My son had approached me a couple of months ago and said that he has been unable to concentrate for a while and feels that his ability to remember stuff has decreased. I have noticed that for some while he has lacks energy and finds it hard to perform simple tasks. He has been very unstable emotionally and gets irked very easily and frequently. He gets very angry at himself and even harmed himself after a bad exam. His results were also very below par. He wasn't like this until about 6 months ago. I haven't seen him this sad before. I'm worried about his mental health and request you to recommend to me the next step I should take. I anxiously await your response. Thanking you.

Dear lybrate-user, It is very nice of you as father, want to help Rahil. Please take him to a child and adolescent psychiatrist for diagnosis and treatment. In persons with epilepsy, there are many mental disturbances due to epilepsy itself or due to the drugs that he is taking. In adolescent age, it manifests more. There may be other stresses also in him that need to be addressed.

Dear lybrate-user, Please try to stop smoking at the earliest. It has many health benefits in the long run. When you stop suddenly, you may get withdrawal symptoms like restlessness, irritability, lack of concentration, increased hunger, sleep disturbance, headache, impatience, anxiety, depression, etc. If you have them please don’t get back to smoking, take nicotine gum or lozenge or patch. They are available in all medical shops. You don’t need a prescription from doctors. Use 2 mg gum if you smoke less than 25 cigarettes/day. Use 4 mg gum if you smoke more than 25 cigarettes/day. First 6 weeks, use 1 gum every 1 – 2 hours. Reduce it in week 7 – 9 to one gum every 2 – 4 hours. In week 10 – 12, reduce it further to 1 gum every 4 -8 hours. By 3 months, you will be able to stop both cigarettes and gum fully without any withdrawal symptoms. Please feel confident that you can do it and give yourself some gift for doing it like vacation or books or new mobile. Smoking is a chronic relapsing condition like diabetes and B.P. So please don’t worry if you restart smoking. Start all over again to stop. You will learn the situations that make you smoke and avoid them. Please think of health benefits and money saved often. Plan how to avoid relapsing situations, like smoking friends, along with alcohol, stressful feeling or depression. Delay, drink, deep breathe, distract and discuss are the helpful tips. Increase of non smoking zones, gift yourself for stopping smoking. Even chewing cinnamon may delay. Please remember that the urge to smoke comes and goes like waves. Delay and see whether it goes away. It will like a passing cloud. All the best.

I am 42 years female. I am feeling very tired always. I feel my bp is falling but when measured it finds normal. I always trying to lie in the bed. I hurried do all tasks and go to bed. I always feel fatigued and giddy. I have no other ailments like thyroid or sugar etc. Dr. could not find any sever disease still I am almost patient who have all ailments dear Dr. I was suffering from depression for last 5 or 6 yrs. After marriage I could not cop up with the hell like situations and went in depression I took censpram, tayo,felicita od .dr treated me and the levels of vit came normal. But still I am not normal. It seems I will be in coma stage one day. I am worried about my 8 years son after me. I want to live. Please help me Dr. Please suggest all tests regarding any micro or macronutrient, enzyme hormone etc which can help me please. I will be thankful to you.

Dear lybrate-user, You are suffering too long with depression. Don't lose hopes. 20% of depressions are treatment resistant. For those like you, psychiatrists have many treatment options. You can take a course of 6 - 9 ECTs (electroconvulsive therapy) or rTMS sessions, or CBT (cognitive behaviour therapy), or you can add samE, Vitamin D3 (if it is low), thyroxine, 5HT, fish oil, etc. Your psychiatrist will be able to prescribe them for you. Please remember that these treatments are in addition to your censpram, tayo and felicita. That too in maximum dose. So far you have not gone to coma. With all your difficulties, you have looked after and brought your son up to 8 years. Please ask your doctor for add ons.

Dear lybrate-user, Mental illnesses are of different types. Each one has different symptomatology. Your sister-in-law's changing moods and withdrawn behaviour (not talking to anyone) are symptoms of mental illness. That does not mean, that your sister-in-law is mentally ill. There are some more criterias and tests that need to be done, before we diagnose. Therefore, please take her to a psychiatrist and get her diagnosed.

Dear lybrate-user, The mood swings are attacks of depression and mania. In between, he may be normal. Now he would have had a relapse or he would have got a mood change. Not that medicines don't work. The mood change may not have come to the notice of the psychiatrist. Please take him to the doctor without stopping dicorate 1 g.

Dear Simanta, Since you are already taking drugs for B.P. and panic, it is better to consult the doctors before you take drugs for ED. Sometimes Nexito kind of drugs or the blood vessel changes due to high B.P. may be causing ED. If we find the cause, it can be treated.

Dear lybrate-user, The pimple at your back bone has nothing to do with your smoking. Please consult a doctor, preferably a surgeon, to diagnose fistula or something else. However quitting smoking has many health advantages. You are young. If you continuously motivate yourself for 12 weeks, you will be able to quit smoking. All the best.

Dear lybrate-user, Probably you are suffering from obsessive ruminations. Depression is common with such obsessions. Probably you are on Clofranil (not Chlorofranil) which is the treatment for your condition. Please consult your doctor again so as to increase the dose of Clofranil or add another drug or Cognitive Behaviour Therapy (CBT). The treatment may last for several months and repeated visits to psychiatrist.

