Hi lybrate-user, I am sorry to know about your stone problem. Since the stone is causing hydronephrosis, I think it is in the ureter (the tube connecting kidney to urinary bladder). Any hydronephrosis lasting more than 4 weeks can cause permanent damage to the kidney. If the pain and hydronephrosis have been there for only a few days to weeks, you can try medical expulsive therapy using alpha-blockers like Tamsulosin. If the ultrasound shows features of kidney damage or if the stone is in upper ureter, then it is better to remove the stone endoscopically as soon as possible. Please visit your local Urologist urgently.

I think you mean that the 5 mm stone is in right ureter. Stones in ureter are the most painful ones. Stones inside kidneys cause pain only when they grow larger. Anti-inflammatory medications (NSAIDs) are the best medicines for ureteric colic (pain due to ureteric stones). However, these are harmful to kidneys if taken for a long time. Another important group of medicines are alpha-bockers. These will dilate the ureter and expedite stone passage. Both these have to be used carefully after assessment of kidney function, allergies and other co-morbidities. However, these will give you complete pain relief.

Please understand that night fall is not a disease. It has been made into a disease by false advertisements by many quacks in India. Semen is continuously produced by the body and once in a while it is ejaculated during sleep. It does not cause any harm to the body.

This condition is called stress urinary incontinence. Your mother needs to be assessed by a urologist for any associated prolapse of urethra or bladder. Treatment in early stages is pelvic floor strengthening exercises (kegel's exercises) and medications. If severe, it can be cured by a minor procedure called transobturator tension free vaginal tape.

Ultrasound and ct scan cannot diagnose glomerulonephritis. Only a kidney biopsy done using a needle under ultrasound guidance can detect glomerulonephritis. Only your treating nephrologist can decide whether you require a biopsy or not.

Hi, you are probably having tight foreskin (phimosis). Please get yourself examined by a urologist. If it is left untreated, it can lead to skin infection, urinary infection or very rarely even penile cancer. In early stages it can be treated with local ointments. If severe, it may need a small surgical correction.

My father is suffering from renal kidney failure and the creatine level is about 5.8 and some doctors have suggested that he had to transplant the kidney so should we go for transplant or can be cured with medicine's.

Felllow in Endourology, MCh Urology, MRCS, MS, MBBS
Urologist,
My father is suffering from renal kidney failure and the creatine level is about 5.8 and some doctors have suggested ...
There are two types of kidney failure - one is acute, another is chronic. In acute kidney failure, the chances of recovery are very good. In chronic kidney disease, the kidney's structure gets permanently damaged. So far, there is no medicine which can regrow a permanently damaged kidney. The current medicines only reduce further damage. Your father probably has got chronic kidney disease. Kidney transplantation is the best solution for chronic kidney disease stage 5, provided the patient is fit for the procedure. However, you need to understand all the pros and cons before undergoing the procedure. At creatinine 5.8, it is not urgent to get a kidney transplantation done. There are a lot of other factors which need to be considered. It is best to meet another nephrologist in person and discuss the options of treatment.

The damaged kidney is not routinely removed. However, if it has got infection, tumour, obstruction, large stones, or if it is creating problems like protein loss, occupying a lot of space, causing hypertension etc then it is removed. The transplanted kidney is usually placed in right or left lower abdomen.

Whether aged persons should restrict the water intake to the minimum required level to reduce the load to the kidney?

Felllow in Endourology, MCh Urology, MRCS, MS, MBBS
Urologist,
Whether aged persons should restrict the water intake to the minimum required level to reduce the load to the kidney?
Water intake should be decided by your thirst level. Brain knows how much water it needs. There are mechanisms to detect how much is the water level (osmolality) in the blood and indicate you by means of thirst. Unless you are taking some drugs that can impair this ability, any kidney disease, heart problems, or brain disorders, you can take a normal 2 to 3 liters of fluid per day. You can reduce fluid intake in evening and night to prevent unnecessary sleep disturbance.

Hi, the best treatment for a 22 mm stone is complete removal through a keyhole. This surgery is called mini- pcnl (percutaneous nephrolithotomy). Laparoscopy is not preferred for kidney stones, except in rare situations. Currently ct urography is the preferred imaging test before stone removal. Ivp is an older and limited imaging option. It is to be done only in places where ct is not available.

