Male infertitly

Male infertility
What is male infertility?
Reproduction (or making a baby) is a simple and natural experience for most couples. However, for some couples it is very difficult to conceive.
A man’s fertility generally relies on the quantity and quality of his sperm. If the number of sperm a man ejaculates is low or if the sperm are of a poor quality, it will be difficult, and sometimes impossible, for him to cause a pregnancy.
Male infertility is diagnosed when, after testing both partners, reproductive problems have been found in the male.
How common is male infertility?
Infertility is a widespread problem. For about one in five infertile couples the problem lies solely in the male partner. The reasons of male infertility is found out so because due to changing of fast new lifestyle. It is estimated that one in 20 men has some kind of fertility problem with low numbers of sperm in his ejaculate. However, only about one in every 100 men has no sperm in his ejaculate.
What are the symptoms of male infertility?
In most cases, there are no obvious signs of infertility. Intercourse, erections and ejaculation will usually happen without difficulty. The quantity and appearance of the ejaculated semen generally appears normal to the naked eye.
Medical tests are needed to find out if a man is infertile.
What causes male infertility?
Male infertility is usually caused by problems that affect either sperm production or sperm transport. Through medical testing, the doctor may be able to find the cause of the problem. About two-thirds of infertile men have a problem with making sperm in the testes. Either low numbers of sperm are made and/or the sperm that are made do not work properly. Sperm transport problems are found in about one in every five infertile men, including men who have had a vasectomy but now wish to have more children. Blockages (often referred to as obstructions) in the tubes leading sperm away from the testes to the penis can cause a complete lack of sperm in the ejaculated semen. Other less common causes of infertility include: sexual problems that affect whether semen is able to enter the woman’s vagina for fertilisation to take place (one in 100 infertile couples); low levels of hormones made in the pituitary gland that act on the testes (one in 100 infertile men); and sperm antibodies (found in one in 16 infertile men). In most men sperm antibodies will not affect the chance of a pregnancy but in some men sperm antibodies reduce fertility.

Known causes of male infertility
1.Sperm production problems

• Chromosomal or genetic causes
• Undescended testes
(failure of the testes to descend at birth)
• Infections
• Torsion (twisting of the testis in scrotum)
• Varicocele (varicose veins of the testes)
• Medicines and chemicals

• Infections
• Prostate-related problems
• Absence of vas deferens
• Vasectomy

2.Sexual problems
(erection and ejaculation problems)
• Retrograde and premature ejaculation
• Failure of ejaculation
• Erectile dysfunction
• Infrequent intercourse
• Spinal cord injury
• Prostate surgery
• Damage to nerves
• Some medicines

3.Hormonal problems
• Pituitary tumours
• Congenital lack of LH/FSH (pituitary problem from birth)
• Anabolic (androgenic) steroid abuse

4.Sperm antibodies
• Vasectomy
• Injury or infection in the epididymis
• Unknown cause

How is male infertility diagnosed?
If a couple has been trying for a pregnancy without success, they should go to their local doctor, family planning clinic or women’s health clinic, and have some initial tests. Both partners should be tested, even if one has a child from another relationship. Diagnosis can involve a medical history from the man and a physical examination along with a semen analysis to check the number, shape and movement of sperm in the ejaculate.
Blood tests may also be done to check the levels of hormones that control sperm production. Genetic investigations and testicular biopsies are also done.
How is male infertility treated?
One in eight infertile men has a treatable condition, and after treatment, couples can become pregnant naturally.
In some cases, the doctor will recommend that the couple seek assisted reproductive technologies (ART), such as IVF (in vitro fertilisation). ART do not cure or treat the cause of infertility but they can help couples achieve a pregnancy, even if the man’s sperm count is very low.
What is ICSI?
Intracytoplasmic sperm injection (ICSI) is a form of IVF where a single sperm is placed directly into each egg by piercing the outer covering of the egg. ICSI is particularly helpful for men with poor sperm production. Sperm are collected from the semen or removed carefully from the testis or epididymis
Can I do anything to prevent male infertility?
It is best to avoid cigarette smoking, excess alcohol, sexually transmitted infections, heat stress from tight fitting underwear, and anabolic steroids (taken for body building or sporting purposes) as these factors can be harmful to the production of sperm. If you work in an occupation that may affect your fertility, it’s important to wear protective clothing and follow all occupational health and safety guidelines. It is recommended that couples trying to conceive avoid exposure to any possibly harmful chemicals.Because of the possible effects on fertility, it’s important to tell your doctor if you have had hormone treatment or had surgery as a child to move your testes into the scrotum.

