Hi, kindly get a Ultrasound of abdomen to rule out gall stone. By that time you can take cap. Pantocid-DSR (1-0-1) half an hour before meal for 5 days. Report me later.

My rectum swallow and pain sometime around rectum while seat. But no bleeding. Pain while go for stool. Stool is two color in one bowl moment half is dark brown and half is light color. And some time green color stool. I suffering from acid reflex symptoms is lump in throat after food and burping. No heart burn.

Hi Lybrate User, you are suffering from Fissure in Ano (Cut in anal canal). Take plenty of water and salads in diet. Take Syp. Cremaffin Plus 30 ml at night to make stool soft, Apply Ditigesic 0.2% ointment 3-4 times a day after passing stool. Report me after 5 days about the progress.

Hi lybrate-user, If you don't have pain abdomen especially in right upper quadrant, then it means you have asymptomatic gall stone which have been detected incidentally. In case don't worry, take plenty of liquids, Avoid oil rich food. This will not harm you life long. If you have symptoms, as mentioned above, kindly opt for a Laparoscopic Removal, as there is no treatment at present to dissolve gall stone as is available for kidney stone (because kidney stone composition is different from gall stone, thats why kidney stone can be dissolved)

Hi, generally fistula doesn't cause pain. It may be a fissure or a perianal abscess. Just consult your doctor again. Get a Trans anal Ultrasound done to get a proper diagnosis.

Recently I had abdominal pain, my doctor suggested to scan whole abdomen. I have undergone a ultra sound. Nothing has come. But one thing has mentioned that is splenic specs of calcification .my doctors said nothing just ignore it. Its very common has told. But I need this opinion shall I go for another doctor for opinion. Or can any one please suggest. In the result mention splenic calcification? Old granulomatous lesion. Can any one please explain?/shall I proceed for any other doctor opinion or not.

Sir do not give her both calcium tablet and Iron syrup together, as calcium neutralizes iron. You can give her Lumia 60 k cap - 1 per week till pregnancy. If she is not tolerating iron syrup, try capsule - orofer xt daily.

I am a 34 year old woman. I have severe constipation. I had urinary Track Infection that lasted for more than a month. But it's medications or other side effects lead to constipation. It initially subsided with Duphalac. But relapsed again for last 4-5 days. It's neither going by Duphalac nor Isapgol. I took home enema day before yesterday, that solved it for that day. But yesterday it happened again so I didn't take enema.

Continue Isabgol (4-5 scoops in luke warm water) after dinner. Take Movicol Sachet - 2 sachet after dinner daily for 4-5 days. Drink plenty of water. Take carrot, cucumber, beet root with each food. Get your Urine culture done (give the first morning urine sample only)

If there is no right upper pain abdomen. Don't do any thing, just forget about it. Currently there is no medicine available in market to dissolve it.

Gall Bladder Stone

Definition:   Presence of stones in the gallbladder is referred to as cholelithiasis.               

PredisposingFactors:

  • Genetics , Gender (more in women)
  • Body weight (Obesity), or rapid loss in weight , Fasting
  • Decreased motility (movement) of the gallbladder
  • Diet
  • Drugs : Cholesterol / Lipid lowering drugs , Estrogen
  • Diseases: Diabetes, blood disorder (Hemolytic anemia etc.)

Composition of Gall Bladder Stone:

Cholesterol Stone: Found in western world, causes accounted to diet rich in cholesterol.

Pigmented stone: generally found in diseases related to blood disorder

Mixed stone: Contains Calcium, magnesium, sodium phosphate ( most common type found in Asian population (India).

Symptoms :

  • (70–85%) are asymptomatic or "Silent Gall Stone"
  • Pain in right upper and central part of abdomen (may radiate to inter-scapular region) mostly after half an hour of food (seen more symptomatic after a fatty diet.)
  • post meal fullness, belching, upper abdominal discomfort
  • Jaundice (yellowing of the skin or eyes)
  • Clay colored stools or dark urine

Investigations required:

  • Blood - Liver Function test , Complete Blood Count,
  • Imaging -Ultrasound Of abdomen and if required MRI (MRCP) abdomen.
  • Complications of Gall Stone
  • Infection of gall bladder {Empyema(pus) / Mucocele of Gall bladder}
  • Perforation of gall bladder
  • Stone slipped in Common Bile Duct resulting in obstruction in the passage of bile leading to Jaundice
  • Stone from gall bladder slipped to common bile duct and entering into pancreatic duct through common channel of opening in Duodenum (small bowel) leading to Pancreatitis.

Indications for Surgery:

  • Symptomatic Gall stone with typical history of pain as mentioned above and proven with Imaging (ultrasound)
  • Asymptomatic or Silent gall stone (Found Incidentally during Ultrasound abdomen) - "DOES NOT REQUIRE SURGERY".

Asymptomatic or Silent gall stone requiring surgery in special circumstances are:

  • Pregnant lady diagnosed with gall stone during first trimester (incidental finding)
  • Any blood disorder and patient is going for any abdominal surgery for any reason.
  • Diabetes mellitus - as chances of infection are high on by ground of diabetes
  • Gall bladder polyp (a soft tissue growth in lumen of gall bladder) size > 1 cm or single stone measuring > 3 cms, as risk of Gall Bladder cancer increase by 10 fold in these cases.

Treatment: Surgery - Laparoscopic Cholecystectomy (Key Hole Surgery) is Gold standard worldwide.

  • Safe surgery, there are no side effects of removing of gall bladder as one has to weigh the complication arising from a "symptomatic" gall stone disease.
  • Can be done as a day care surgery
  • Post surgery patients are advised to refrain from fatty meal for a week so as to acclimatize the body to a diluted bile. Normal activity is allowed from day 1 of post surgery.

We had done Sonography yesterday and it has shown stone in gall bladder of 8-10 cms. We are going for surgery asap. Would like to know in one of my friends case he had cancer cells in gall bladder but they didn't know and had done laparoscopic surgery and the cancer spread to whole body. As a precaution what should be done to avoid such situation. Pls your urgent advise is required.

First of all, any incidental finding of a gall stone is not an indication for surgery. Your stone size may not be 8-10 cms, which you have written, it would have 8-10 mm I feel. Asymptomatic gall stone should not be touched. If one is willing to go ahed of surgery, A Laparoscopic approach is gold standard. If in histopathology its, a cancer, then one will have to find out the limit of spread (thickness/layer of gall bladder involvement, depending upon next surgery needed.