Can drinking water with lemon prevent kidney stones? Also, I have been told not to drink city water. What is the best...
Hi, lemon has a high potassium and citrate content. Both these help in dissolving existing stone and preventing further stone formation. The amount of water is more important than the type of water. In general avoid ro demineralized water. Natural minerals are essential for normal body and kidney function. Reverse osmosis removes most of the minerals from water. Aim to keep urine colour very pale throughout day and night. Keep urine output at least 2.5 litres per day. Avoid drinking a lot of water at one time and not drinking for many hours. Uniform and continuous hydration is important, even at night. Apart from good water intake, daily aerobic exercises, reduction in salt and animal protein intake and adequate calcium intake (reduced calcium intake can increase stone risk due to elevated oxalate) are important to prevent kidney stones. Wish you good health.

Urologist,
I m faiz ahmed age 34. I have few 2mm stone in my left kidney and 3mm in vuj left. Please advice me.
Hi, all these are small stones which can be effectively treated with medicines. Vuj is the junction between ureter and bladder. This stone is treated with a group of drugs called alpha blockers and the kidney stones can be treated with drugs to reduce calcium and uric acid and increase potassium & citrate in urine. Please get prescription from your local urologist. Wish you a good health

Hi, I am sorry to hear about your problem. Usually ureteric stones (stones in tube connecting kidney with bladder) are dangerous to kidneys, since they block the urine drainage from kidney and cause swelling of kidney (hydronephrosis). Most of the ureteric stones less than 7 mm will pass easily with medicines. Since your stone size is 9.2 mm, it is a little difficult to manage with medicines. However, medicines can be tried for a few weeks if the kidney function is good and swelling is not much. Only your treating urologist can decide on that. If it is causing damage to your kidney, then it needs to be removed using ureteroscopy (urs). Any ureteric stone if left untreated for more than 4 weeks will cause permanent damage to the kidney. Moreover, it can also damage the ureter (leading to ureteric stricture) at the place where it gets blocked. Usually small kidney stones less than 5 mm can be treated medically. Larger stones can get dissolved depending on their composition. Uric acid stones are easy to treat medically. After the treatment of ureteric stone, you can get treated for kidney stones using medicines. If they are not getting dissolved in 3 months, you can undergo flexible ureteroscopy with laser for complete stone removal. Wish you a good health,

Hello sir I want to ask about crestine level increase problem my coligue child have a series problem of this Creatine increase level due to this he is on dialysis Section please suggest any cure or suggestion to recover this problem.

Felllow in Endourology, MCh Urology, MRCS, MS, MBBS
Urologist,
Hi, high creatinine is suggestive of poor kidney function. Since the child is on dialysis, most probably he is having kidney failure. There are two types: acute or chronic. If it is an acute kidney failure, the treating nephrologist will be able to make the kidney function again with his treatment. In case the kidney is permanently damaged, kidney transplantation is the best option.

Hi, please get a urine microscopy test done. If it is normal, just take lot of fluids and lemon juice. If it shows any abnormality, get a ultrasound of kidney and bladder region. These tests will rule out any stone or urinary infection which are the common causes of these symptoms. If both these tests are normal and still you continue to have these symptoms, please consult a urologist. Wish you a good health,

Hi, we do not get urine infection from food. Urine infection occurs due to bacteria which grow in genital region and enter the bladder through urinary passage. The following measures will help you to reduce infection. 1. Drink plenty of fluids and pass urine every 3 to 4 hours. 2. Always wipe or clean from front to back. Bacteria from anal region should not enter the vaginal region. 3. Take vitamin c regularly. It acidifies urine and reduces bacterial growth. 4. Take cranberry juice, apple and lemon regularly. 5. Avoid douches 6. Avoid bathtubs 7. Use cotton underwear and loose clothes which reduce moisture locally. Moisture favors bacterial growth. 8. Use estrogen vaginal cream every night (after consultation with your gynecologist or urologist). 9. Pass urine after having intercourse. Bacteria normally enters during intercourse. They get washed away while passing urine. 10. Take yogurt or curd regularly. This can increase the health bacteria like lactobacilli in vaginal region. This will prevent growth of harmful bacteria. If you still get recurrent infections, please get yourself examined by a urologist. Problems like narrow urinary passage and prolapse of bladder can also cause urine infection. Urologist can identify and correct these problems. Get your sugar level checked regularly to rule out diabetes.
