Piles - Is Constipation The Real Reason?

There are many people worldwide who suffer from a condition known as Piles or Haemorrhoids. In individuals with piles, the veins in the anal canal become inflamed and swollen causing pain and extreme discomfort (anal bleeding after defecation) during the bowel movement. There may be anal itching along with a mucous discharge from the anus. While in some people piles or hemorrhoids may appear within the inner part of the anal canal (Internal Piles), in others they may be present on the outer side of the rectum (External Piles).

Is constipation the main cause?
One of the underlying factors that trigger piles is chronic constipation. In constipation, the stool becomes hard, dry, and large. There is straining during the bowel movement. All these exert immense pressure on the rectum resulting in the blood vessels present around the rectum and the anus to undergo swelling and inflammation giving rise to hemorrhoids or piles.

In addition to constipation, the other factors that may cause piles include

  1. Severe diarrhea
  2. Pregnancy
  3. Lack of physical activity or a sedentary lifestyle
  4. Obesity can also act as a trigger
  5. In some people, piles may be a genetic predisposition

Ayurveda for piles
It is a well-known fact that constipation acts as a catalyst resulting in piles. Ayurveda works towards not only treating piles but also eliminating the underlying factors. As per Ayurveda, constipation occurs when there is a dip in the digestive fire. There is a disruption of the balance between the doshas (Vata, Kapha, and Pitta). As a result, toxins (Ama) build up in the body (especially in the rectum resulting in constipation). The disruption of the dosha balance can be brought about by a plethora of factors including lack of physical activity, unhealthy diet, and even obesity. Thus, the first step towards treating piles involves the elimination of the Ama from the body so that the balance between the three doshas can be restored.

  • Jimikand and Haritaki are wonder herbs that play a pivotal role in dealing with constipation and piles by improving the overall functioning of the digestive system.
  • The diet also undergoes healthy modifications. Consume fruits and vegetables such as mangoes, dried figs, papaya, bitter gourd, radish, onion, ginger, green leafy vegetables, which act as a tonic in the case of piles and constipation. Whole grains, legumes, brown rice are healthy options. However, processed and packaged foods, coffee, tea, aerated drinks, and beverages, colocasia, potato, should be avoided.
  • Drink lots of water. A well-hydrated body will be more effective in dealing with constipation and also eliminate the toxins from the body.
  • Physical activities, yoga, and exercise can go a long way to improve the condition of piles as well as constipation.
  • Avoid having cold or stale foods.
  • Oil massage using a specially prepared herbal oil can play a significant role in the detoxification of the body.
  • Panchakarma (especially Basti where the toxins accumulated in the lower colon are eliminated using an Ayurvedic enema) can also help in the detoxification of the body thereby improving the condition of constipation and piles.

Pilonidal Sinus - How Ayurveda Can Help Treat It?

Pilonidal refers to a nest of hair. A pilonidal sinus or PNS is an abscess or a small cyst that is often witnessed at the top left-hand corner of the buttocks. It is a tube-like structure that possesses a tuft of hair. PNS consists of debris, hair, and dirt resulting from infection leading to severe pain. In case PNS becomes infected, it can ooze blood and pus along with a foul smell. This condition is often witnessed in people who are required to sit for a very long time at one go. A truck driver for instance often suffers from this condition.

What are the symptoms?
There are no visible symptoms during the initial phase of this condition. There could be a small depression in the skin that can develop into a cyst. Some of the notable signs of this disease include pain while sitting, sore skin, reddening of the area, a cyst swelling to a considerable extent, formation of multiple cysts. At times, mild fever can also be present along with the other symptoms.

What are the possible causes of pilonidal sinus?
Although the exact cause of PNS is not known, researchers believe that it happens due to a combination of friction of clothes, changing hormones, a growth of hair inside the skin etc. Apart from this, some of the other reasons include over sweating, hair twisting inside the skin, lack of hygiene in the sacral region, improper sitting posture etc.

How Ayurveda can treat PNS?
Ayurveda treats pilonidal sinus with the help of Kshra sutra treatment. This treatment is believed to be the best replacement to modern medicine and can be an alternative to the complex surgeries and pain. Here is how the treatment proceeds:

  1. The patient is kept in a prone position
  2. With proper anaesthesia and an aseptic measure in place, the track is identified with a probe
  3. The track is then cleaned and a herbal medicated thread (Ayurveda calls this Kshara sutra) is placed and temporarily fixed onto that place
  4. Turmeric is a key ingredient of this herbal mixture. It has been scientifically proved that turmeric has tremendous healing power.
  5. The thread produces a hygroscopic action due to the presence of proteolytic enzymes, this causes the debris to come out of the cyst
  6. Kshara Sutra has the capability to cut through the cyst and simultaneously heal it as well
  7. It is necessary to change the Kshara Sutra every week to speed up the healing process

With every visit, the length of the track and the Kshara Sutra that was inserted keeps decreasing. Finally, the track gets completely cut through with the impact of the sutra and the disease gets cured from within by shedding debris.

