Is There A Relation Between Weight Loss Surgery & Type 2 Diabetes?

MBBS, MS - General Surgery
Bariatrician, Mumbai
Is There A Relation Between Weight Loss Surgery & Type 2 Diabetes?

Is there a relation between weight loss surgery and type 2 diabetes? Yes, there is and a very promising one. Many diabetics may not know this but a key hole procedure called metabolic/ bariatric surgery can help resolve type 2 diabetes.

It’s scientifically proven that these surgeries can normalize blood sugar levels and control diabetes in most patients. This means that you may not need any medicines post such a surgery. One long-term study which tracked 400 people with type 2 diabetes showed that six years after bariatric surgery, 62% diabetics showed normal blood sugar levels. Apart from this, they also showed better blood pressure, cholesterol and blood triglyceride levels that can cause heart disease.

Should you get a metabolic/bariatric surgery done? You are a fit candidate for bariatric surgery if your:

  1. BMI is 27.5 or higher
  2. And if are finding it difficult to maintain blood sugar with medicines & lifestyle changes.

Your doctor will give you a physical exam to check whether you are fit for a major surgery and if you are emotionally ready to commit to necessary lifestyle changes like eating healthy and exercising regularly.

Types of weight loss/diabetes surgery:

The most popular type of weight loss surgery is one in which the size of your stomach is reduced so that you feel full even after small meals. Other types of bariatric surgeries change the way your body absorbs calories and nutrients and vitamins. A few other types of bariatric surgeries do both.

  1. Gastric Bypass: In this surgery, your surgeon makes a small stomach pouch by dividing the top of the stomach from the rest. This small pouch is connected to intestine so other part of stomach is bypassed. So, when you eat, food goes into this small pouch, which makes you feel full faster and also makes it possible for your body to absorb fewer calories. This is a very promising type of bariatric surgery for diabetics as up to 80% of people have normal blood sugar after it. And they also lose 60% to 80% of their extra weight on an average.
  2. Gastric Sleeve: In this surgery, your surgeon will remove a major part of your stomach. When your stomach is small, you will feel full faster. It also reduces hunger pangs. This type of surgery is also very effective in reducing diabetes and losing weight.
  3. Adjustable Gastric Band: In this surgery, your surgeon puts an inflatable band around the top of your stomach. This forms a small pouch which makes you feel full faster. The good point is that your stomach is not cut and your intestines are not shifted. There are thus lesser complications. It also helps in controlling diabetes. The con is that you lose less weight in this type of surgery.
  4. Biliopancreatic DiversionThis is an uncommon surgery but most effective for diabetics. In this the doctor removes a large part of your stomach and also changes the way the food goes to your intestines. This helps in losing weight but also affects digestion long term.

As with any major operation, all bariatric surgeries come with potential risks of bleeding, infection, and leaks in the digestive system. It is necessary to change lifestyle to avoid weight regain.

Liposuction - Do You Need It?

Liposuction - Do You Need It?

Liposuction is a surgical procedure of removing stubborn fat bulges from the body. Age is not a major consideration with liposuction as long as your skin is firm and elastic. Else, your skin may begin to sag in places from where the fat has been removed.

Liposuction surgery techniques involve ultrasound and laser for breaking down fatty substances and ultimately removing them. After a liposuction surgery, you may not see the results immediately. Compression garments and/or elastic garments are used to get your affected body part to adapt to its new shape.

When should you opt for the procedure?

  1. You are putting your best efforts to gain a contoured body: If you are doing regular work out and stay very much focused on achieving a stunning contoured body or in other words, visually chiseled abs and arms, but still fat doesn't go away then liposuction surgery can help you achieve a complete body contouring you so wish for.
  2. Despite exercising and eating healthy, you are unable to shed stubborn fat: It is quite natural to lose weight after exercising and eating healthy. But even if you manage to attain a leaner and fit body, some fatty deposits are often left behind. So when your daily efforts prove insufficient and you see no striking change, then it is quite likely that you should go for liposuction which will help you eliminate excess fat from some specific parts of your body.
  3. Even though you are happy with your body shape, it seems you have a double chin: If it appears that you have a double chin despite having optimum weight, you are perhaps putting up with fat under your chin and possibly around your neck which you are unable to address on your own. Liposuction, therefore, can help you get rid of these fat deposits for a smoother and youthful appearance that complements the rest of your body.
  4. You are embarrassed by your body shape due to bulging or sagging body areas: If your body appearance makes you feel embarrassed because of sagging body areas you cannot address, then liposuction can easily help give you back your self-confidence. If a smoother and fit body shape can help you improve your social and personal life, then this can be the cosmetic procedure that makes your life turn around.

Colorectal Surgery - Know Disorders Treated Through It!

Colorectal Surgery - Know Disorders Treated Through It!

