he will bath/ brush two or three times. If we are reminding this he will get angry and will try to go out of home As per Neurologist it is because of one silent brain attack and dementia He is a retired Mathematics teacher, and this symptoms started some 4 year back. Now some days he is very violent because of the above reason Waiting for your feedback.

MBBS, DPM
Psychiatrist, Bangalore
Der Dileep, Sorry to hear your father, Maths teacher, has dementia. The behavioral problems are not tolerated by the family members. If you start attending the family members' group, you will understand why your father gets angry and leaves home. If the family understands his present level of memory and accepts it as such, behaviour of your father will change. If you people point out accusingly or treat him as though he does it purposely, it will worsen. Donepezil is helpful in reducing behavioural problems. None of the antipsychotics are approved in dementia because it can cause falls and medical problems become worse. You have to decide which is worse and use Quetiapine whenever necessary.

I was used to smoke cigarettes 2 months back twice or thrice a week but from last two months I had stopped taking cigarettes. And next month I have to go abroad hence I want to remove nicotine from my body.

MBBS, DPM
Psychiatrist, Bangalore
Dear Vasu, Congratulations for quitting smoking. You have managed without any withdrawal symptoms so far. Please continue to motivate yourself to say "NO" to cigarettes. Nicotine does not stay in the body. So when you go abroad, it will not be detected. If you are with others who smoke, the smell will stick to your cloths. It is also for sometime - definitely NOT for 3 months.

Dear Karthik, Smoking causes nausea and lack of appetite. Many persons smoke when they can not afford food. It depends on how long you smoke and how much you smoke. Since yours is only months, this is the time to say "NO" to smoking. It will be more difficult later. Apart from relief from weird taste and nausea, stopping smoking has more health benefits. You will be able to enjoy your food better.

Dear lybrate-user, Sorry to know about your brother. MRI brain will not reveal psychosis or depression which are biochemical changes in brain. But MRI checking will motivate your brother to go and consult a psychiatrist. Use it, as a pretext. Ranchi has the best central institute of mental health. The psychiatrist there will diagnose and treat him properly. Let not psychosis or depression spoil his future. All the best.

Dear lybrate-user, The disease depression has diurnal variation. That means, they are worse in the morning and by afternoon they improve. Some days they are fine and some other days they are worse. All these are without any major cause. Tiredness and no interest in anything also are symptoms of disease depression. It is a TREATABLE disease with antidepressant tablets and therapy by any psychiatrist. Some trained physicians also will treat her with antidepressant tablets.

Dear lybrate-user, All your fear, anxiety and panic seem to be due a serious mental disorder. Your psychiatrist is treating them with the tablets that you have mentioned. Do not stop or reduce or change these medicines without consulting the psychiatrist. You can do pranayam in addition with these tablets.

Dear lybrate-user, Try educating your friend about depression this way. Both depression and anxiety are disorders that need treatment. A psychiatrist will be able to treat them after diagnosing the disorder. Suppose, you have temperature of 102* instead of 98.6*, don't you go to a doctor? Like that if depression and anxiety are much higher than what is normal, you should consult the doctor, psychiatrist. As you know, so many disorders can raise your temperature, like that so many disorders may increase your depression and anxiety. The psychiatrist will find out the cause - it may be a hormonal problem, it may be a neurological problem, it may be a bipolar disorder, or it may be a drug side effect. After diagnosing, the psychiatrist will treat accordingly - both depression & anxiety and underlying cause. Please inform all your other family members and friends about it. Just like talking about fever, let us talk about our feelings also. You need to consult a psychiatrist who will request few blood tests to find out other age related medical diseases. If they are NORMAL, he may prescribe you with a tablet of antidepressant for few months. The dose will be changed according to your improvement and side effects of the tablets. It might take 3-6 weeks for you to improve and be active like 2 years back. Still you need to take those tablets for many months. Please note that there are many antidepressants that do NOT increase sleep.

