I am 41 years old female. I feel pain in my each and every joint but not frequently in all joint. Pain in my joint moves one place to another. My body weight is 72 kg. Bulky body. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned rheumatoid arthritis as a condition you are suffering from. It is a chronic inflammatory joint disease which has a genetic predisposition. Since this is a genetically linked disease hence as of now there is no permanent cure for it, but new intravenous medications are available which help in disease control. You need a very active lifestyle with intensive regular physical therapy, take adequate calcium and vitamin d supplementation you also need to be in touch of a rheumatologist as it needs a long term supervised care. Why my hands and feet are releasing too much off heat. I avoid sugar and salt . Whenever I take sugar or salt my leg below knee start paining a different kind of pain . During that pain duration. I have urge to walk or have to hit that area very hard . Is this a disease or some mineral deficiency?

I am 45 years old male and I have back ache since last 2 years. What should I do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to have a persistent back pain as a health issue. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like dynamic x rays of lower back and few blood tests for evaluation. I have sever pain in the calfs and back of the thighs especially when bending to pick up anything. Some say it could it be lack of vitamin d or spinal problem. Please help.

This is quite a common condition rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take caldikind plus (mankind)1tab odx 10days paracetamol 250 mg od & sos x 5 days do back (spine) exercises contact me again if need be. Make sure you are not allergic to any of the medicines you are going to take.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Sleep on a hard bed with soft bedding on it. Spring folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. For emergency treatment contact your nearest hospital or family doctor. If it does not give relief in 4-5days, contact me again. Why not discuss with me in a video conference? Hot Fomentation. Gentle Shoulder Stretching. Core Strengthening Exercises. Gentle Neck Exercises. Avoid sleeping on affected side.. Warm up and cool down is a must.
