Dear lybrate-user, There is nothing to fear about. Anxiety. Usually manifests as cold fingers and feet. Your BP is okay. Since you have tachycardia and anxiety. It is better to rule out thyroid problem by undergoing thyroid profile blood test. Some time. You may need to take an echocardiogram also. If they both are normal, it may be a generalised anxiety disorder which can be treated with anxiolytic tablets or relaxation exercises and cognitive behaviour therapy.

Dear lybrate-user, You seem to be having medical problems like obesity, diabetes and high bp. The psychiatrist has assessed both your medical and psychiatric disorder and advised the treatment. You need to follow her/his advice. Side effect has to be experienced by you and to be discussed with the psychiatrist so that she/he can adjust the dose. You may not experience any side effect at all. You need to wait for 2-3 weeks for these tablets to start giving you any benefit.

Patient is 81 year old. Is being prescribed olimelt 2. 5 twice a day. But he is sleeping all the time. Can we reduce the dosage and give only at night I. E one time only.

Dear lybrate-user, You should send the details of his symptoms and diagnosis for which he was prescribed olimelt. Of course olimelt is sedating. You can reduce and even stop olimelt, provided it is not given for any psychotic symptoms.

Dear Bhupinder, If your liver is normal and NOT cirrhotic, you can take ecosprin. Alcohol not only causes cirrhosis of liver, but also mild gastritis, hepatitis and at times pancreatitis. All these have bleeding tendencies. Ecosprin adds to the bleeding. If you stop ecosprin, with high cholesterol and triglycerides, your chances of heart attack is high. If you stop alcohol, your chances of cirrhosis and gastrointestinal bleedings are low. You get other health benefits also. The choice is yours.

Dear lybrate-user, I am not sure that you have followed up with your doctor recently. You seem to be on a low dose of Olanzapine. Probably you may need a mood stabiliser (if you are diagnosed as Schizoaffective Disorder) or an antidepressant in addition. Please consult your psychiatrist. You might get help to get out of depression like how you got out of occipital pain.

Someone I knew was suicidal and his doctor had prescribed her Pexep 12.5 mg. I looked up this medicine for her, it had side effects and one of them was suicidal thoughts. My question is, when she (the doctor) knew that her patient was struggling with this, why did she give her a medicine that threatens the very thing she is trying to conquer? She was suffering from OCD and some signs of BPD as well.

Dear Cho, All the side effects don't occur in all the patients in all doses. In many antidepressants, suicidal thoughts are mentioned as side effect. That does not mean that such thoughts have come because of Pexep. It might have been there already. Pexep would not have its effect in the first 2-3 weeks. 12.5 mg is a low dose. Let your friend take it. If depression is lifted, she will feel less suicidal.

I want to know if alcoholism and that too desi one can confidently stop drinking and refuse even when alone. He was in mumbai and never drank but now gone out of town and got again into it. And once starts drinking will not stop until his liver issue begins or does not lose all. Is there a confident solution towards this.

Dear lybrate-user, It is nice of you to think about alcohol use in Jagdish. May be, Jagdish can distract and postpone his craving for alcohol in Mumbai. Such distractions may not be available outside Mumbai. So he gets tempted and drinks. Once he lapses, it becomes a vicious cycle. It is better that he discusses these lapses, relapses and craving with a psychiatrist in Mumbai when he is sober itself. Sometime he needs to take anti craving tablets for 1 year or so along with relapse prevention strategies. You can help him in these consultations as co therapist.

I had undergone all cardiac test, ECG, doppler, STRESS TEST etc. As I was feeling restless. Following that I had developed stress, so sleeping problems appeared. For the last 4-5 month I am taking antidepressant. For last few weeks I am having palpitations. I am having palpitations problem for the last 15-20 years. Dear lybrate-user, Have you tested for thyroid functions also? You have been having palpitations since 15-20 years. It was treated with Mirtaz 7.5 mg since 4-5 months. With treatment, you were better - less restlessness, better sleep and less stress. So Mirtaz is not the cause for frequent palps. If you consult your psychiatrist again, she/he will increase the dose, or change the antidepressant, or add another drug or therapy.

Dear Vishal, Weed stays in the body for longer time depending on your lung capacity. But it will go away unless you smoke again. Please wait with any sort of breathing exercise.

I want to quit smoking but I fail every time what do I do I have tried 5 time to quit but keep on failing to quit?

Dear lybrate-user, Congratulations for wanting to quit smoking. Please try to stop smoking at the earliest. It has many health benefits in the long run. When you stop suddenly, you may get withdrawal symptoms like restlessness, irritability, lack of concentration, increased hunger, sleep disturbance, headache, impatience, anxiety, depression, etc. If you have them please don’t get back to smoking, take nicotine gum or lozenge or patch. They are available in all medical shops. You don’t need a prescription from doctors. Use 2 mg gum if you smoke less than 25 cigarettes/day. Use 4 mg gum if you smoke more than 25 cigarettes/day. First 6 weeks, use 1 gum every 1 – 2 hours. Reduce it in week 7 – 9 to one gum every 2 – 4 hours. In week 10 – 12, reduce it further to 1 gum every 4 -8 hours. By 3 months, you will be able to stop both cigarettes and gum fully without any withdrawal symptoms. Please feel confident that you can do it and give yourself some gift for doing it like vacation or books or new mobile. Smoking is a chronic relapsing condition like diabetes and B.P. So please don’t worry if you restart smoking. Start all over again to stop. You will learn the situations that make you smoke and avoid them. Please think of health benefits and money saved often. Plan how to avoid relapsing situations, like smoking friends, along with alcohol, stressful feeling or depression. Delay, drink, deep breathe, distract and discuss are the helpful tips. Increase of non smoking zones, gift yourself for stopping smoking. Even chewing cinnamon may delay. Please remember that the urge to smoke comes and goes like waves. Delay and see whether it goes away. It will like a passing cloud.