Respected sir, I have developed cluster phobia recently. Unable to travel in flight and AC compartment of train for l...
Dear lybrate-user, They are specific phobias. They are treated by cognitive behaviour therapy or systematic desensitisation. They need many sessions over 5-6 months time. For tomorrow's travel, it can not be done. You can try with short acting anxiolytics temporarily. Please try to stop smoking at the earliest. It has many health benefits in the long run. When you stop suddenly, you may get withdrawal symptoms like restlessness, irritability, lack of concentration, increased hunger, sleep disturbance, headache, impatience, anxiety, depression, etc. If you have them please don’t get back to smoking, take nicotine gum or lozenge or patch. They are available in all medical shops. You don’t need a prescription from doctors. Use 2 mg gum if you smoke less than 25 cigarettes/day. Use 4 mg gum if you smoke more than 25 cigarettes/day. First 6 weeks, use 1 gum every 1 – 2 hours. Reduce it in week 7 – 9 to one gum every 2 – 4 hours. In week 10 – 12, reduce it further to 1 gum every 4 -8 hours. By 3 months, you will be able to stop both cigarettes and gum fully without any withdrawal symptoms. Please feel confident that you can do it and give yourself some gift for doing it like vacation or books or new mobile. Smoking is a chronic relapsing condition like diabetes and B.P. So please don’t worry if you restart smoking. Start all over again to stop. You will learn the situations that make you smoke and avoid them. Please think of health benefits and money saved often. Plan how to avoid relapsing situations, like smoking friends, along with alcohol, stressful feeling or depression. Delay, drink, deep breathe, distract and discuss are the helpful tips. Increase of non smoking zones, gift yourself for stopping smoking. Even chewing cinnamon may delay. Please remember that the urge to smoke comes and goes like waves. Delay and see whether it goes away. It will like a passing cloud.

Dear lybrate-user, Dementia is an old/middle age problem. Memory loss at your age, will NOT be due to dementia. Probably your neurologist is right in saying, it is depression. He is NOT hiding dementia. After 2-3 weeks of Stalopam (antidepressant - not sleeping pills - take it in the morning itself) and betacap, get back to him. He may adjust the dose depending on your memory abilities and fluency. Or he may refer you to a psychiatrist for better management of your preoccupation of dementia.

Dear lybrate-user, Please write in detail about the medicines that you are taking. Mercury amalgam does not have any effect on schizophrenia or sleep disturbances. If you are finding any improvement with medicines, you should not think of get out of medicines. Job is a must at this age of 32 years. It is not for earning alone. It is to keep your brain occupied. If your sleep is disturbed, please consult your psychiatrist again, he/she may diagnose and treat it.

Hi, I'm a acute transient psychosis patient diagnosed at the age of 14. Initially my dosage was very high now im 25 years old consuming 500 mg Divalporate and 50 mg Quitapin plain. Im good when I do not have any relapses and very effective. I completed B. Tech Mechanical with 60 % and got placed in a company as well. Right now im doing software courses. I have few questions: 1 .When I will get rid of my medicine? 2. With these dosage if I can act like a average person so I wonder if im not consuming these medicines I will be more effective with work as well as studies but my relapse will happen in a year or sometime after 2 years. So, is there any alternative for these medicines? 3. I visited many doctors, some say you are suffering from psychosis disorder, some say bipolar or schizophrenia, some say sleeping problems. Why this many doubts in my case. So, what exactly im suffering from? Though I consumed many dosages initially but when I was reviewed with a doctor (Psychiatrist) He advised me the doctors are giving high dosage it can be reduced to minimum. Since then im in contact with that doctor and that is how my dosage is reduced but I want to know whether it can reduced or is there any need of these medicines. I suffer from relapses in a year, sometime 2 years. I will appreciate if any psychiatrist clears my doubts. Thank you.

Dear lybrate-user, Whether you have transient psychosis or bipolar or schizophrenia or sleeping problem, it is for the psychiatrist to diagnose and treat you properly. With this dose you are without relapse and acting like average person. Why do you want to lose those effects of drugs. Bipolar disorder is a relapsing disorder - sometimes with high overconfident mood sometimes with low self esteem sad mood. Both will be kept under control with valproate and quetiapine. During active phase you may need different dose and during normal maintenance phase you will need different dose. Leave the dose decision to the doctor. Each relapse at this age will stagnate your future - mania because you will lose your name, fame and money - depression you will your energy, interest and may lose life by suicide. Why do you want to have relapses? Continue valproate and quetiapine as prescribed.

Dear lybrate-user, Alcohol acts as an antidepressant at certain dose - whatever is the drink. The amount of alcohol in that drink is important. But alcohol causes tolerance - it means after some time the same amount of alcohol will not give the antidepressant effect. You may need more. In the course of time, it will give depressant effect instead of antidepressant effect. If you feel like stopping it, you get severe bodily and psychological withdrawal effects. A double edged blade. So, think before choosing alcohol as an antidepressant.

Dear lybrate-user, Please motivate her to seek psychiatric help. Allow her to talk her feelings and give value to her feelings. After consulting the psychiatrist if antidepressant is prescribed, encourage her to take the tablets and counsel her to wait patiently for tablets to work on her brain. It may take 2-3 weeks. Don't ask her to forget family disputes. She too wants to forget - but it comes automatically to her. Please remove sources of harmful suicidal attempts, like tablets, poisons, locked rooms with fan, etc.