Can older age cause male fertility problems?
Healthy men in their 70s and beyond can still father children, however the time taken for a partner to become pregnant is longer when a man is middle-aged or older. Reasons for this may include a decrease in sexual activity, lower semen volume, changes to sperm motility (movement), a lower number of motile sperm, and possibly lower sperm function and DNA quality.
The chance of the child having some genetic or chromosomal problem is also increased for older men.
What emotions might a man experience when diagnosed with infertility?
Men may be shocked when they find out they are infertile. There is still a common but incorrect belief that infertility is a woman’s problem. Therefore, when men are told there is a sperm problem, they are often unprepared. Being told that there is a sperm problem can affect a man’s sense of masculinity, sexuality and potency. Most infertile men at some time struggle with the idea that they are not able to do what other men can.
An emotional response to infertility is normal. Talking with a doctor or a sexual therapist or counsellor can help men with their feelings about infertility.

Hello Sir/Madam ,I am 51 years old Asthma patient. I use Asthalin inhaler three times a day continuously for past 30 years. Is it harmful to use Asthalin regularly? Will it be less effective for me at the time of acute asthma attacks due to continuous use of it?

Hello Sir/Madam ,I am 51 years old Asthma patient. I use Asthalin inhaler three times a day continuously for past 30 ...
Hi Mr. lybrate-user, There are two aspects of treating asthma. One is relieving an attack. Second is preventing further attack. Asthalin is only to relieve an attack. It does not prevent asthma from worsening. Hence if you are requiring asthma three times daily you may need inhalers with extracontroller. Yet using asthalin Does not cause major side effects. Yes. Using it this frequently can cause it to be less effective. I hope I answered your queries.

Want to know about bipolar disorder and its side effects and complete consultancy. Please tell.

Reparenting Technique, BA, BEd

Want to know about bipolar disorder and its side effects and complete consultancy. Please tell.
It can be a crippling disease if not treated and if treated early the prognosis is good. So quickly visit a psychiatrist and seek his/her opinion and you must then follow the medication prescribed. Comply with all the medical prescriptions and work in tandem with a counselor for a long time and learn coping skills and techniques to handle the daily challenges you will face. There are certain adaptations that will suit you during the elation phase and there are certain behaviors that are required during the depressed phases. In the meantime you can do certain things that will give you benefits anyway. Emotional therapy will alleviate a lot of the problems faced in this condition because it is a mood disorder. You need to particularly look at your levels of anger both for its reasonableness and to express it in appropriate manner. I suspect that much of this anger may have origins in early childhood that you may never be aware of now. Talk to your parents and explore that angle. This information will help the counselor too. Also you will need to check whether there is any genetic factor involved. You must exercise regularly, eat healthily and sleep normally too. If your motivation and cooperation is good much can be achieved. You must have a time-structured schedule filled with interesting and reasonable activities and it must be monitored for compliance and delivery. The reason I say this is that many times you will be inclined to have grandiose notions and confidently believe that you can do without the medication. This will set you back many months in the treatment and delay and sabotage the recovery: forewarned you are forearmed. If this combination of medication with therapy is maintained for at least three years without any episodes or breakdowns, it is possible to taper the medication in consonance with the doctor. Bipolar is characterized by two extreme poles i.e. Feeling very high (mania) and suddenly feeling very low (depression). You can ask the doctor for the symptoms he noticed in you for a personal understanding. Regarding some of the symptoms here goes: Mania is a term that describes the emotional highs of bipolar disorder. In general, symptoms of a manic episode may include: •High energy, excessive activity, and/or restlessness •Overly good mood •Irritability •Fast, erratic talking •Racing thoughts •Inability to concentrate •Little need for sleep •Feelings of power •Poor judgment •Reckless spending •High sex drive •Alcohol or drug abuse •Aggression •Refusal to admit that there is a problem Bipolar Symptoms: Depression In bipolar disorder, manic episodes alternate with periods of emotional lows, which are known as depressive episodes. Signs of a depressive episode may include: •Sadness, anxiety, or a feeling of emptiness •Hopelessness •Feelings of guilt, feeling worthless, or feeling helpless •Lack of interest in activities that were once enjoyable, such as sex •Low energy •Trouble with concentration or memory •Restlessness or irritability •Excessive sleeping or insomnia •Changes in appetite or weight •Pain or other physical symptoms not explained by an illness or injury Thoughts of death, or suicidal thoughts or attempts to kill the self.

