Thyroid is a small butterfly shaped gland, which  is present at the lower frontal region of the neck, right beneath the voice box. It produces hormones which regulate metabolism (the breakdown of food by the body to convert it into energy). It even plays a pivotal role in boosting organ functions as well as in helping the body to sustain heat. However, too much of hormone production by the thyroid gland might yield structural problems, for instance, growth of nodules (abnormal tissue growth) or cysts (non-cancerous sac-like structures containing fluid) and swelling. Hence, a thyroid surgery is a must once these problems occur. The surgery, administered with general anesthesia, eliminates the thyroid gland either wholly or partially.

Why do you need surgery?

  1. The presence of tumors or nodules on one’s thyroid gland is one of the reasons why one should go for the surgery. Although most of the nodules are benign, few can be pre-cancerous or cancerous too. Also, those benign nodules can spell trouble, if they expand in size, thus obstructing the throat. They can be problematic as well if they cause the thyroid gland to overproduce hormones, giving rise to a condition known as hyperthyroidism.
  2. Hyperthyroidism can be corrected through surgery. It is often an outcome of Grave’s disease, an autoimmune disorder wherein the body misidentifies the thyroid gland as a foreign body, thereby creating antibodies to combat it. The thyroid gland gets inflamed in the process, resulting in the overproduction of hormones.
  3. Another reason is the enlargement or swelling up of the thyroid gland, termed as goiter. Similar to large nodules, goiter too can clog the throat, thus interfering with one’s breathing, speaking and eating.

Types of Surgery

  1. Lobectomy: This procedure calls for partial removal of the lobes when a nodule or an inflammation affects just half of the thyroid gland.
  2. Subtotal Thyroidectomy: Here, a small proportion of the thyroid tissue is left behind even after the elimination of the thyroid gland.
  3. Total Thyroidectomy: Through this procedure, the entire thyroid gland is taken out along with the thyroid tissue.

You are using very low dose and I find it very difficult to believe that such a low dose corrected your TSH levels. First, consult a physician and follow the advise of the physician. Hypothyroidism is a chronic disease. It is not curable. You have to take the medicine lifelong or risk getting complications.

Change your lifestyle, walking for 45 minutes minimum in a day, also do some pranayam and yoga ,take methi water, avoid heavy meals, curd,spicy meal, junk food and drink lukewarm water alway. Take castor oil with milk when you go to sleep.

Thyroid is a hormonal organ. Hormones are signals in our body which effect various vital processes of the body. You can link them to traffic lights for the smooth and safe flow of traffic in the body. Unfortunately due to several known and unknown reasons, some hormonal organs like thyroid start to underperform or over perform leading to hypo or hyper function respectively. When we prescribe medicine for hypothyroid patients - we just provide supplementation of deficient hormone in the body. We currently have no medicine to cure the thyroid gland - we just have medicine to fulfil the hormone that was supposed to be manufactured by the thyroid gland. 7 Diabetes Related Skin Problems 

Diabetes is a metabolic disorder, chiefly caused by insulin fluctuation in the body. A study shows that, 1 out of 3 people suffering from diabetes are likely to develop a skin condition sooner or later; and in some cases, these skin problems serve as symptoms of diabetes. Although these skin problems can be cured easily if treated early, proper care must be administered at all times. Here are 7 skin conditions that affect diabetics:

1. Bacterial infection

Although anyone can get a bacterial infection, people suffering from diabetes are likelier to contract them. Usual bacterial skin problems that tend to trouble diabetics are boils, lumps near the eyelids, nail infections and carbuncles. Diabetics are also prone to infections deep in the skin and of the underlying tissues. The affected area is usually warm, red and swollen.

2. Fungal infections

People who have diabetes have an increased risk of contracting the fungal infection, especially the one known as candida Albicans. The fungus tends to create a red, itchy rash, which is surrounded by blisters and scales. These are usually found in warm places of the body like the armpits, or between the fingers of the hand or perhaps even the toes. Common fungal infections in diabetic people include athlete's foot, jock itch and ringworm.

3. Tingling and itching

Among the many causes of itching, diabetes is one. Yeast infection, dry skin and poor circulation are the primary causes of itching among patients of diabetes. Usually, the lower part of the leg tends to itch the most, if poor circulation is to be blamed.

4. Skin problems related to neuropathy

Diabetes can be the cause of nerve damage in some cases; and neuropathy is a common symptom of diabetes. This type of damage can cause a loss of sensation in a part of the body. On event of an injury, one may not be able to feel it. As a result, an open sore or wound, if left unattended, can develop into an infection.

5. Digital sclerosis

Digital sclerosis, a condition marked by tight, waxy, thick skin that develops on the back of the hand, is common among people who have type 1 diabetes. In this condition, the joints of the fingers get stiff and, thus, lose their mobility and elasticity. Skin of the toes, forehead and knees may also get affected.

