Paraesthesia: Causes & Treatments

Often paraesthesia is caused due to the needle injury. When the dentist inserts the needle for a block, the patient might experience a sharp sensation on the tongue equivalent to that of an electric shock. This is known as paraesthesia and is defined as a change in the sensation or anesthesia that is persistent and the duration generally extends. This condition cannot be prevented and is a complication in some patients who undergo a dental treatment. Though it is commonly seen in the implant therapy, paraesthesia is more of a dental malpractice.

The feeling of the electric shock is felt when the needle comes in close contact with a nearby nerve. This is enough to develop paraesthesia. Severing of the nerves with a local anesthesia and small gauge needle is uncommon. The exact cause of paresthesia has not been ruled out, but it is believed that the block happens because of using 4% solutions of local anesthesias. In case a paresthesia occurs, then it usually gets resolved within some days, weeks or months, but if it lasts for more than 6-9 months, then it is considered to be permanent.

When the paraesthesia is due to a surgical trauma, then getting help from an oral and maxillofacial surgeon is recommended. They assess that if a surgical intervention can be used to fix the problem. In case the paraesthesia is due to the anesthesia given locally by the dentist, then he/she does the following:

  1. Patient Management: Reassurance of the patient is must as they get jittery about the situation. The dentist often speaks about the condition with the patient personally. The patient is made to understand how a paraesthesia might have occurred and how much time it would take to resolve. This is also recorded in the patient book so as to avoid any such incidents in the future.
  2. Examination of the patient: The dentist should discuss the whole phenomenon and procedure of how paraesthesia can take place. It is important to let the patient know that the condition subsides, but it might take some time. The extent and the degree of paraesthesia should be assessed and the findings should be noted in the examination records.
  3. Follow up with patient: It is the dentist's duty to keep a tab on the follow up with the patient. The patient should go for a re-checkup within 1 month of developing a paraesthesia and then again in 1-2 month intervals. The visits could be more and can last until the paraesthesia completely resolves. Improving signs and symptoms usually promise that the paraesthesia is getting better. If paraesthesia is still persistent, then help from an oral and maxillofacial surgeon should be taken for a surgical approach.

All individuals requires scaling (cleaning) to be done atleast once in 6 months to maintain our oral hygiene clean. Our toothbrush is not much finely designed to clean all the plaque and calculi in the interdental areas of the teeth, which is the area where most of the calculus accumulate much and has to be removed regularly using scalers by the dentist.

Hello! Sensitivity in teeth could be normal in certain individuals to a certain extent. It could also be an indicator to a cavity or loss of tooth structure. You need to get your teeth examined to identify the cause and get treated accordingly. Meanwhile you can use a toothpaste like Sensodyne or Thermoseal RA to reduce the sensitivity. I strongly advice to get a check up done first, to avoid delaying treatment for the cause.

First of all, you need to stop this habit of chewing tobacco as it is cancerous. Also bad for gum hygiene. It erodes the gums and hence weekends it. So you need to stop it and need to have Vit-B1. Which helps in gum strengthening.

Although excessive saliva is not too common, there are a number of conditions that can cause it - some harmless, others more serious. It is possible that your saliva problems are just a result of the way you speak. Some people speak faster or slower than others, just as some people blink more often, breathe faster, or have a more rapid heartbeat. This may just be what's normal for you. Since it is negatively affecting your life, I suggest your problem can be ignored. However there are medical conditions that causes excessive salivation injection glycopyrrolate reduces secretions in the mouth, throat, airway, and stomach.

Hi. Today morning when I got up, I am having pain on the left side of face that comes from the lower jaw to the joint and then to the ear. Not able open my mouth fully. Also not able to eat food because of pain. I never had such problem. Also it's not tooth ache as well.

Its mostly related to joint which connects jaw bone to skull. Take painkillers for couple of days if not resolved consult doctor.

Dear patient its absolutely fine to have 28 teeth. Although there is still some time that your wisdom teeth can come out. But nothing to worry whether they come or not. Now a days many people have only 28teeth. To be sure whether they are inside or not you can go for opg i. E. Full mouth x-ray. Even if they are inside and there is no problem you can let them sit inside.

