I have hypothyroid which is autoimmune. Taking levothyroxine from 3 months. I am frequently getting indigestion, gummy stools, stomach ache. Sometimes nausea. Is it related to thyroid. Lybrate-user, hashimoto's thyroiditis (ht), which is the autoimmune condition, causing hypothyroidism. Treatment is same like hypothyroidism. With thyroxine tablets. The problems mentioned could be part of the symptoms of ht. However, some times digestive problems may not be connected with that condition. It is better if you increase intake of green leafy vegetables and fruits, that will help in reducing this. Plus hypothyroidism has to be well controlled.

PCOS And Diabetes!

PCOS is a hormonal disorder seen in most of the women. It affects the ability of the women to produce eggs. In this condition, the ovaries appear with enlarged size and present multiple small sac-like structures (cysts) on the outer edges. These cysts contain immature/undeveloped eggs which may lead to infertility. The exact cause for PCOS is unknown yet. Mostly genetical and environmental factors play a major role in the development of PCOS.

How is PCOS linked with Diabetes?

  • PCOS and diabetes are linked with each other by a major factor i.e., excess fat and insulin resistance.

  • Excess fat leads to insulin resistance, which in turn results in development of diabetes.

  • Reducing excess fat in the body and controlling insulin sensitivity may make the ovaries to “breathe” easier.

  • Several studies have found that women who had PCOS were more likely to develop type 2 diabetes compared to women who didn’t have. PCOS is associated with higher levels of insulin in the blood. Higher levels of insulin are seen in case of few patients with type 2 diabetes, who are unable to utilise their insulin due to insulin resistance.

  • PCOS has also been implicated with type 1 diabetes.

Diabetes - A Major Risk Factor for Kidney Disease!

Diabetes is a disorder that is characterized by an inability of the pancreas to produce enough insulin or effectively utilize the insulin produced by the body. Insulin is the hormone that is produced by the pancreas to metabolize sugar in the body, the sugar that is present in the food that you consume.

Diabetes is commonly classified into two types: Type 1 and Type 2 diabetes.

In Type 1 diabetes, the cells in the pancreas do not produce insulin in the required amounts; this disorder usually tends to occur in children. Type 2 diabetes generally occurs in people who are above 40 years of age; this type is characterized by an inability of the body to efficiently utilize the insulin produced by the pancreas.

How can diabetes affect your kidneys?

Too much glucose, also called sugar, in your blood from diabetes damages your kidneys’ filters. If the filters are damaged, a protein called albumin, which you need to stay healthy, leaks out of your blood and into your urine. Damaged kidneys do not do a good job of filtering wastes and extra fluid from your blood. The wastes and extra fluid build up in your blood and make you sick.

Diabetes is a leading cause of kidney disease. Diabetic kidney disease is the medical term for kidney disease caused by diabetes. Diabetic kidney disease affects both kidneys at the same time.

Signs of Kidney Disease in Patients with Diabetes

  1. Albumin/protein in the urine
  2. High blood pressure
  3. Ankle and leg swelling, leg cramps
  4. Going to the bathroom more often at night
  5. High levels of BUN and creatinine in blood
  6. Less need for insulin or antidiabetic medications
  7. Morning sickness, nausea and vomiting
  8. Weakness, paleness and anemia
  9. Itching

What are the possible complications?

  1. End-stage kidney failure: If this occurs then you would need kidney dialysis or a kidney transplant.
  2. Cardiovascular diseases: Diabetics have an increased risk of developing cardiovascular diseases, such as heart disease, stroke and peripheral arterial disease. If you have diabetes and diabetic kidney disease, your risk of developing cardiovascular diseases is increased further. 
  3. High blood pressure: Kidney disease has a tendency to increase blood pressure. In addition, increased blood pressure has a tendency to make kidney disease worse. Treatment of high blood pressure is one of the main treatments of diabetic kidney disease.

What to do?

  1. It is essential to get your urine checked at least once every year to detect signs of kidney damage.
  2. Some other symptoms that you may experience are swelling in the ankles, weight gain and a rise in your blood pressure.
  3. The first step to treat kidney damage caused by diabetes is to get your blood sugar levels under control.
  4. You should also avoid consuming medications that can cause damage to the kidneys.
  5. A kidney transplant or dialysis may be advised if the damage to kidneys is significant. If you wish to discuss about any specific problem, you can consult a Nephrologist.

Dear lybrate user with the limited amount of information provided it appears that you are suffering from diabetes. It could had been better had you provided us with your last prescription, latest fasting sugars, hba1c levels and any other recent relevant investigations along with the complete medication list. For the successful treatment of diabetes it is advised if you can start exercising on a regular basis preferably 30-45 minutes, diet modifications with low carbohydrate and high fibre diet along with medications as advised. Hope that solves your query.

Mr. Lybrate-user chandra, the blood sugar levels mentioned are fairly good, particularly fasting. Pp should come down a bit more for good control. From now on you have to see that blood sugar levels are maintained with in expected levels (fasting near 100 mg and pp 150 to 160 mg). Take prescribed medication regularly, without missing a dose. Exercise daily for at least 1 hour and strive to maintain near normal weight. Take a restricted diet giving exact calories based on ideal body weight (60 kgs). You a bmi of 34.375 kg/m2 which signifies you have grade 1 obesity. Reducing weight it self will help in better blood sugar control. Diabetes is condition that will remain for life time and keeping it under control is essential to prevent or postpone long term complication.