6) Xerosis

Xerosis is another common skin condition that accompanies diabetes. In this condition, the skin becomes very dry and itchy.

7) Acanthosis nigricans

Acanthosis nigricans is the condition where diabetics get darkening of skin leading to skin tags around neck and armpits.

When the sugar concentration in the blood increases suddenly and unreasonably, the resulting condition is known as diabetes. Sugar or glucose is generally used in order to provide much needed energy to our bodies. But when the pancreas dysfunctions, it leads to a problem in the insulin production. As a result, glucose or sugar gets directly absorbed into the blood, instead of getting absorbed by insulin. And this leads to the condition of diabetes.

Diabetes Treatment in Ayurveda
For Ayurvedic treatment of diabetes, the first step is usually dietary planning and lifestyle change. Adopting a more active lifestyle, and a healthy, balanced diet low on sugars and starches, is a must. You must begin by avoiding sugars in all forms. This means you will have to get rid of rice, potatoes, white bread, sugar coated cereals, bananas, colocasia and much more. Add a lot of green leafy vegetables to your diet to improve your nutritional status and metabolism. In addition to these, also add some herbs to your diet. Ayurvedic herbs that act as natural medicine for diabetes include turmeric, bitter gourd, gurmar leaves, bael, fenugreek and many more.

Apart from diabetes Ayurvedic treatment, you could also practice yoga, which can help you improve your health. Several yogic asanas help massage your internal organs so that they are healthier and can function much better. Some of the asanas are especially beneficial for the pancreas, which produces insulin. You may have to make some changes to your lifestyle as well. For starters, you will have to maintain a more active lifestyle. You will also have to avoid sleeping in the daytime. Avoid smoking and consuming alcohol and take extra care of your feet.

As an ancient science based on lifestyle related treatment methods that include herbs, massages and life changing tweaks, Ayurveda also provides various methods of managing this ailment. Here's how!

  1. Bitter Gourd: Known as an ingredient that is rich in antioxidants, this can be consumed to take care of diabetes. You will need to first remove the peel of at least four to five bitter gourds before crushing them to create a paste. This paste has to be put through a sieve so that the juice of the gourds can be extracted. You will then have to drink this juice every morning, on an empty stomach.
  2. Cinnamon: From treating coughs and colds to diabetes and other serious ailments, this is a wonder spice that can be found in most Indian kitchens. You can start by boiling a litre of water and letting it simmer on the gas for about twenty minutes. Thereafter, you can add three teaspoons of cinnamon powder to this simmering water. Strain this mixture and drink the entire batch every day to clear out the toxins.
  3. Fenugreek Seeds: For this, you will need to take about four to five tablespoons of fenugreek seeds. These will have to be soaked in a glass of water or about 250ml of water, overnight. In the morning, the softened seeds will need to be crushed before the mixture is strained. The water that escapes should be collected and ingested every day for a period of two months.
  4. Turmeric: Turmeric is a powerful and well-known ingredient that is used for curing and managing many ailments. You will need to mix about half a teaspoon of turmeric powder with one teaspoon of amla powder as well as half a tablespoon of methi seeds or fenugreek seeds powder. This concoction will have to mixed in half a glass of water and taken daily on an empty stomach. This can be used by patients suffering from heart ailments as well.
  5. Diet: Cereals like ragi, corn, kulit, green millet, and barley are known to beneficial for patients suffering from diabetes. Leafy greens are also a must for such patients along with protein-rich foods like soya beans.

Hello Madam, Generalised body pains can be due to many reasons- like Anemia, low calcium and Vitamin D levels, hypothyroidism, lack of proper nutrition in your diet leading to multivitamin deficiency etc. You can get a complete blood picture, calcium and vitamin D levels, TSH levels checked and consult a general physician. High blood suagr and pressure can cause changes in kidney. Think of it like the holes of the filter are bif and protiens are leaking out into the urine. He will need to control both sugar and pressure very fast and keep it maintained in normal levels.

Mr. lybrate-user, I have gone through your query, and understand your concern. If diabetes is uncontrolled, then there is considerable loss of glucose through urine, due to which you feel exhausted and also loose weight. Blood glucose 445 mg is too high. It needs to be controlled rapidly. Ideally blood glucose should be Fasting closer to 100 mg, PP 150 to 160 mg & HbA1c%<6.8, then only one is able to prevent or postpone all the long term complications of diabetes. Otherwise there can be very severe consequences. You have not mentioned whether there is any treatment going on currently, if yes give details. If not then it has to be initiated forthwith. For which you will have to come in a private conversation with following details: Fasting, PP blood glucose levels, HbA1c%, dietary patter, extent of daily exercise. Based on those I will be able to guide and get your diabetes under control. In this communication we are supposed to give only general guidelines and no prescriptions. Thanks.