Does Premature ejaculation cause bad to future? Sir please tell in an allopathic way?

C.S.C, D.C.H, M.B.B.S
General Physician,
Does Premature ejaculation cause bad to future? Sir please tell in an allopathic way?
It will not cause any problem. YOu can gain time practice. Ejaculation of semen during sexual intercourse before or immediately after penetration. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common sexual complaint. Estimates vary, but as many as 1 out of 3 men say they experience this problem at some time. As long as it happens infrequently, it's not cause for concern. The primary symptom of premature ejaculation is the inability to delay ejaculation for more than one minute after penetration. However, the problem may occur in all sexual situations, even during masturbation. Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. Many men feel that they have symptoms of premature ejaculation, but the symptoms do not meet the diagnostic criteria for premature ejaculation. Instead these may have natural variable premature ejaculation, which is characterized by periods of rapid ejaculation as well as periods of normal ejaculation. Psychological causes Some doctors believe that early sexual experiences may establish a pattern that can be difficult to change later in life, such as: •Situations in which you may have hurried to reach climax in order to avoid being discovered •Guilty feelings that increase your tendency to rush through sexual encounters Other factors that can play a role in causing premature ejaculation include: •Erectile dysfunction. Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. •Anxiety. Many men with premature ejaculation also have problems with anxiety — either specifically about sexual performance or related to other issues. •Relationship problems. If you have had satisfying sexual relationships with other partners in which premature ejaculation happened infrequently or not at all, it's possible that interpersonal issues between you and your current partner are contributing to the problem. Biological causes A number of biological factors may contribute to premature ejaculation, including: •Abnormal hormone levels •Abnormal levels of brain chemicals called neurotransmitters •Abnormal reflex activity of the ejaculatory system •Certain thyroid problems •Inflammation and infection of the prostate or urethra •Inherited traits •Nerve damage from surgery or trauma (rare) Common treatment options for premature ejaculation include behavioural techniques, topical anaesthetics, oral medications and counselling. Keep in mind that it may take a little time to find the treatment or combination of treatments that will work for you. Behavioural techniques In some cases, therapy for premature ejaculation may involve taking simple steps, such as masturbating an hour or two before intercourse so that you're able to delay ejaculation during sex. You may try avoiding intercourse for a period of time and focusing on other types of sexual play so that pressure is removed from your sexual encounters. You can also try diverting your mind before ejaculation is ready to come and proceeding after a brief break, thereby prolonging duration of intercourse / action The pause-squeeze technique You and your partner can use of a method called the pause-squeeze technique. This method works as follows: 1.Begin sexual activity as usual, including stimulation of the penis, until you feel almost ready to ejaculate. 2.Have your partner squeeze the end of your penis, at the point where the head (glans) joins the shaft, and maintain the squeeze for several seconds, until the urge to ejaculate passes. 3.After the squeeze is released, wait for about 30 seconds, then go back to foreplay. You may notice that squeezing the penis causes it to become less erect, but when sexual stimulation is resumed, it soon regains full erection. 4.If you again feel you're about to ejaculate, have your partner repeat the squeeze process. By repeating this as many times as necessary, you can reach the point of entering your partner without ejaculating. After a few practice sessions, the feeling of knowing how to delay ejaculation may become a habit that no longer requires the pause-squeeze technique. Topical anaesthetics Anaesthetic creams and sprays that contain a numbing agent, such as lidocaine or procaine, are sometimes used to treat premature ejaculation. These products are applied to the penis a short time before sex to reduce sensation and thus help delay ejaculation. A lidocaine spray for premature ejaculation is available.
