Physiotherapy For Carpal Tunnel Syndrome

Carpal tunnel syndrome is a painful disorder of the hand caused by pressure on your median nerve as it runs through the carpal tunnel of the wrist. Symptoms include numbness, pins and needles, and pain (particularly at night). Anything that causes swelling inside the wrist can cause carpal tunnel syndrome, including repetitive hand movements, pregnancy and arthritis.

Cause of Carpal Tunnel Syndrome
Carpal Tunnel Syndrome symptoms begin when the pressure inside the tunnel becomes too high. This results in your median nerve becoming compressed as it passes through the small tunnel. 

The carpal tunnel pressure increase occurs when either of two things happens:

  1. The tunnel space decreases, such as when the wrist swells eg after a traumatic injury, partial subluxation of the carpal bones or fracture or tenosynovitis.
  2. When the contents of the tunnel (median nerve, blood vessels and tendons) enlarge. 
  3. Both of these situations increase the pressure on the nerve, leading to the carpal tunnel symptoms.

Symptoms of Carpal Tunnel Syndrome

Carpal Tunnel Syndrome (CTS) sufferers will usually experience the following symptoms in their hand or fingers:

  1. hand pain or aching
  2. pins and needles
  3. numbness esp at night of with wrist flexing
  4. burning
  5. weakness or cramping
  6. perceived swelling

The symptoms are usually worse at night and your grip will weaken as the condition progresses. Eventually you will notice muscle atrophy of the thenar (thumb) muscles and loss of hand function or clumsiness. If this sounds like you, suspect carpal tunnel syndrome.

Physical Therapy Treatment

  1. Gliding Exercises: Moving your fingers in a specified pattern of exercises may help your tendons and nerves glide more smoothly through your carpal tunnel. While there’s some evidence that gliding exercises can help relieve symptoms when used alone, these exercises appear to work better in combination with other treatments -such as splinting.
  2. Graston Technique/Manual Therapy: The Graston Technique incorporates a patented form of instrument-assisted soft tissue mobilization that enables clinicians to effectively detect and treat scar tissue and restrictions that effect normal function. Manual therapy techniques such as myofascial release or Active Release Technique are hands-on techniques that release tight tendons and musculature.
  3. Ultrasound: Deep, pulsed ultrasound directed at the carpal tunnel can reduce pain and numbness, and improve hand strength. Continuous superficial ultrasound doesn’t help.
  4. Traction: Some people obtain relief from carpal tunnel symptoms by using a hand traction device, called C-Trac. You might try this device, which can be purchased for home use, if other conservative treatment options have failed.
  5. Splints: Splints (braces) are the most commonly used nonsurgical treatment for carpal tunnel syndrome. Splints that immobilize the wrist in a neutral (unbent) position are most likely to relieve discomfort. An unbent wrist maximizes the size of the carpal tunnel, which reduces pressure on the median nerve, relieving your symptoms. I am 74 yrs old while sitting on chair or by downing legs anywhere, I got swelling on lower portion off both legs. Is there anyh remediy. I take aquadize tab, but sometime it is normal, again it deveelopes.

Rule out high blood pressure or any other kidney problem pain & swelling in legs is a common condition. Do hot fomentation keep a pillow under the legs at night &/when resting. Any way take caldikind plus (mankind) 1tab odx10days paracetamol 250mg od & sos x 5days do knee exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You seem to be having thigh strain which is very common if you exert too much while training, the answer is to stop all your physical activities till your pain subsides, start gentle anti inflammatory medications for 7-10 days, ice packs at home, limb elevation especially if it is associated with swelling, physical therapy focusing on scientific muscle training, pre activity warm up and gradual stretching of muscles before physical activity. You should also focus on increasing your physical activity gradually in a step wise manner so that muscles do not get fatigued and are adequately strengthened over a period of time. This may be happening due to some nerve compression. Pls visit a neurologist to identify which nerve is getting compressed and the cause. In the mean time you can do some wrist exercises like clockwise and anticlockwise rotation.

Details of your heel pain pattern are needed to reach a diagnosis. Most common cause of heel pain is Plantar fascitis. The treatment mostly is conservative and lifestyle modifications to achieve maximum functionality with minimum pain. Follow these lifestyle changes religiously to get benefit. 1. Reduce body weight as obesity will aggravate the stress on heel. 2. Avoid exercises like running / walking on hard surfaces. Stop such activities when you start developing pain. 3. Roll your feet on bottle of cold water for 20 mins 4 times a day or can apply ice pack. 4. Use shoes with silicone sole inserts. 5. Do calf muscle and heel stretches daily. 6. Avoid prolonged standing or sitting. 7. Use night splint to keep foot flexed at night. Stop using it once pain is relieved. 8. Use analgesics like Ibuprofen for decreasing pain and inflammation. In severe cases, therapy like PRP injection can give good results. Consult a pain specialist for such treatment options. With proper lifestyle changes as above 90 % patients have relief and functionality. Only 10% patients would require surgical intervention like plantar fascia release or gastrocnemius recession.

Pain at more than one place or all over the body is quite common. Kindly rule out diabetes & vitamin d deficiency or any other metabolic disorder. Proper physiotherapy will help. Medicines will also be needed. Antioxidants like caldikind plus (from mankind) 1 tab /day helps. Hello, Some primary headaches can be triggered by lifestyle factors, including: Alcohol, particularly red wine Certain foods, such as processed meats that contain nitrates Changes in sleep or lack of sleep Poor posture Skipped meals Stress insufficient sleep or rest Overexertion Nutritional deficiencies A secondary headache is a symptom of a disease that can activate the pain-sensitive nerves of the head. Any number of conditions — varying greatly in severity — may cause secondary headaches. Get your blood pressure checked, make a chart. Let me know about the readings, will give you medication accordingly. Also, get eyes tested. Medication: Meantime, can start with homoeopathic medicine - Bakson's Mig Aid tablet/ after every 2 hours. For Backache: Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. This itching is caused by continuous sleeping in same posture. Fungal growth starts there. Take candid dusting powder and dust the affected area with this powder.

Hello lybrate user, get your x-ray of the knee joint in standing weight bearing position in anteroposterior and lateral projections. Mostly it is has osteoarthritis of the knee joint wherein the articular cartilage of the joint gets progressively degenerative. I am a 50 years old lady. I am getting heavy back pain at sleeping time in the bed.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. My son 38 years old developed Foot Drop about year back. He was also operated for decompression spine nerves. But even after one year of taking physiotherapy foot drop has not able to recover. Any other procedure or alternate treat ment available to treat foot drop.