An anorectal abscess can be a combination of various conditions such as a rectal abscess, perianal abscess, anal abscess etc. It is a condition where pus and stool ooze out from areas of the rectum and the anus. Some of the possible causes of an anorectal abscess include infection in the anal area, sexually transmitted infections and blocked glands in the anal region. Disorders such as diverticulitis and Crohn’s disease can also lead to a deep rectal abscess.

What are the risk factors for this disease?
Some of the common risk factors of this condition include sexual activities involving the anal region, certain medications such as chemotherapy drugs, uncontrollable diabetes, a weak immune system due to conditions such as AIDs and HIV. An anorectal abscess can also be witnessed in toddlers if there exists a family history of this disease.

What are the common symptoms of this condition?
Some of the common symptoms of this condition include

  1. Frequent constipation
  2. Discharge of pus from the anal region
  3. Nausea and fatigue
  4. General malaise
  5. Formation of a nodule or lump in the rectum area,
  6. Night sweats
  7. Hardened tissue

Diagnosis and complications:
A rectal exam conducted by the doctor can reveal the condition. In case of any doubt, a doctor often prescribes a proctosigmoidoscopy to rule out other conditions. The doctor might still prescribe tests such as MRI scan, CT scan, ultrasound etc. to get a detailed picture of the area where pus is getting collected near the rectal region. Complications of an anorectal abscess includes scars, anal fistula, constant pain, sepsis and recurrence of pus.

What are the treatment options available in Ayurveda?

  1. Curcumin: Curcumin is the pigment extracted from turmeric. This pigment is considered to be the most powerful naturally occurring anti-inflammatory agent. Its medicinal effect can cure many diseases. For patients suffering from an anorectal abscess, curcumin can control pain and inflammation in the anal region. Furthermore, it ensures that the condition does not aggravate to the next stage.
  2. Triphala: Triphala is a combination of three herbs- harara, bahera and amla. These three herbs should be consumed in a powdery form twice a day to clean the wound and balance the doshas. Triphala can speed up the process of recovery for patients suffering from an anorectal abscess.
  3. Guggul: Guggul is a gum resin that is extracted from the tree known as the Commiphora Mukul. It is used to treat a wide range of medical conditions. Although it seldom cures the condition alone, it cures many ailments when used in union with Triphala and other herbs.
  4. Neem: Neem leaves have many medicinal properties. It is anti-inflammatory in nature and its application in the powdery/paste form can pay rich dividends in curing an anorectal abscess. Furthermore, neem leaves when boiled and applied near the rectal region helps to ease the muscles and remove the blocked pus in the rectum.

Anal Fissures - Different Ayurvedic Remedies That Can Help!

Many people dread going to the toilet, all thanks to the extreme pain, bleeding (not always though), or itchiness that one experiences during the bowel movement. These are some of the discomfort and complications associated with a condition referred to as Fissure-in-Ano (also known as Anal fissures). Age or sex has no impact in triggering anal fissures as the condition can affect all and sundry (though infants are more susceptible).

Medically, anal fissures are nothing but cuts or cracks (can be chronic or acute longitudinal tears that mainly develop in the anoderm or the epithelial lining of the anal canal) that affects the anal as well as an anal canal (terminal end of the gastrointestinal tract). Without proper treatment and medication, the condition will only deteriorate further piling on the agony and discomfort.

What triggers Anal Fissures?

  • In the majority of the cases, the anal fissures, (that mostly appears in the midline posteriorly of the anal canal), are triggered by Constipation or passing of hard stools that cause repeated wear and tear of the anal mucosa resulting in the cracks or fissures.
  • Crohn’s Disease (an Inflammatory Bowel Disease that often results in the digestive tract getting inflamed) can play a significant role in the formation of fissures.
  • Anal fissures can also result from childbirth, severe diarrhea, HIV, Tuberculosis, anal cancer, to name a few.
  • Anal sex can, at times, play a pivotal role in triggering anal fissures.

