You can take sildenafil with these tablets. But if you have difficulty of erection during sexual activity, find the cause and treat it, rather than using sildenafil. Sometime the cause may be sertraline 100 mg. Please discuss with your psychiatrist your problem and find the cause and take treatment accordingly.

I need to stop my smoking what I do. And am eats more food but am not get weight what I do.

MBBS, DPM
Psychiatrist, Bangalore
Dear lybrate-user, Congratulations for stopping smoking. It you stop smoking, you feel more hungry. You may put on weight in due course. Please try to stop smoking at the earliest. It has many health benefits in the long run. When you stop suddenly, you may get withdrawal symptoms like restlessness, irritability, lack of concentration, increased hunger, sleep disturbance, headache, impatience, anxiety, depression, etc. If you have them please don’t get back to smoking, take nicotine gum or lozenge or patch. They are available in all medical shops. You don’t need a prescription from doctors. Use 2 mg gum if you smoke less than 25 cigarettes/day. Use 4 mg gum if you smoke more than 25 cigarettes/day. First 6 weeks, use 1 gum every 1 – 2 hours. Reduce it in week 7 – 9 to one gum every 2 – 4 hours. In week 10 – 12, reduce it further to 1 gum every 4 -8 hours. By 3 months, you will be able to stop both cigarettes and gum fully without any withdrawal symptoms. Please feel confident that you can do it and give yourself some gift for doing it like vacation or books or new mobile. Smoking is a chronic relapsing condition like diabetes and B.P. So please don’t worry if you restart smoking. Start all over again to stop. You will learn the situations that make you smoke and avoid them. Please think of health benefits and money saved often. Plan how to avoid relapsing situations, like smoking friends, use of alcohol, stressful feeling or depression. Delay, drink, deep breathe, distract and discuss are the helpful tips. Increase of non smoking zones, gift yourself for stopping smoking. Even chewing cinnamon may delay. Please remember that the urge to smoke comes and goes like waves. Delay and see whether it goes away. It will be like a passing cloud.

Dear lybrate-user, Please give a police complaint that your aunt needs treatment but refuses it. The police comes home and takes her to the hospital. After psychiatric assessment, if she needs treatment, they will get magistrate's permission to treat her. Find out whether there are any local special ambulances that take mentally ill to the hospital. They come with one woman, either driver or counsellor, talk to her, sometimes inject her to take to hospital for treatment. It is better to take her to a new place with treatment for her mental condition.

Dear lybrate-user, I wish you were on treatment for depression continuously since 3 years. Probably you did not take it and fell prey to alcohol and cigarettes. Now apart from depression, you are also alcohol dependent and nicotine dependent. All 3 diagnosis need treatment together and simultaneously. You need to come out of all these "friends" with treatment to achieve your goal. Please consult a psychiatrist as early as possible and start the treatment. Treatment also takes minimum 6 months, if you are regular and co operate with it. Decade ago, you did not suffer from depression and alcohol and nicotine dependence and so you felt better.

Dear Jana, You should ask your doctor rgis question. Or you can ask your family member who took you to the doctor. With the available information, i think that you were going astray probably on weed or alcohol. It might be due to your depression for which Escitalopram is prescribed. May be you had irregular sleeping habits for which a hypnotic Zolpidem 10 prescribed. Probably your family would have thought if you sleep well and feel happy and confident, you may not seek alcohol or weed. Alcohol, weed, zolpidem and escitalopram work on the various neurochemicals in the brain to make you feel happy and confident. Together, the dose may be too much for you, making you croggy, imbalanced, poor reflexes while riding leading you to accidents, dizziness, drowsiness etc. Please remember, it all depends on the dose of alcohol and cannabis - what drink, how much quantity, the duration of drinking, type of food along with it, etc. Better to avoid alcohol and weed and continue to use escitalopram and zolpidem. All the best

My mother is suffering from bipolar disorder from last 25 years she was taking medicines and was absolutely fine from past 20 years but from past 5 years her medication are not working very efficiently, her sleep patterns are messed up either she wakes up too early or goes on to sleep till afternoon. She is also having diabetes type 2 but it is well controlled. I have consulted various senior psychiatrists in Delhi but no solution. She is getting maniac episodes every 6 months I want to know is this treatable or she's gonna be like this for rest of her life. Thank You.

Dear Harshit, It looks as though Kavita was given tablets for manic episode at present. There is no mood stabilizer in the list you have given. For 20 years, what are the medicines she was taking that kept her fine for many years. Lithium carbonate, divalproex or carbamazepine? At what dose and what blood level (lithium and divalproex). Probably the same drug, if it does not cause disturbance in her diabetes treatment, will be prescribed to her after controlling the present manic episode. Did she stop mood stabiliser, just because she was normal? Bipolar disorder is a lifelong illness. Recurrences are difficult to treat, more so with diabetes, so it is better to prevent the recurrences with proper dose of mood stabilizers.

Dear Afsal, Panazep, paxidep and tryptomer 10 are NOT the reason for your low sperm count. There will be other reasons for it like hormonal problems, diabetes, bp, cholesterol disturbances. Please go to doctor and get them checked and treated. Since 3 years are you sexually ok? If you have problems with erection, it may be due to these tablets. You need to talk to the doctor about it and change to other antidepressants without erection problems. If you lose interest in sex, it may be because of your panic attacks due to depression. It needs better treatment with antidepressants for some more time. BP, cholesterol, hiatus hernia, infertility, etc are lift style disorders needing lifestyle changes.