Dear lybrate-user, It is sad that Anand smokes, drinks, overweight and no exercise. All these are risk factors for heart problems. I don't know his age. However at any age if his life style changes, his health can be maintained. His liver is compromised since AST, ALT are above normal. GGT is 3 times increased. The liver is affected more with alcohol than any other cause. Let him go through these reports with your doctor and make the doctor to advice on healthy life style. Slowly you can increase his motivation towards the change. Psychiatrists and clinical psychologist can help in motivation enhancement therapy - 12 weekly sessions.

Dear lybrate-user, If you want to leave smoking and drinking, you can do it. Please motivate yourself continuously for at least 12 weeks. Appreciate yourself for managing one day at a time. If you have any withdrawal symptoms or craving, please seek help. They are tablets to manage them. After stopping both smoking and drinking, please see whether your sex power is increased, If not consult a sexologist.

Dear lybrate-user, Rabies is a serious disease. You are living for 5 years means, you don't have rabies. You need not test or take treatment for rabies. But you may need treatment for rabies fear (disease phobia) and depression. Please consult a psychiatrist or clinical psychologist.

Dear Sir/Madam, I want help for my mother. She is 62 years old and suffering from some mental tension for past 7-8 years. The problem she is facing is that she never remains happy with anyone at home whether it is me or my father or my wife. She always remain angry and also she does not take proper diet as well. She hardly eats 1 meal in the day and does not takes interest in other activities as well like travelling, watching movies, going for shopping, she does not like anything. Despite of so much attention given to her she always feels and remain silent, quite and angry. Most of the time she does not even reply to us. All this was there from last 7-8 years but from last 2 years or so it got further deteriorated that was after we lost her brother (my uncle or mama). But on the contrary she does not act in the same way with the outsiders, I mean she is fine with neighbors and other relatives, her rude behavior is just for us her own people who loves her very much. Please advice what I shall do in order to help her from this situation. I have asked her to visit a doctor for this problem but she always refuses. Lastly please advice me which doctor can help me I.e a psychologist or a psychiatrist as I do not know the difference between both. Thanks and Regards

You can also consult a clinical psychologist who will spend more time with your mother and treat with therapy. It depends on the comfort level of your mother with the therapist, the distance, fees, availability, etc. You may need to visit the therapist frequently to find improvement. Remember these mental tensions are treatable just like any other illnesses. If she is refusing, please contact the therapist yourself. You will learn how to deal with her refusal. All the best.

Dear Nirvic, i don't know how long you have been smoking. However yo try and stop smoking. Breathing problems are common in smokers. If yo stop smoking, there are other health benefits also in the long run. Please motivate yourself continuously for 12 weeks not to smoke. If you have withdrawal symptoms, please take the help of nicotine gum or patch, or Bupropion tablets. All the best.

Dear Sunny, Just now you are smoking 1-2/day. You are only 23 years, imagine the amount of nicotine in your system at the age of 65 years. Won't it be harmful? At old age one has to be healthy. Nicotine is very addicting. As days go by you want more and more cigarettes to keep you active. Nicotine gets absorbed into the blood stream by puffing only and it goes to the brain within seconds. So quit smoking now itself. Keep an emotionally significant day as quit day and continue to motivate yourself for 12 weeks at least. Lemon juice or any fluid is useful in washing out the excesses in the body.

Sir I am just 21 years old & was in severe depression for 6 month I have tried my best to get out of it but evrytime I failed .At the last stage of depression my legs got heavy & felt tingling and numbness. I have consulted many neurologist but no result. Finally I consulted a psychiatrist and he has put me medication since 35 days still I am feeling emotionless & tingling in my hand and legs. Doctor said my steronin level got down to zero. I am taking the medicine still I am thinking of suicide as I am not getting emotional pleasure and I have lost interest in everything. After taking the medication somewhat it manage the symptoms I used to get earlier like giving face expression like a joker & feeling self created god .Now also I am smiling without any reason & not getting the views of others. I have attached the prescription report given by my consultant psychiatrist.

Dear lybrate-user, You are in proper hands of a psychiatrist now. You are taking antidepressants like Nexito and Prothiaden. 35 days is too early to expect full results. Please wait and your psychiatrist will change the dose or the drug or add another, if necessary, next time you visit. You need to visit your psychiatrist many times before you feel the full result. Please talk about your suicidal ideas with someone. Inform that person about the drugs, you are prescribed. It will be nice if you could deposit all your tablets with her/him, and take daily.

Dear lybrate-user, Please consult a psychiatrist and get diagnosed. She/he may order few routine and hormone tests, to rule out medical causes for your mood. If they are normal, she/he will treat you for depression, both with tablets and with therapy. With treatment, you will not feel so sad and you can be happy.

Dear Tushant, Congratulations for quitting smoking. You need to motivate yourself for 12 more weeks to continue quitting. Quitting smoking has many health benefits apart from stop shivering and cleaning lungs. Drink lots of liquid and try and do pranayama daily for next 3 months. If you still have shivering, please consult a doctor for diagnosing its cause.