Symptoms
A person with anal fissures complain of

  1. Pain and discomfort while passing stool. At times, there may be rectal bleeding as well.
  2. A skin tag often appears on the skin close to the fissure.
  3. The skin around the anus appears cracked with irritation and itching.


Ayurveda and Anal Fissures
Many patients with anal fissures, even severe and chronic cases, have been completely cured and treated (ensuring there is no recurrence of the condition) with Ayurveda.

  1. With constipation being a trigger for anal fissures, Ayurvedic medicines are given to soften the stool and also to treat constipation. In this regard, Triphala deserves a special mention. It works magically to provide relief from constipation and the associated discomfort that comes tagged along.
  2. The use of Panchakarma can also work wonders to treat anal fissures. Panchakarma involves the use of various herbal oil baths and massages to cleanse the body of the harmful toxins. Anal fissures can be treated using Basti (a part of Panchakarma therapy that eliminates the toxins present in the lower colon using an Ayurvedic enema such as milk, ghee).
  3. The diet should also be taken care off. Increase the intake of dietary fibers (citrus fruits, prunes, whole grains, beans, nuts, seeds, Brussels Sprouts, peas, to name a few).
  4. Make it a habit to indulge in exercise and physical activities daily.
  5. Increase the fluid intake (water, fresh fruit juice, vegetable soup) to keep the body hydrated.

Note: For maximum results, the dosage of the medicine (Triphala or any other Ayurvedic medicines) should be as per the doctor recommended.

Take Rich fiber diet in food like leaf vegetables. Take Erandabrushta Harde tablet 4 at bed time. So you will have soft stool or consult me for the full course of medicine.

Fistula-in-Ano - Understanding The Ayurvedic Perspective On It!

Ayurveda takes a holistic approach to treat any disease. It aims to treat fistula-in-ano with a combination of herbs, good lifestyle, healthy diet etc. Ayurveda suggests that fistula occurs due to the disparity of the 3 doshas. The treatment aims at balancing the doshas and healing the condition from within without any side effects.

What is anal fistula?
Anal fistula is an opening other than the normal opening of the anus. Most people face oozing of stool and pus from the other opening. The surrounding area of the anus becomes painful and tender. If the opening closes for a few days, the pus gets trapped leading to fever and abscess. It is, therefore, important to get rid of the mucus and the pus completely and heal the fistula. In case of multiple openings, modern medicine suggests surgery to get rid of the situation. Ayurveda on the other hand, uses oils, herbs, and other such non-inflammatory means to cure the condition.

Digestive fire from the Ayurvedic viewpoint:
Digestion only happens smoothly when the Jatharagni/Agni is strong. With natural food and supplements and herbs it is entirely possible to balance the digestive fire. Raw fruits, vegetables, and nuts are the ideal diet to heal this condition. Some foods to avoid to ensure that the aggravation of fistula doesn’t take place include aerated drinks, processed foods, milk and milk related products. The herbs that are prescribed by the Ayurvedic practitioner should be continued for at least 3-6 months.

Ayurvedic treatment for fistula-inano:

Ayurveda has a very precise dosage of herbs mentioned in the ancient books. It is suggested that two tablespoons of Triphala Guggul and 1 tablespoon of Vara Churna should be consumed daily, twice a day, Nirgundi oil should be applied locally every day and 2 tablespoons of curcumin should be consumed daily.

  1. Triphala Guggul: This formulation consists of two very important herbs- Guggul and Triphala. Both these herbs helps the body’s defence mechanism. This formulation is anti-inflammatory and anti-bacterial in nature. Both the herbs help in toning the gastrointestinal tract, streamline the flow of bile and aids the intestine to effectively eradicate bodily waste.
  2. Vara Churna: Vara Churna is a formulation that contains several herbs such as amalaki, bibhitaka, haritaki etc. This formulation helps clean the colon and strengthen the GIT. This formulation is effective for vata, kapha, and pitta. Vara Churna is extremely effective for rectifying constipation, aiding blood circulation improving liver function and balancing the blood pressure.
  3. Nirgundi Oil: This oil is made up from the herb Nigundo. This herb is pungent, bitter and astringent in flavour. It is a natural antibiotic and is an anti-inflammatory. This oil can relax the muscles, reduce congestion and pain.

I had loose motion few days ago. My motion was watery but now I am fine. I don't have any problem regarding motion. But when I go to the loo, after defecation my anus pains a lot. There is irritation, and pain which is unbearable. Please suggest home remedy.

You might be suffering from Fissure. Take Sitz bath twice daily. Apply Ghee in the anus it will heal your Fissure and relief somewhat in pain. Drink lukewarm water 2 glass at night.