Colorectal surgery is the broad term for surgical procedures performed on the colon, the rectum and the anus. There are various different surgical procedures which fall under colorectal surgery and these are used to treat a vast array of disorders, such as:

1.  Anal cancer
2. Colorectal cancer
3. Severe complications related to constipation
4. Anal injuries
5. Inflammatory conditions of the gastrointestinal tract
6. Congenital defects

Procedures

The surgeries under colorectal surgery are performed after diagnostic tests such as proctoscopy, sigmoidoscopy and defecating proctography. The most common diagnostic test is colonoscopy. These help to identify the origin and nature of the problem and decide which surgical procedure is to be followed. The procedures under colorectal surgery are as follows:

1. Colectomy
This procedure involves the removal of a section of the large intestine. This is known as a partial colectomy. In extreme cases, such as advanced cancer or severe gastrointestinal infection, the entire colon is removed and this is called total colectomy. Sometimes, the rectum is also taken out along with the colon and this is called proctocolectomy.

2. Colonic polypectomy
Abnormal growth of tissues of the inner lining of an organ is known as a polyp. Colonic polypectomy is done to eliminate polyps from the colon and rectum before they become malignant. This can be done endoscopically. Surgery is required in case of large polyps.

3. Strictureplasty
Chronic or repeated bowel inflammation causes scar tissue to accumulate in the large intestine. This results in the narrowing of the colon. Stricturoplasty removes the scar tissue so that proper flow of digestive contents is resumed.

4. Colostomy or Ileostomy
A damaged section of the colon is removed and the shortened intestine is then attached to another opening (stoma) in the anterior wall of the abdomen.

5. Hemorrhoidectomy
This surgical process is used for swollen hemorrhoids or blood vessels which form in the anal canal. Hemorrhoidectomy is extremely effective in removing hemorrhoids but the surgery also involves a number of complications.

6. Anoplasty
Anoplasty or imperforate anus correction is done to correct birth defects in the rectum and the anus. The structural flaw does not allow the stool to pass properly from the rectum and so it is repaired using surgery.

How Does Sugar Affect Your Skin?

MBBS Bachelor of Medicine and Bachelor of Surgery, MD- Dermatology, Venerology & Leprosy

How Does Sugar Affect Your Skin?

Unbeknownst, you often consume a much higher amount of sugar than you actually realize, as it is widely added in a very high number of foods and beverages. Research suggests that this excessive sugar consumption may ultimately be very harmful for your health - and that it may completely wreck your wishes for smooth and supple skin. Experts believe that overeating sugar can make your skin appear to be wrinkled or dull, and also lead to a host of other skin-related disorders.

Here are the ways in which excessive sugar consumption affects your skin:

Causes inflammation:

Inflammation, caused by excess sugar intake, can be really damaging for the body - especially because it negatively impacts your immune system, and may lead to frequent breakouts on the skins and increased chances of pimples or warts.

Increases wrinkling:

Sugar's inflammatory properties not only lead to breakouts on the skin, but also cause wrinkles. Collagen, which is essential for healthy, supple skin, is broken down by sugar, which makes the skin look dehydrated and dull, also increasing chances of premature wrinkling.

Causes imbalances in blood sugar level:

Excessive sugar consumption causes irregularities in blood sugar levels by initially increasing sugar levels, which then reduces when insulin in released into your cells. It often ends up making you hungry, moody or shaky and increases stress levels leading to dull looking skin.

Leads to occurrence of acne:

Increased intake of sugar results in stimulation of hormone levels which raises sebum production and in turn affects growth of skin cells. This may be reason of acne.

Causes dehydration and irritation:

Sugar consumption in high amounts causes rashes or redness chiefly due to inflammation, which leads to yeast imbalances in your system. It also causes dehydration, by drawing out the water from your cells amounting to dry and puffy skin. Drinking adequate amounts of water and reducing sugar intake is essential in this regard.

What Must Be Done Before Considering A Chemical Peel?

Are you bored with how your skin appears? Well, there are techniques to give it a new life. Your skin’s natural baby look can be back with techniques like chemical peels. It is a technique that can improve the appearance of your skin. A chemical solution is put on the skin. It causes the skin to exfoliate and later beef off. The new skin looks smooth and it is free from wrinkles. The new skin is more sensitive to the sun on a temporary basis. There are three types of chemical peel:

  1. Superficial or Lunchtime Peel: In this technique, alpha-hydroxy acid or some mild acid is made to penetrate into the outer layer of skin. This exfoliates the skin. It helps to deal with mild skin discoloration and rough skin.

  2. Medium Peel: Glycolic or trichloroacetic acid is put on the skin so as to penetrate the middle layers of skin. It removes damaged skin cells. The treatment is used for improving age spots, fine lines and wrinkles.

  3. Deep Peel: Trichloroacetic acid or phenol is used for penetrating the middle layer of the skin. It will help in removing moderate lines, and shallow scars.

