I've been smoking for last 3 years and I want to quit this bad habit. I tried number of times but failed to quit because of the craving and headache. Please tell me some medicines which can help me in quitting smoking.

Dear lybrate-user, Please try to stop smoking at the earliest. It has many health benefits in the long run. When you stop suddenly, you may get withdrawal symptoms like restlessness, irritability, lack of concentration, increased hunger, sleep disturbance, headache, impatience, anxiety, depression, etc. If you have them please don’t get back to smoking, take nicotine gum or lozenge or patch. They are available in all medical shops. You don’t need a prescription from doctors. Use 2 mg gum if you smoke less than 25 cigarettes/day. Use 4 mg gum if you smoke more than 25 cigarettes/day. First 6 weeks, use 1 gum every 1 – 2 hours. Reduce it in week 7 – 9 to one gum every 2 – 4 hours. In week 10 – 12, reduce it further to 1 gum every 4 -8 hours. By 3 months, you will be able to stop both cigarettes and gum fully without any withdrawal symptoms. Please feel confident that you can do it and give yourself some gift for doing it like vacation or books or new mobile. Smoking is a chronic relapsing condition like diabetes and B.P. So please don’t worry if you restart smoking. Start all over again to stop. You will learn the situations that make you smoke and avoid them. Please think of health benefits and money saved often. Plan how to avoid relapsing situations, like smoking friends, along with alcohol, stressful feeling or depression. Delay, drink, deep breathe, distract and discuss are the helpful tips. Increase of non smoking zones, gift yourself for stopping smoking. Even chewing cinnamon may delay. Please remember that the urge to smoke comes and goes like waves. Delay and see whether it goes away. It will like a passing cloud.

I have consulted a neurologist of apollo hospital hyderabad but he has tried to hide the things for mmy good sake saying that I am just in depression and now my legs are getting heavy and weak gradually. Dear lybrate-user, Dementia is an old/middle age problem. Memory loss at your age, will NOT be due to dementia. Probably your neurologist is right in saying, it is depression. He is NOT hiding dementia. After 2-3 weeks of Stalopam (antidepressant - not sleeping pills - take it in the morning itself) and betacap, get back to him. He may adjust the dose depending on your memory abilities and fluency. Or he may refer you to a psychiatrist for better management of your preoccupation of dementia.

Dear lybrate-user, Please write in detail about the medicines that you are taking. Mercury amalgam does not have any effect on schizophrenia or sleep disturbances. If you are finding any improvement with medicines, you should not think of get out of medicines. Job is a must at this age of 32 years. It is not for earning alone. It is to keep your brain occupied. If your sleep is disturbed, please consult your psychiatrist again, he/she may diagnose and treat it.

Hi, I'm a acute transient psychosis patient diagnosed at the age of 14. Initially my dosage was very high now im 25 years old consuming 500 mg Divalporate and 50 mg Quitapin plain. Im good when I do not have any relapses and very effective. I completed B. Tech Mechanical with 60 % and got placed in a company as well. Right now im doing software courses. I have few questions: 1 .When I will get rid of my medicine? 2. With these dosage if I can act like a average person so I wonder if im not consuming these medicines I will be more effective with work as well as studies but my relapse will happen in a year or sometime after 2 years. So, is there any alternative for these medicines? 3. I visited many doctors, some say you are suffering from psychosis disorder, some say bipolar or schizophrenia, some say sleeping problems. Why this many doubts in my case. So, what exactly im suffering from? Though I consumed many dosages initially but when I was reviewed with a doctor (Psychiatrist) He advised me the doctors are giving high dosage it can be reduced to minimum. Since then im in contact with that doctor and that is how my dosage is reduced but I want to know whether it can reduced or is there any need of these medicines. I suffer from relapses in a year, sometime 2 years. I will appreciate if any psychiatrist clears my doubts. Thank you.

Dear lybrate-user, Whether you have transient psychosis or bipolar or schizophrenia or sleeping problem, it is for the psychiatrist to diagnose and treat you properly. With this dose you are without relapse and acting like average person. Why do you want to lose those effects of drugs. Bipolar disorder is a relapsing disorder - sometimes with high overconfident mood sometimes with low self esteem sad mood. Both will be kept under control with valproate and quetiapine. During active phase you may need different dose and during normal maintenance phase you will need different dose. Leave the dose decision to the doctor. Each relapse at this age will stagnate your future - mania because you will lose your name, fame and money - depression you will your energy, interest and may lose life by suicide. Why do you want to have relapses? Continue valproate and quetiapine as prescribed.