Kindly consult a dentist in person for further suggestion. We need more investigations with clinical examination to decide upon treatment. You may need root canal treatment. Until then apply clove oil on the decayed tooth. Rinse frequently with mouth wash. Advise c. Amox 500mg 1-1-1, thrice daily, t. Metrogyl 400mg 1-1-1, thrice daily, t. Imol 1-1-1, thrice daily (contraindicated in conditions with gastric ulcer, asthma, wheezing, allergy), c. Becosules 1-0-0 once daily all for 3 days only. Advance procedures can be done with laser. Dental tips: - visit a dentist every six months for cleaning and a thorough dental check-up. Limit sugary food to avoid tooth decay. Gargle your mouth thoroughly after every meal. Scrub gently to clean your tongue with a tongue cleaner. Floss all your teeth inter dentally & brush twice daily, morning & night, up & down short vertical strokes, with ultra-soft bristles, indicator brush. Tooth brush to be changed every 2 months.

At times it happens that your fixed cap comes out while chewing something but not to worry, you need to preserve that cap and book an appointment to your dentist for the same. Your dentist which check for any problem in the tooth or cap and vl cement the same cap if possible.

Know how Root Canal treatment helps to save a tooth

Hi friends I am doctor Premendra Goyal. Lot of questions are asked to us about whether to save a tooth or to extract it. I will talk to you about root canal treatment today which is a treatment by which we can save your tooth and it can serve for a lifetime with good maintenance. A tooth basically is a lot of hard tissue with some tissue in the center, you can it the heart of your tooth which in our language is called as pulp and layman end up calling it something as nerve. Once that nerve or pulp portion of your tooth is exposed to the oral cavity in some way may be a fracture of your tooth, may be decay, cavities- as we call them, once the heart of your tooth, the pulp of your tooth, the nerve of your tooth is exposed to the oral cavity your tooth is in danger. Tissue goes bad, infection travels, it goes deeper and it enters your bone area.

Now to save that tooth or not that is the first question that comes to the mind. By saving that tooth we are saving the natural thing that nature has given us. We have been trying to replace or replicate what nature has given us, but 100% replication of what nature has given us is not possible till date. So the best option is always to save a natural thing, and that can be easily done by root canal treatment. In root canal treatment I will try to explain in as simple language as simple the procedure is for the patient. I will reiterate- for the patient, for the clinician or for your dentist it definitely is a process which involves a lot of intricacies but for a patient it is a very simple procedure, wherein local anesthesia is given and the area is numbed and you don’t feel any pain when you undergo the treatment.

A small hole is drilled on the top of the tooth, if it is a grinder, if it is the front tooth the hole is drilled on the back side of your tooth and the nerve area is accessed. And through small instruments the inside and the root area of your tooth is cleaned, up it is shaped, it is disinfected with lot of medicaments and then it is finally filled up with artificial material and then your tooth is restored to normalcy with a cap sometimes or a post which is a support inside the root and building up with the tooth canal material or a material of your choice.

I will give you a broad overview about how a root canal treatment goes about in the pictorial representation. (Displaying pictures) Suppose this was the decayed tooth, a small opening is made on the top of the tooth, we enter the tooth with small instruments, and with the advent of newer technologies it is possible today to complete your root canal in one single sitting, a half an hour to 45 minute appointment and you can end up with a painless tooth. First the root canal is cleaned, it is disinfected with certain medicaments, a artificial filling is placed in the root and then the tooth is restored on the top. So the question of whether to save a tooth or whether to extract it ends up with a proper restored tooth which works for a very long time serving you, thank you.

Dear bad Odour from mouth is a very common problem. Although it can have various local as well as systemic causes, the most common cause of the problem is poor oral hygiene. It seems you already brush twice daily and also use mouth wash. However how often do you clean your tongue? unclean tongue is a very frequent source of bad breath. You can either use your brush or the tongue cleaner to properly clean your tongue after meals. In addition I will suggest you to start using dental floss at least once daily. Dietary & lifestyle modification like avoiding odorous food like onion, garlic, smoking (if you smoke) can also help. Don't let your mouth dry, sip water intermittently. I am sure you will get over the problem. Sometimes other diseases like infection of respiratory tract (sinusitis) or digestive system disturbances can also be the culprit but think about these only if you don't get relief by following the above suggested steps. The simplest method of removing minor surface stains is polishing, or micro-abrasion if staining is more intrinsic. Provided that your discolouration is not due to inadequate enamel exposing the dentin, tooth whitening (bleaching) is often the most effective treatment for yellow stained teeth. Advance procedures can be done with laser. Use snowdent tooth paste for whitening until then. Apply snowdent paste on the teeth. Wait for 10 to 15 min. Then brush your teeth with the same paste. Applying it overnight for better results.