What must be done before considering a chemical peel?

It is always advisable to consult a dermatologist first before taking up the decision to try a chemical peel.

What are the skin flaws that can be treated by chemical peels?

Chemical peels are used for treating a number of skin problems, including the following ones:

  2. Ageing skin

  3. Crow's feet

  4. Scars

  5. Sun damaged skin

  6. Sagging skin

  7. Wrinkles

It is generally believed that light-haired and fair-skinned people can go for chemical peels. It doesn’t go well with dark-skinned patients. It is also not recommended for people living with some infection.

Laser Hair Removal - Must Know Things About It!

Laser Hair Removal - Must Know Things About It!

Body hair are typically seen as a manly attribution. For this reason, both men and women are constantly trying to keep their body hair free. Common ways of removing body hair includes shaving, waxing and the use of Depilatories. However, none of these techniques can permanently remove hair and hence need to be repeated regularly. The only way to permanently remove body hair is through laser hair removal therapy.

Laser hair removal can be used to address and remove hair on almost any part of the body including the face, arms, legs and bikini line. This procedure involves focusing intense light on the hair follicle pigments known as melanin without affecting the surrounding skin. The melanin absorbs this light energy and converts it to heat energy which in turn stops the stem cells in the hair roots from regenerating. This weakens the hair follicles to the point of falling off.

Hair removal is considered permanent when the hair follicles do not grow back. After the first session, new hair growth is often lighter and less dense than before. On an average, complete hair removal may require 5 to 6 laser sessions. Since laser therapy can be used to address only those follicles that are in the growing stage, these sessions are usually spaced 4 to 6 weeks apart. During this time the hair follicles that were in the resting phase can grow out. The number of sessions required also depends on the quality and number of hair follicles, individual’s hormone levels, and metabolism. In some cases where hair growth is very dense, maintenance treatment may be required once in six months or annually.

The procedure is relatively pain free. During the treatment, the patient may experience a faint prick or sensation of warmth. When treating sensitive areas, topical anesthesia may also be used. However, this is rarely needed. This form of laser therapy can take anywhere between 5 to 45 minutes for a single session depending on the amount of area being addressed.

Laser hair removal is considered a safe procedure and has minimal side effects. In some cases, it may cause a temporary change in the color of the skin of the treated area, but this usually resolves itself without medication. This can be prevented by using a good sunscreen with UVA and UVB protection and avoiding exposure to sunlight. In rare cases, people with a dark complexion may develop blisters and experience swelling and redness of the skin.

Sensitive Skin Care - 5 Must Know Things For You!

MBBS, MD (D.V.L.)
Dermatologist, Kolkata
Sensitive Skin Care - 5 Must Know Things For You!

Sensitive skin can make life miserable. Factors, such as environmental and climatic changes, certain beauty products and medications can affect individuals with sensitive skin. There may be skin inflammation, irritation, rashes, redness, dryness and flakiness. The extent of the sensitive reaction can vary from one individual to another.

In many cases, it has been observed that sensitive skin appears to be thinner than the normal skin. Thus, people with such delicate skin type need to be extra careful. Do not try self medication. It will make things worse. Always seek an expert advice. A dermatologist knows what is best for your skin.

People with sensitive skin need to follow some basic skin care regimes:

  1. It is very important to keep a sensitive skin hydrated and moisturised all the time. A dehydrated skin not only appears tighter, but also results in extreme skin dryness and irritation. Drink at least 6 to 8 glasses of water daily. It is very important to flush out the toxins and maintain the alkaline skin pH. Avoid taking very hot water or ice baths. The extreme temperatures will do your skin more harm than good. A warm water bath is an excellent option. Apply generous amounts of moisturiser, whenever your skin feels dry and itchy.
  2. It is best to avoid products that are harsh on your skin. Consult a dermatologist before using products containing alpha hydroxyl acids. These are strong acids and their application can lead to skin irritation and rashes. It is a healthy practice to keep your skin clean. However, not all soaps are suitable for sensitive skin. Soaps (strong alkali) are a big no. It is best to use a medicated soap, specially formulated for sensitive skin. To cleanse your face, use a mild (soap free) face wash or a cleanser.
  3. For people with sensitive skin, it is best not to experiment with the skin care products. Play it safe, stick to the brands you have been using for long. Certain beauty products are strongly fragranced. These can trigger severe skin and allergic reactions. Lanolin, artificial mineral oil and alcohol containing products are no better. It is best to use natural and herbal skin care products.
  4. Do not use cosmetics and beauty products beyond a certain period of time. Face powders, silicone based foundations, sunscreen (titanium oxide or zinc oxide based) are best for sensitive skin. These products are known to be less harsh and thus have little or no side effects.
  5. Before using any products, know the ingredients and chemicals that goes into its manufacture.