What can be help as an anti depressant. Whisky,rum, beer or any other related to alcohol. Rather than all r not gud for health bt in my condtn I prefer to drink bt which will be better. As an anti depressant.

Dear lybrate-user, Alcohol acts as an antidepressant at certain dose - whatever is the drink. The amount of alcohol in that drink is important. But alcohol causes tolerance - it means after some time the same amount of alcohol will not give the antidepressant effect. You may need more. In the course of time, it will give depressant effect instead of antidepressant effect. If you feel like stopping it, you get severe bodily and psychological withdrawal effects. A double edged blade. So, think before choosing alcohol as an antidepressant.

Dear lybrate-user, Please motivate her to seek psychiatric help. Allow her to talk her feelings and give value to her feelings. After consulting the psychiatrist if antidepressant is prescribed, encourage her to take the tablets and counsel her to wait patiently for tablets to work on her brain. It may take 2-3 weeks. Don't ask her to forget family disputes. She too wants to forget - but it comes automatically to her. Please remove sources of harmful suicidal attempts, like tablets, poisons, locked rooms with fan, etc.

I am anjali (18 years old), suffering from major depression since last two-three years, scored 87 in goldberg's depression test and this year was the worst of all I somehow suffered from hypomania as I learned about its symptoms now online. I am not able to judge my awkward behaviour. I even had anxiety attacks not very much but more than rare. Depression took place from last three to four years back leading me to live all alone without telling anyone about my problems as ultimately no one believes in what I say. And everyone including my family members tend to say I talk rubbish, can never stick to a point while explaining something. It lead to me with poor results in school, family and everywhere and all over I am trying to get rid of this disgusting life and tried to commit suicide from last two years but unfortunately I am not able to judge my unusual behaviour which makes me a fool in front of everyone and no one is able to get my awkward behaviour. I always forget things happen just a minute before, have racing thoughts in my mind, always lack self confidence and become nervous but I don't know how I become so much active and become overconfident, excessively cry always but sometimes I laugh excessively and try to joke poorly and realize afterwards what have I done in front of everyone. I am totally confused about me and my life. Even while trying to get rid of this life I think that after this also everyone will blame me of being responsible for all this. What should I do? Please help me out.

Dear lybrate-user, Probably you are suffering from a Bipolar Disorder which has 4 different phases in the same person. 1. Depression - as you have suffered 2. Hypomania or mania - as you have self diagnosed at present. 3. Mixed phase - both depressive and manic symptoms occur together for few days and 4. Normal phase. Instead of taking only a depression scale, please take a Bipolar scale. Since you are only 18 years, psychiatrists may not diagnose without family history or disturbances in your life due to mood changes, etc. Kindly consult a psychiatrist preferably with your parents and get diagnosed. Bipolar disorder can be treated with long term mood stabiliser tablets to improve your quality of life.

Hello team, I am 28 years old and used to smoke for last 2 years, I generally smoke 6 to 7 cegerates daily, The problem has arises that during 2 weeks I found a little lump on the mouth just below the teeth, whenever I smoke the lump has been increased in size, whether that may be made of flesh or a vein, I don't know. Dear lybrate-user, It is better that you consult a dentist or general physician to diagnose the lump in the mouth. Prevention is better than cure. But in future, if you stop smoking, your chances of NOT getting such problems will be reduced. Since it is just 2 years since you started smoking, it will not be very difficult for to motivate yourselves continuously to quit smoking. All the best in your effort.

Dear Anup, Both depression and anxiety occur together. So you need not bother about this or that. i wish you could calm yourself. If so, no need of any medication. If you can not, go in for medication without further suffering.

Dear Vignesh, i am answering after 3 days. I hope you are fine now after acute intoxication effect of alcohol. This is the best time for you to quit both alcohol and smoking. You are young and have other interests to come up in life. Please continuously motivate yourself to quit both from alcohol and smoking for 12 weeks. i know it is difficult but not impossible. i can help you with nicotine replacement gum or patch and anticraving drugs for alcohol if necessary.

I have ocd .which is getting worse over tym. Its been 3 years m suffering frm it. I have episodes of depression too. None of therapy works. Neither medicine nor CBT. What should I do now?

Dear Sukhpreet, OCD is a chronic disorder. It causes lot of distress in you. OCD generally causes depression and high dose of antidepressants are the treatment for OCD. i hope you have tried with highest doses of 2 different type of tablets for sufficient length, over 3 years continuously. i believe your CBT went ahead upto the level of ERP fully, according to the hierarchy of symptoms. If so, you can opt for ECT, rTMS, DBT or gamma knife surgery.