For the benefit of couples suffering from infertility, modern medical science has introduced several innovative procedures. Some of the popular procedures are In-vitro Fertilization (IVF), Intra Uterine Insemination (IUI), Gamete Intrafallopian Transfer (GIFT), Intracytoplasmic Sperm Injection (ICSI), donor eggs and embryos and so on. In addition to these, there are several drugs and surgical procedures that help the couple in getting rid of infertility. Among all these procedures, IUI has gained popularity in the field of gynaecology and infertility treatment procedures. The IUI treatment is also popularly called as artificial insemination procedure. Although this is a popular procedure, it is appropriate that you should also understand its pros and cons.

IUI procedure in brief:
In simple terms, the IUI procedure involves placing the sperm inside the womb or uterus, which in turn would assist in fertilization of the egg. As a result of this procedure, the sperm reaches the fallopian tube, which enhances the chances or rate of egg fertilisation.

Conditions precedent of IUI Procedure:
Before initiating the IUI procedure, the fast moving eggs are separated from the slow moving eggs. This separation is done in the laboratory. Further, in order to undergo IUI procedure, the women should be less than 40 years of age. On the other hand, apart from healthy fallopian tube, the women should also have higher ovarian reserves. Also, the sperm should have minimum mortality rate. However, IUI procedure is adopted only if the fallopian tube is healthy. IUI procedure is suggested in case the couple is having difficulty in vaginal intercourse, either because of psychosexual reasons or for reasons of physical disability.

The IUI procedure can be performed either with the partner’s egg or with the donor’s egg. Some of the other important aspects of IUI procedure are briefly discussed here:

  1. The IUI procedure is a short duration procedure and it can be completed within a few minutes. This procedure does not cause any discomfort or pain. The procedure does not require any hospitalisation or administration of anesthesia. Further, the procedure also does not cause any side effects. In fact, compared to the IVF procedure, IUI is cost-effective.
  2. In order to enhance the level of success, the gynaecologist may suggest IUI procedure every month. In some cases, the gynaecologist may also prescribe a few medicines to simulate the ovulation procedure. Except this, IUI may not involve extensive medication.

Summary:
You may undergo the IUI procedure under the supervision of sufficiently experienced gynaecologist. Further, you may also ensure the hospital is equipped with modern state of art machineries for carrying out the IUI procedure.

All About Congenital Hypothyroidism!

What is congenital hypothyroidism?
Newborn babies who are unable to make enough thyroid hormone have congenital hypothyroidism, meaning they are born with an underactive or absent thyroid gland.  If the condition is not found and treated, children can have mental retardation and growth failure. But the good news is that early diagnosis and proper treatment can prevent these problems. In most cases, the condition is permanent and your child will need lifelong treatment. 

What causes congenital hypothyroidism?
 The most common causes are

  1. An underdeveloped thyroid gland
  2. A thyroid gland that’s not located where it should be (in the neck below the voice box or larynx)  
  3. A missing thyroid gland

 Other possible causes include

  1. Defective production of thyroid hormone (an inherited condition)
  2. Problems with the pituitary gland (located at the base of the brain), which tells the thyroid to make thyroid hormone
  3. Less commonly, a mother’s thyroid disease or medicines taken during pregnancy can cause congenital hypothyroidism.

What are the signs and symptoms of congenital hypothyroidism?

Many babies with congenital hypothyroidism appear normal at birth or for several months after birth.  But others may have these signs and symptoms:

  • Jaundice (yellowing of the skin and eyes)
  • Constipation
  • Poor muscle tone
  • Sluggishness
  • A hoarse cry
  • They might also have
  • Feeding problems
  • A thick, large tongue
  • A puffy-looking face
  • A swollen abdomen, sometimes with “outpouching” of the belly button
  • Large soft spots of the skull

Thyroid hormone deficiency can also occur in older babies or young children, even if test results at birth were normal.

Treatment
Early diagnosis is very important. Most of the effects of hypothyroidism are easy to reverse. For this reason, doctors always ask to get screening for hypothyroidism of your newborn.

Thyroxine is usually given to treat hypothyroidism. Once the child starts taking this medicine, blood tests are regularly done to make sure thyroid hormone levels are in a normal range.

When to Contact a Medical Professional
Call your health care provider if:

  • You feel your infant shows signs or symptoms of hypothyroidism
  • You are pregnant and have been exposed to antithyroid drugs or procedures

Prevention
If a pregnant woman takes radioactive iodine for thyroid cancer, the thyroid gland may be destroyed in the developing fetus. Infants whose mothers have taken such medicines should be observed carefully after birth for signs of hypothyroidism.