Inspite of taking 22 units of insulin in the evening and 50 units in the morning I find through Aviva Accu - check that my evening RPS is between 300 - 400 and the morning FPS is between 190 - 300. I do admit to a exercise bereft lifestyle but I keep a sharp check on my diet. Am I undergoing insulin resistance?

The very fact that you have diabetes means that insulin resistance has been present for a long time even prior to the diagnosis of diabetes in you. I personally feel that a few oral medicines can be tried out in you rather than taking huge doses of insulin. Also, do ensure that your insulin administration technique is correct. A lot depends on that. And lastly as you have mentioned of an exercise bereft lifestyle, even a small effort in starting exercise will go a long way in overcoming the insulin resistance. Hypothyroid and latest test showed normal free T3 and T4 and Tsh was 8.5. Now on 50 mcg thyroxine. Previous test showed prolactin levels to be 29.8 and just completed 30 days course of MCBM 69 and B-Long. On cycle day 42 today and sexually active, no sign so early pregnancy and haven't taken a HPT yet. What is your advice? When should I repeat my tests?

Lybrate-user, if you are having hypothyroidism and taking treatment, then it is essential to bring tsh levels down to <2 mu/l, otherwise conception may not occur. Further through out pregnancy tsh should be maintained as follows: 1st trimester 0.1 to 2 mu/l, 2nd trimester 0.3 to 3 mu/l & 3rd trimester 0.3 to 3.1 mu/l. If tsh levels go higher than above mentioned, then fetal development is affected, miscarriage may occur, pregnancy out come at full term may not good. So please take care of the tsh levels. Thanks.

First of all you should continue thyroid drugs and ensure that it is controlled. Weight gain is another problem that causes mens irregularities. So, your first treatment is in your hand- weight loss, weight loss and weight loss (diet and exercise). Some of my patients are doing very well simply by weight loss without any drugs. If despite weight loss (when you are sincerely trying), the problem persists, you should take drugs after consulting doctors

Diet control and exercise are equally important for diabetes person. Diet high in protein for the health of diabetes because it supplies the essential amino acids needed for tissue repair. Pulsese are rich in protein and fibre. Take lukewarm water with half spoon methidana powder in the morning. Complex carbohydrates and fibre should be included in diet. You should avoid fasting and feasting. Timely and balanced diet is as important as regular medicine intake to control diabetes. Use sufficient amount of given onion, garlic, coriander, lemon, greenchilli, black paper, fenugreek seeds, cinnamon, bay leaves in food items. No long gapes and skipping of meals. Chapatis of bran flour (unrefined) is given to diabetes patients and to increase the nutritional value add gram and soyabean flour to it.

Mr. Lybrate-user, looking at the details given show that diabetes is uncontrolled. Needs better control, either through improved treatment and/or diet and exercise. First thing your fasting and pp should be 100 mg and 150 to 160 mg and hba1c% <7. So it is essential to achieve that kind of control, then only all symptoms can be controlled. As regards the precautions: take a balanced diet, providing exact calories based on blood sugar levels, ideal body weight and extent of daily exercise. Plus you should consume more of green leafy vegetables, salads and permitted fruits (100 g a day). Plus exercise daily for at least 1 hour. Avoid alcohol and smoking. In other words you have to follow a modified lifestyle.

Hypothyroidism is a condition which can only be treated by taking tab thyroxine (thyronorm), to begin with start on 50 mcg daily, to be taken early morning on empty stomach and after that for 2 h nothing to be taken. Since you are pregnant medication has to be started right away and to be continued with out break. After 3 - 4 weeks check tsh, t3 & t4 levels.

Hi Doc, If I turn vegan, from where do I get daily requirements of protein and minerals needed for healthy body. Isn't it better to have all types of food instead of adapting a vegan lifestyle and do some sort of physical exercise daily. What is real benefit of turning vegan besides improved cholesterol level and less risk of diabetes and other lifestyle diseases.

Mr. Lybrate-user, being a vegetarian is by ones own choice. Vegetarian diet also does provide all the necessary nutrients and proteins, when proper food selection is made. Whole grain foods, multiple grains usage, use of pulses in good quantity, use of soy beans and peanuts, chanas, etc. Does give good proteins. Milk and dahi, gives a good variety of proteins. Green leafy vegetables, multicloured vegetables, fruits and berries, dry fruits offer almost all needed nutrients. Vegetarian diet is good, advantageous but if you want to shift to other foods, there is no barrier. It always by choice. Thanks.

Hi looking at your height and weight, you are having grade 1 obesity, with bmi of 30.35 kgs/m2. The reason for this could be hypothyroidism, as it increases weight. Unless well controlled efforts to reduce weigh will not be fruitful. As regards the treament with thyroxine, yes it has to be started immediately. Aim is to bring down tsh levels to around 3 to 4 milli units/l. This is a condition occurring due to deficiency of hormone thyroxine produced by thyroid gland. Since that hormone is required daily for multiple metabolic functions and functioning of brain, replacement has to be continued for life time. It is very safe treatment. Thanks for the question.

It's better to start medicine Atorvastatin 10mg once a day for a minimum of one year and do aerobics exercise and eat a low fat content in your diet And yes high levels of lipoprotein A is dangerous and can cause stroke or myocardial infarction even at a young age

If you have diabetes and your wife does not have diabetes or does not have family history of diabetes then there are 50% chance of your child developing type -2 diabetes at later stage in life. However if life style modification done in early stage 8years that is before your child becomes 18 years of age can cause genetic suppression or silencing of diabetic gene and scientific possibility that your child may never develop diabetes or pass diabetic genes to next generation.