Dear Nawal, It is really sad to see your father dementing. Please let me know the type of dementia CMC or RINPAS diagnosed him. He is too young to get Alzheimer's type of dementia. If we investigate 10 persons, only one of them will have a treatable cause. Unfortunately it looks your father did not have a treatable cause. So we may not be able to bring him back to his original self. But there are few cholinesterase inhibitor kind of drugs available. They at least stop further deterioration of dementia. The care takers need counselling to prevent burn out. Few day care and in house care help them to preserve whatever brain function that are not lost. Please don't spend much money and time in taking him to place to place. Instead spend that money for the care taker or rehabilitation places.

Dear lybrate-user, All the medicines you are taking are short acting medicines. If you can stop all of them and get menses periods, you can plan pregnancy next cycle itself. But before stopping the drugs, think how serious will be if your OCD relapses, will it affect you bringing up the child, will it affect your marriage, will it affect your self care during pregnancy, etc. Prepare your husband and mother or mother-in-law so that they can tolerate your symptoms and care for your marriage and child for few months. However you can restart your medicines after 12 weeks normal u/s scan of child. You have to consult your psychiatrist - she/he may use one drug in high dose instead of 3. All the best.

Dear lybrate-user, You may be evolving into a borderline personality disorder. Depression, cutting wrist, feeling alone, sharing with a boy etc may be due to your personality weakness. Still you have not fixed your personality at 16 years of age. You can develop a better personality if you take treatment like Cognitive Behaviour therapy or DBT. Please consult a clinical psychologist or psychiatrist who is trained in these therapies for help.

Dear Himanshu, Congratulations for your decision of leaving smoking. Nicotine in cigarettes make the bowels move. So, if you leave smoking, you maynot pass motion freely or constipated. It is a common withdrawal symptom. If you persist not to smoke for 12 weeks, it will disappear and you will be able to pass motion freely. Till then take some purgative daily at night. Luckily you don't have other withdrawal symptoms to keep using cigarettes. All the best.

Dear lybrate-user, It looks like your daughter has a mental disorder. If it is a recent behaviour, less than 1 year, her treatment may be completely useful. If it is more than 1 year, the treatment will bring about 80-90% change to her old behaviour. However she needs treatment by a psychiatrist as early as possible. She is qualified. Her carrier and marriage may be affected with this disorder without treatment for long time.

You are requested to kindly help us by informing the safest method to block it like zinc nootropics (eg. Memantine, inositol) or as you know better like ayurvedic, homeopathic remedies). Dear lybrate-user, Any new drug undergoes many stages in research. For every 10000 drugs discovered, harly one drug comes out after the 3 stages of drug development. Mglur5 is still in animal studies stage. It may take another 10 or more long years to come for human trials (not to market) - that too if it works. It may die like all 9999 drugs before that. Both memantine and inositol are available in India. Though not approved for OCD, still you can try, if you can tolerate. Proven medication for OCD now is SSRI. It needs high doses for many months or years. Other treatments are ERP, rTMS, deep brain stimulation, surgery on brain with gamma knife. You have to consult ayurvedic and homeopathic doctors for remedies in their system.

Dear Sohil, Many persons fighting with depression, get addicted to tobacco. Antidepressants and nicotine in tobacco, act on the same nerve cells. So their interaction may be not be effective, if they are used together. Why don't you plan and quit tobacco and take only antidepressant for improving your health? Quitting tobacco may be difficult, but not impossible. Try for 12 weeks continuously. All the best.

I had uterine cancer scare in 2015. I had undergone hysterectomy. Though all the tests were negative for cancer I have developed a fobia of cancer. I always feel sick. My stomach aches all the time. N in dreams also I always see sickness or sick people. Most of the time myself .Is this neurological disorders. I try to remain happy n keep myself busy. But subconsciously I always see myself taking medication and injections. Please advice.

Dear lybrate-user, Health anxiety is one of the disorders of mind. It is not neurological - it is psychological. That means, your mind finds you sick though your brain knows you are not having cancer. This kind of health anxiety is common in hypothyroidism. With whatever dose of thyronorm you are taking, please get TSH level done on your blood. With that report, please consult a psychiatrist or clinical psychologist who can treat you with psychotherapy. It needs many sessions and your co operation. All the best.
